Protocol of a phase II study to evaluate the efficacy and safety of deep-inspiration breath-hold daily online adaptive radiotherapy for centrally located lung tumours (PUDDING study).

BACKGROUND: Centrally located lung tumours present a challenge because of their tendency to exhibit symptoms such as airway obstruction, atelectasis, and bleeding. Surgical resection of these tumours often requires sacrificing the lungs, making definitive radiotherapy the preferred alternative to avoid pneumonectomy. However, the proximity of these tumours to mediastinal organs at risk increases the potential for severe adverse events. To mitigate this risk, we propose a dual-method approach: deep inspiration breath-hold (DIBH) radiotherapy combined with adaptive radiotherapy. The aim of this

Brachytherapy and external beam radiation in the management of primary penile cancer - Game changer for organ preservation?

OBJECTIVE: In squamous cell carcinoma of the penis (PeCa), treatment options for primary tumors vary by disease stage and may include surgery, radiation, topical chemotherapy, or laser excision. This review aims to highlight the current evidence on the value of radiotherapy as an organ-preserving strategy in primary PeCa. MATERIAL AND METHODS: Manuscripts on primary PeCa treatment with external beam radiotherapy (EBRT) and brachytherapy were evaluated via Scopus, PubMed/MEDLINE, and Web of ScienceTM (2013-2023) to assess their efficacy and safety. Animal studies, studies with <5 patients, and

Experiences using and organizing HIV self-testing.

OBJECTIVE: HIV self-testing (HIVST) is now officially recommended by the WHO, yet much of HIVST evidence to date has focused on quantitative data and hypothetical concerns. Effective scale-up of HIVST in diverse local contexts requires qualitative data from experiences using and organizing HIVST. This qualitative systematic review aims to appraise and synthesize research evidence on experiences using and organizing HIVST. METHODS: We conducted a systematic search of seven primary literature databases, four gray literature sources, and reference lists reporting qualitative evidence on HIVST. D

Lung Ultrasound Utility in the Management of the Neurologically Deceased Organ Donor.

CONTEXT: Lung transplantation is limited by donor lung availability with ∼20% of deceased donor lungs transplanted. Diagnostic testing identifying pulmonary derangements guide donor management strategies to maximize lung transplantation. Lung ultrasound (LUS) identifies pathology in critically ill patients equivalent or superior to chest radiograph (CXR) or computed tomography (CT) scans. No published studies have reported on LUS in neurologically deceased donors (DNDDs). OBJECTIVE: We evaluated LUS in identifying abnormal lung pathology in DNDDs and related these findings to the standard app

Hand hygiene in the intensive care unit.

Healthcare-associated infections affect 1.4 million patients at any time worldwide, as estimated by the World Health Organization. In intensive care units, the burden of healthcare-associated infections is greatly increased, causing additional morbidity and mortality. Multidrug-resistant pathogens are commonly involved in such infections and render effective treatment challenging. Proper hand hygiene is the single most important, simplest, and least expensive means of preventing healthcare-associated infections. In addition, it is equally important to stop transmission of multidrug-resistant p

Fall Risk Identification with Innovative Technologies for Aging in Place: A Clinical Needs Assessment Study.

BACKGROUND: Falls are a leading cause of injury and loss of independence among older adults. While gait-monitoring technologies offer promise for early fall risk detection, adoption is hindered by usability, cost, and integration challenges. Few studies incorporate perspectives from both patients and providers to inform user-centred design. OBJECTIVE: To identify barriers, facilitators, and design considerations for implementing gait-monitoring and fall-assessment technologies among older adults, caregivers, and healthcare providers. METHODS: A qualitative needs assessment was conducted with

HIV/AIDS post-exposure prophylaxis knowledge and uptake among health professionals in Africa: Systematic review and meta-analysis.

BACKGROUND: Healthcare workers in developing countries are at particularly increased risk of infections from blood-borne pathogens because of the high prevalence of such pathogens in their communities as well as the lack of basic personal protective equipment, such as gloves, gowns and goggles. For those exposed healthcare workers, the immediate administration of antiretroviral drugs following exposure to potentially infected blood or other bodily fluids is essential in order to minimize the risk of acquiring HIV infection. This review is aimed at estimating the pooled prevalence of knowledge

Decentralizing PrEP delivery: Implementation and dissemination strategies to increase PrEP uptake among MSM in Toronto, Canada.

Pre-exposure prophylaxis (PrEP) is traditionally prescribed by HIV specialist physicians. Given finite specialist resources, there is a need to scale up PrEP delivery by decentralizing services via other healthcare professionals. We aimed to assess the feasibility of delivering PrEP to men who have sex with men (MSM) through primary care physicians and sexual health clinic nurses. We piloted a multi-component, implementation and dissemination research program to increase provision of PrEP through primary care physicians and sexual health clinic nurses in Toronto, Canada. A Delphi consultation to assess indicators of readiness to provide quality health facility-based lymphoedema management services.

BACKGROUND: The World Health Organization (WHO) in collaboration with partners is developing a toolkit of resources to guide lymphatic filariasis (LF) morbidity management and disability prevention (MMDP) implementation and evaluation. Direct health facility inspection is the preferred method for documenting the readiness of a country programme to provide quality lymphoedema management services, one of the three MMDP criteria used to demonstrate the elimination of LF as a public health problem. METHODOLOGY/PRINCIPAL FINDINGS: As component of tool development, a Delphi consultation was impleme

Content analysis of homeless smokers' perspectives on established and alternative smoking interventions.

BACKGROUND: Cigarette smoking is 5 times more prevalent among homeless individuals than in the general population, and homeless individuals are disproportionately affected by smoking-related morbidity and mortality. Homeless smokers report interest in changing their smoking behavior; however, established smoking cessation interventions are neither desirable to nor highly effective for most members of this population. The aim of this study was to document homeless smokers' perceptions of established smoking interventions as well as self-generated, alternative smoking interventions to elucidate

Guidelines for the Treatment of Latent Tuberculosis Infection: Recommendations from the National Tuberculosis Controllers Association and CDC, 2020.

Comprehensive guidelines for treatment of latent tuberculosis infection (LTBI) among persons living in the United States were last published in 2000 (American Thoracic Society. CDC targeted tuberculin testing and treatment of latent tuberculosis infection. Am J Respir Crit Care Med 2000;161:S221-47). Since then, several new regimens have been evaluated in clinical trials. To update previous guidelines, the National Tuberculosis Controllers Association (NTCA) and CDC convened a committee to conduct a systematic literature review and make new recommendations for the most effective and least toxi

Identifying types of non-pharmacological interventions administered by healthcare providers for older adults with neurocognitive disorders who have lower urinary tract symptoms: A scoping review.

AIMS: Older adults with neurocognitive disorders (NCDs) often experience lower urinary tract symptoms (LUTS), which reduce quality of life (QOL) and increase care needs. Although non-pharmacological interventions are recommended, their types and applications have not been comprehensively synthesized by severity levels or care settings. This scoping review aimed to identify the types of non-pharmacological interventions administered by healthcare providers for older adults with NCDs who have LUTS, and to classify them by severity levels and care settings. This represents the first comprehensive

Biotinidase Activity Inhibition as a Biomarker of Effect to Mercury: Evidence from Amazonian Riverside Populations, In Vitro Assays, and In Silico Analyses.

Mercury is one of the most toxic substances found in nature and can be harmful to both the environment and human health. The brain is a critical organ that is sensitive to mercury exposure, especially during development, when it can cause neurological dysfunction and abnormalities as well as other neurodevelopmental deficits. In this context, the identification of new biomarkers of mercury exposure and intoxication will provide valuable tools for the prevention, response, and treatment of environmental contamination. In this study, we propose biotinidase activity as a biomarker of effect, whic
