[Postural tachycardia syndrome (PoTS): An up-to-date].

Postural tachycardia syndrome (PoTS) is a multifactorial syndrome defined by an increase in heart rate ≥30bpm, within 10minutes of standing (or during a head up tilt test to at least 60°), in absence of orthostatic hypotension. It is associated with symptoms of cerebral hypoperfusion that are worse when upright and improve in supine position. Patients have an intense fatigue with a high incidence on quality of life. This syndrome can be explained by many pathophysiological mechanisms. It can be associated with Ehlers-Danlos disease and some autoimmune disorders. The treatment is based on nonph

Postural Tremor and Ataxia Progression in Spinocerebellar Ataxias.

BACKGROUND: Postural tremor can sometimes occur in spinocerebellar ataxias (SCAs). However, the prevalence and clinical characteristics of postural tremor in SCAs are poorly understood, and whether SCA patients with postural tremor have different ataxia progression is not known. METHODS: We studied postural tremor in 315 patients with SCA1, 2, 3, and 6 recruited from the Clinical Research Consortium for Spinocerebellar Ataxias (CRC-SCA), which consists of 12 participating centers in the United States, and we evaluated ataxia progression in these patients from January 2010 to August 2012. Assessment of the Postural Stability of Female and Male Athletes.

OBJECTIVE: Postural stability is often affected by sport-related injuries and subsequently evaluated during postinjury examinations. Intrinsic factors, however, may also affect postural control. We sought to compare the postural control of female and male athletes as measured simultaneously by (1) the modified balance error scoring system (mBESS) and (2) a video-force plate system. DESIGN: Cross-sectional study. SETTING: Sports injury prevention center. PARTICIPANTS: Pediatric, adolescent, and young adult athletes who performed mBESS during an injury prevention evaluation. INDEPENDENT VARI

Ankle intrinsic stiffness is modulated by postural sway.

Dynamic ankle stiffness, which defines the relationship between joint position and the torque acting about it, determines the joint resistance to external perturbations. The contribution of joint stiffness to stance control is usually assumed to be constant; however, the stiffness may be modulated due to the ongoing sway during stance. This paper describes the results of the direct estimation of ankle intrinsic stiffness and its modulation with background torque during stance. Three subjects stood on a standing apparatus, while subjected to pulse perturbations of ankle position. The angle of c

Influence of Obesity on Postural Stability in Young Adults.

OBJECTIVES: The purpose of this study was to determine whether obesity is associated with less postural stability in young adults, and whether it is influenced by anterior pelvic tilt angle and sensory dysfunction. METHODS: Center of gravity (COG) velocity and total sway distance with eyes open or eyes closed on firm or foam floors were determined in 12 obese individuals and 12 individuals with normal weight. RESULTS: On firm and foam floors with eyes closed, center of gravity velocity and total sway distance were significantly greater in the obese group than in the normal-weight group. Howe

Sleep disorders in patients with postural tachycardia syndrome.

OBJECTIVE: Patients with postural tachycardia syndrome (POTS) often describe symptoms of fatigue, sleepiness, and lack of refreshing sleep. We aimed to provide further objective measures of sleep in patients with POTS. METHODS: POTS patients (n = 18) were selected based on autonomic testing and evaluation at our center. Controls (n = 16) of similar age, gender, and BMI were selected from new patients referred to the Stanford Sleep Disorders Clinic for any sleep-related complaint. All patients underwent polysomnography and completed several sleep questionnaires and a 2-week sleep diary. RESUL

Coupling of postural activity with motion of a ship at sea.

On land, body sway during stance becomes coupled with imposed oscillations of the illuminated environment or of the support surface. This coupling appears to have the function of stabilizing the body relative to the illuminated or inertial environment. In previous research, the stimulus has been limited to motion in a single axis. Little is known about our ability to couple postural activity with complex, multi-axis oscillations. On a ship at sea, we evaluated postural activity using measures of body movement, as such, and we separately evaluated a direct measure of coupling between body movem

Abnormal postural reflexes in a patient with pontine ischaemia.

The control of body posture is a complex activity that needs a very close relationship between different structures, such as the vestibular system, and the muscle and joint receptors of the neck. Damage of even one of these structures can lead to abnormal postural reflexes. We describe a case of a woman with a left pontine ischaemia who developed a 'dystonic' extensor posture of the left limbs while turned on the right side. This clinical picture differs from previous reports on the subject, and may relate to ischaemic damage of a pontine structure involved in posture control, or of adjacent n…

Neuronal and hormonal perturbations in postural tachycardia syndrome.

The Postural Tachycardia Syndrome (POTS) is the most common disorder seen in autonomic clinics. Cardinal hemodynamic feature of this chronic and debilitating disorder of orthostatic tolerance is an exaggerated orthostatic tachycardia (≥30 bpm increase in HR with standing) in the absence of orthostatic hypotension. There are multiple pathophysiological mechanisms that underlie POTS. Some patients with POTS have evidence of elevated sympathoneural tone. This hyperadrenergic state is likely a driver of the excessive orthostatic tachycardia. Another common pathophysiological mechanism in POTS is a

Ice skating promotes postural control in children.

High fall rates causing injury and enormous financial costs are reported for children. However, only few studies investigated the effects of balance training in children and these studies did not find enhanced balance performance in postural (transfer) tests. Consequently, it was previously speculated that classical balance training might not be stimulating enough for children to adequately perform these exercises. Therefore, the aim of this study is to evaluate the influence of ice skating as an alternative form of balance training. Volunteers of an intervention (n = 17; INT: 13.1 ± 0.4 years

[Pyridostigmine in the treatment of postural orthostatic tachycardia syndrome].

A 34-year-old female patient was admitted with the complaints of inability to stand upright, palpitations, dizziness, and fatigue in the upright posture for the last one year. She was found to stand upright for less than one minute without symptoms. Tilt table testing showed that, compared to baseline her heart rate increased 55 beats/min in the fifth minute of the test with the symptoms of palpitations, fatigue and sweating without any significant change in her blood pressure. Postural orthostatic tachycardia syndrome was diagnosed, and pyridostigmine treatment was started. Four months after …

A reversible cause of skin hyperpigmentation and postural hypotension.

Vitamin B12 deficiency results in neuropsychiatric, hematologic, gynecologic, cardiovascular, and cutaneous manifestations. It is seen most commonly in the elderly, malabsorption diseases  (>60% of all cases), vegans, and vegetarians. Manifestations of pernicious anemia may be similar to Addison disease and may lead to a misdiagnosis. Herein, we report two cases of vitamin B12 deficiency in which clinical features shared many similarities with Addison disease. Both patients presented with progressive darkening of hands and postural hypotension that reversed with replenishment of vitamin B12. V

Biological determinants of postural disorders in elderly women.

Postural control impairments and dizziness, which are major health problems with high secondary morbidity and mortality, increase with aging. Elevated homocysteine (Hcy) level is an age-related metabolic disorder, known to be involved in cardiovascular, neurological, and multisensory dysfunctions. Elevated Hcy level might be involved in sensory balance control systems impairment and dizziness occurrence. Dizziness, fitness Instrumental Activity of Daily Living scale (fitness IADL), systolic arterial pressure with ankle-brachial blood pressure index and homocysteinemia were studied in 61 nonins

Postural tachycardia syndrome: a heterogeneous and multifactorial disorder.

Postural tachycardia syndrome (POTS) is defined by a heart rate increment of 30 beats/min or more within 10 minutes of standing or head-up tilt in the absence of orthostatic hypotension; the standing heart rate is often 120 beats/min or higher. POTS manifests with symptoms of cerebral hypoperfusion and excessive sympathoexcitation. The pathophysiology of POTS is heterogeneous and includes impaired sympathetically mediated vasoconstriction, excessive sympathetic drive, volume dysregulation, and deconditioning. POTS is frequently included in the differential diagnosis of chronic unexplained symp

Chosen postures during specific sitting activities.

This research study analysed the interaction between people's postures and activities while in semi-public/leisure situations and during transportation (journey by train). In addition, the use of small electronic devices received particular emphasis. Video recordings in German trains and photographs in Dutch semi-public spaces were analysed using a variation of Branton and Grayson's (An evaluation of train seats by observation of sitting behaviour. Ergonomics, 10 (1), (1967), 35-51) postural targeting forms and photos. The analysis suggests a significant relationship between most activities an…

Embodied visual perception of distorted finger postures.

Bodily abnormalities in other persons often evoke an uneasy feeling, even disgust. Here, we studied the brain basis of such perceptual salience by presenting static pictures of distorted hand postures to healthy subjects during functional magnetic resonance imaging. Cortical activation sensitive to distorted (vs. natural) finger postures was found-with right-hemispheric dominance-in the primary motor cortex, postcentral somatosensory areas, amygdala, and insula, and bilaterally in the putamen. This activation pattern suggests that the instantaneous "gut feelings" during the observation of bodi…

[Postural tremor as first symptom of Huntington disease].

INTRODUCTION: Initial manifestations of Huntington's disease (HD) are varied and chorea is not always the first motor symptom. CASE REPORT: We report the case of a 44-year-old woman, with a family history of HD, who presented isolated head and upper limbs tremor for 4 years. Genetic testing confirmed the diagnosis of HD and no cause of secondary postural tremor was found. Propanolol was introduced with success. CONCLUSION: This kind of presentation is unusual and has mostly been reported in the juvenile form of HD.

Postural Tachycardia Syndrome associated with peripartum cardiomyopathy.

Here we describe a 26-year-old woman who developed peripartum cardiomyopathy characterized by fatigue, poor exercise tolerance, and palpitations. Initial echocardiography showed global left ventricular dysfunction with an ejection fraction of 40%. Subsequently, the patient developed a resting tachycardia associated with symptomatic postural tachycardia. A diagnosis of Inappropriate Sinus Tachycardia was made based on Holter monitoring studies with an average heart rate of 121 beat per minute (bpm). Standard autonomic evaluation confirmed the diagnosis of Postural Tachycardia Syndrome. The card
