Awareness, Competencies, and Practice Patterns in Tracheobronchomalacia: A Survey of Pulmonologists.

BACKGROUND: Tracheobronchomalacia (TBM) is a disorder of expiratory central airway collapse. TBM is separate from excessive dynamic airway collapse. Historically TBM has lacked a universally accepted definition. No consensus recommendations on evaluation and management exist. We suspect these unresolved issues contribute to deficits in pulmonologists' awareness and management of TBM. METHODS: We created a 20-question survey obtaining information about overall awareness, knowledge base, competencies, and practice patterns in managing TBM. The survey was disseminated via email by American Colle

Dutch guideline for the management of electrolyte disorders--2012 revision.

Electrolyte disorders are common and often challenging in terms of differential diagnosis and appropriate treatment. To facilitate this, the first Dutch guideline was developed in 2005, which focused on hypernatraemia, hyponatraemia, hyperkalaemia, and hypokalaemia. This guideline was recently revised. Here, we summarise the key points of the revised guideline, including the major complications of each electrolyte disorder, differential diagnosis and recommended treatment. In addition to summarising the guideline, the aim of this review is also to provide a practical guide for the clinician an

Second, which migraine characteristics were associated with receiving migraine-specific medication? METHODS: Migraine patients (age ≥ 18 years) who visited the neurology outpatient clinic for the first time were included. Migraine characteristics and pharmacological status were collected retrospectively for each patient fr

A survey of primary care physician practices in the diagnosis and management of women with interstitial cystitis/painful bladder syndrome.

OBJECTIVES: To describe the practice patterns among primary care physicians' (PCPs) managing patients with symptoms suggestive of interstitial cystitis/painful bladder syndrome (IC/PBS). METHODS: We developed a clinical vignette describing a woman with typical IC/PBS symptoms to elicit questions about etiology, management strategies, and familiarity with this syndrome. We mailed the questionnaire to 556 PCPs, including academicians and community physicians, in Boston, Los Angeles, and Chicago. RESULTS: We received 290 completed questionnaires (response rate, 52%). Nineteen percent of respond

Evidence-based treatment of acute infective conjunctivitis: Breaking the cycle of antibiotic prescribing.

OBJECTIVE: To discover the best treatments for acute infective conjunctivitis and to discern whether antibiotics are necessary for the resolution of bacterial conjunctivitis in particular. QUALITY OF EVIDENCE: MEDLINE, EMBASE, and the Cochrane Database of Systematic reviews were searched. Findings were limited to full-text articles from core clinical journals in the English language, and are based on level I or level II evidence. Clinical Evidence was also searched, from which moderate-quality results have been cited. MAIN MESSAGE: Infective conjunctivitis should be managed conservatively, w

[Summary of the practice guideline 'Viral hepatitis and other liver diseases' (second revision) from the Dutch College of General Practitioners].

The revised Dutch College of General Practitioners' practice guideline 'Viral hepatitis and other liver diseases' offers advice in the diagnosis and management of viral hepatitis A, B and C and other liver diseases. The guideline is important for general practitioners as well as specialists in internal medicine and gastroenterology. The emphasis is on the management of chronic hepatitis B en C, because the prevalence of these diseases has increased in the Netherlands and, in addition, the treatment options for chronic hepatitis have improved. Consequently, timely recognition and adequate refer

Antimicrobial prescription patterns for common acute infections in some rural & urban health facilities of India.

BACKGROUND & OBJECTIVES: Irrational use of antimicrobials is a key factor behind rapidly spreading antimicrobial resistance in microorganisms. This study was undertaken to determine the rate and pattern of antimicrobial prescribing in patients with uncomplicated acute respiratory infections, fever and diarrhoea attending a few rural and urban health settings. METHODS: The study was done in primary and secondary health care facilities of public/government and private settings at four sites in India. Patients with fever, cough, diarrhoea or ear, nose or throat infections of < 7 days were includ

[Anxiety disorders. Causes, clinical picture and treatment].

Anxiety disorders are among the most frequent mental disorders together with affective disorders with a lifetime prevalence of 18%. The pathogenesis of these disorders is complex and includes biological and psychosocial factors. Modern diagnostic classification systems (ICD-10, DSM-IV) differentiate between panic disorder with or without agoraphobia, generalized anxiety disorder, social phobia and specific phobia. Recommended treatment approaches include psychotherapy as well as pharmacotherapy, which have to be preceded by an empathic psychoeducation. Today, cognitive behavioural psychotherap

[Local surgical therapy of venous leg ulcers].

Leg ulcer is a polyaetiologic condition the successful treatment of which is decisively influenced by correct diagnosis of the underlying cause. About 76% of leg ulcer cases are due to chronic venous insufficiency. Nonhealing venous leg ulcers are the domain of local surgical therapy with a variety of symptomatic and causal surgical methods. Shave therapy is the treatment of choice, with relatively low invasiveness and good long-term results. Today subfascial endoscopic perforator vein surgery and the surgical treatment of fascia cruris (fasciotomy, fasciectomy) are used rarely but increasingl

Common cardiovascular manifestations, most of which are substantial contributors to mortality, include annuloaortic ectasia with or without aortic valve insufficiency, aortic dissection, aortic aneurysm, pulmonary artery dilatation, and mitral valve prolapse. Scoliosis, pectus excavatum and carinatum, arachnodactyly, and acetabular protrusion are common musculoskeletal manifestations. Dural ectasi

[Cutaneous lymphomas: classification and stage-adjusted therapy].

Cutaneous lymphomas are a heterogeneous group of clonal proliferations of T and B lymphocytes with various clinical manifestations and prognosis. The new EORTC WHO classification of cutaneous T- and B-cell lymphomas provides a uniform nomenclature based on clinical, histologic, cytologic and molecular biological features. Accurate classification is a prerequisite for uniform therapeutic concepts. For office-based dermatologist, more than 50% of the therapies deal with classic forms of cutaneous T-cell lymphomas, type mycosis fungoides. In recent years the paradigm for the therapy of cutaneous

[Malaria protection for short-term travelers].

An average of 800 malaria cases are recorded annually in Germany. One person in a hundred dies of the disease. Travel advice given by experienced physicians is essential to raise awareness of malaria risk in endemic areas. Rational mosquito protection on skin and textiles reduces the risk of malaria and other arthropod-transmitted diseases (e. g. dengue fever). Regular intake of an antimalarial drugs to suppress the development forms the basis of reliable chemo prophylaxis in highly endemic areas. The concept of standby emergency treatment (SET) in areas of low or moderate risk needs to be com

[Syncope].

Syncope is a transient, self-limiting loss of consciousness usually leading to a fall. The onset of syncope is relatively rapid and the subsequent recovery is spontaneous, complete and usually prompt. As syncope is a symptom, the aim of the diagnostic work-up is to assess whether there is a syncope or another "nonsyncopal" condition, whether there are clinical features suggesting the diagnosis, whether the patient has an increased risk for mortality or recurrent episodes, and whether the patient must be admitted to hospital. The diagnostic work-up is given for two cases: a 68-year-old male wit
