Pseudophakic Dysphotopsia: Review of Incidence, Cause, and Treatment of Positive and Negative Dysphotopsia.

We reviewed the literature concerning positive dysphotopsia (PD) and negative dysphotopsia (ND) regarding cause, incidence, and clinical and surgical management. In addition, we summarized our surgical experience in managing dysphotopsia. A PubMed review, limited to English language articles, yielded 149 citations; multifocal (diffractive optic) and phakic intraocular lens (IOL) dysphotopsia were excluded. Overall, 39 articles were determined to be relevant for the objectives of this investigation. Regarding PD, 7 articles corroborated that the cause of PD is related primarily to internal refl

Long-term visual outcomes and rotational stability of two diffractive toric intraocular lenses with different design combinations in cataract patients with long axial length.

BACKGROUND: Selecting intraocular lenses (IOLs) for cataract patients with long axial length remains challenging. Two diffractive toric IOLs with distinct design combinations-differing in material (hydrophobic vs. hydrophilic acrylic), optical design (trifocal vs. bifocal), haptic configuration (C-loop vs. plate-haptic), and IOL color (yellow-tinted vs. clear)-represent commonly used options. This study compared their long-term visual outcomes and rotational stability. Notably, this study was not designed to isolate individual contributions of material or IOL color, as these factors covary; po

Surgical Anatomy of the Rectus-Sparing Approach for Periacetabular Osteotomy: A Cadaveric Study.

BACKGROUND: The Bernese periacetabular osteotomy (PAO) is a widely used technique for the management of acetabular dysplasia and other hip deformities in adolescents and young adults. Originally, the approach was described with a release of both origins of the rectus femoris muscle1. In the more recently described rectus-sparing approach, both heads remain attached2,3. It has been proposed that this modification may decrease pain, ease postoperative rehabilitation, and avoid heterotopic ossifications, without limitations of the surgical overview. DESCRIPTION: Both the original and the rectus-

Effect of Acetabular Labral Tear Orientation on Hip Joint Kinematics: A Comparison of Radial Tears, Chondrolabral Junction Tears and Complex Tears in Cadaveric Hips.

BACKGROUND: Acetabular labral tear morphology or orientation may influence hip stability. HYPOTHESIS: A radial tear of the acetabular labrum would result in greater rotational and translational motion compared with a chondrolabral separation. STUDY DESIGN: Controlled laboratory study. METHODS: Included were 12 unpaired nonarthritic hip specimens, none of which had capsular laxity (8 male; mean age, 34.5 years). The specimens were stripped of all soft tissue except the hip capsule and labrum and, then potted using a custom jig. In 6 specimens, a 1-cm anterosuperior separation of the labrum f

Iris defect management in the context of presbyopia-correcting intraocular lenses.

A 65-year-old man had uneventful cataract surgery in the right eye with a toric diffractive intraocular lens (IOL) placed fully within the capsule bag. On postoperative day 1 and week 1, the IOL was well positioned and his eye was healing normally. The plan was to proceed with cataract surgery in the left eye in the near future. One month postoperatively, he presented with blurred vision, glare, and halos and was noted to have iris prolapse out of the temporal clear corneal main incision. Of interest, the patient reported some itching and eye rubbing in the early postoperative period. He was t

An experimental assessment of the risk of compartment syndrome during knee arthroscopy.

The purpose of this study was to objectively evaluate the risk of compartment syndrome as a complication during arthroscopy, particularly with the use of mechanical infusion systems. Little experimental data are available. This study shows that when elevated compartment pressures occur, extravasated fluid dissipates quickly, minimizing the risk of compartment syndrome and subsequent neuromuscular damage. Twelve live pig hind limbs (six swine) were used. Three additional limbs were used as shams. After anesthesia and portal placement for mechanical fluid infusion, two standardized capsulotomies

MyoD is required for the differentiation of Myo/Nog cell progenitors of myofibroblasts in explants of human lens tissue.

INTRODUCTION: Posterior capsule opacification (PCO) is a complication of cataract surgery that impairs vision. Clouding and distortion of the posterior capsule occur as a result of cell migration, deposition of extracellular matrix proteins and contractions of myofibroblasts. The focus of this study is a subpopulation of cells within the lens, called Myo/Nog cells, that differentiate into myofibroblasts in response to wounding. Myo/Nog cells express the skeletal muscle specific transcription factor MyoD, bone morphogenetic protein inhibitor Noggin and brain-specific angiogenesis inhibitor (BAI

Laser-assisted cataract surgery versus standard ultrasound phacoemulsification cataract surgery.

BACKGROUND: Cataract is the leading cause of blindness in the world, and cataract surgery is one of the most commonly performed operations in the Western world. Preferred surgical techniques have changed dramatically over the past half century with associated improvements in outcomes and safety. Femtosecond laser platforms that can accurately and reproducibly perform key steps in cataract surgery, including corneal incisions, capsulotomy and lens fragmentation, are now available. The potential advantages of laser-assisted surgery are broad, and include greater safety and better visual outcomes

Laser-assisted cataract surgery versus standard ultrasound phacoemulsification cataract surgery.

BACKGROUND: Cataract is the leading cause of blindness in the world and, as such, cataract surgery is one of the most commonly performed operations globally. Surgical techniques have changed dramatically over the past half century with associated improvements in outcomes and safety. Femtosecond lasers can be used to perform the key steps in cataract surgery, such as corneal incisions, lens capsulotomy and fragmentation. The potential advantage of femtosecond laser-assisted cataract surgery (FLACS) is greater precision and reproducibility of these steps compared to manual techniques. The disadv

Ocular complications with the use of radium-223: a case series.

BackgroundRadium-223 is used for the treatment of osseous metastases in castrate-resistant prostate cancer, and has been shown to increase time to the first skeletal-related event, reduce the rate of hospitalization, and improve quality of life. It is well tolerated, with hematologic toxicity as the main adverse event. Thus far, no ocular complication has been reported in the literature after initial administration of radium-223 with a single case reported of ocular complications after a patient's second course of radium-223.Case presentationsWe present three cases of ocular complications afte

Trifocal intraocular lenses versus bifocal intraocular lenses after cataract extraction among participants with presbyopia.

BACKGROUND: Presbyopia occurs when the lens of the eyes loses its elasticity leading to loss of accommodation. The lens may also progress to develop cataract, affecting visual acuity and contrast sensitivity. One option of care for individuals with presbyopia and cataract is the use of multifocal or extended depth of focus intraocular lens (IOL) after cataract surgery. Although trifocal and bifocal IOLs are designed to restore three and two focal points respectively, trifocal lens may be preferable because it restores near, intermediate, and far vision, and may also provide a greater range of
