Suboptimal Levothyroxine Treatment is Associated with Increases in Mean Blood Pressure and Blood Pressure Variability.

CONTEXT: Thyroid hormone homeostasis is critical to the metabolic function of cardiovascular tissues. The degree to which the suboptimal treatment of hypothyroidism with levothyroxine (LT4) impacts blood pressure (BP) and blood pressure variability (BPV) is not known. OBJECTIVES: Determine the relationship between LT4 time above range (TAR) (TSH level >4.5 mIU/L), time below range (TBR) (TSH <0.4 mIU/L), and changes in annual mean systolic (SBP) and diastolic blood pressure (DBP) and visit-to-visit BPV. METHODS: Using longitudinal data from 2203 adult patients treated with LT4, we modeled wi…

A novel oscillometric method for blood pressure measurement with reduced cuff pressure and measurement time.

The oscillometric method is the predominant technique for non-invasive blood pressure (BP) measurement worldwide, and has enabled widespread adoption of home BP monitoring, which is crucial for hypertension management. However, limitations in the usability of BP monitors, and particularly the pain and discomfort caused by excessive cuff inflation, can discourage consistent daily use of home BP monitors. To address this, we developed a novel oscillometric-based technology that reduces maximum cuff pressure (MaxCP) and measurement time while maintaining accuracy. We validated the performance of …

Attention to blood pressure cuff sizes is important for home and in-clinic blood pressure measurement.

Arm circumference determines appropriate blood pressure cuff size, which is critical for accurate measurements. This cross-sectional analysis aimed to assess cuff size needs according to mid-arm circumferences of Australian adults. Based on typical in-clinic cuff sizes, most Australians would require a medium (51.7%) or large (44.5%) cuff, which means that the cuff would need to be changed for almost every second patient. Most home devices are supplied with a standard (22-32 cm) or wide-range (22-42 cm) cuff, these were found to be unsuitable for 9 million adults (48.3%) and over 701,995 adult…

A novel analytical framework for noninvasive estimation of left ventricular pressure and pressure-volume loops.

Objective. The left ventricle (LV) pressure-volume (PV) loop provides comprehensive characteristic information into ventricular mechanics, aiding in the assessment of systolic and diastolic function. However, its routine clinical application is limited due to the invasiveness of conventional LV catheterization procedures. This study introduces a novel analytical framework for estimating LV pressure (LVP) waveforms noninvasively, using carotid pressure waveforms and routine cardiac imaging.Approach.The proposed method consists of a five-step analytical approach that integrates physical and physi…

The role of pleural pressure in inducing pneumothorax and other adverse effects of positive pressure ventilation.

Mechanical ventilation, essential for critically ill patients, contrasts with natural respiration, primarily due to differences in pleural pressure (Ppleural ). Natural inspiration decreases Ppleural , pulling the lungs away from the thoracic wall, whereas positive pressure inspiration increases Ppleural , pushing the lungs against the thoracic wall. This shift has several consequences. First, elevated Ppleural during positive pressure ventilation can lead to cyclic airway closure, particularly in the dependent lung regions. This increases the risk of atelectasis, that impairs oxygenation and …

The Pressurized Skin: A Review on the Pathological Effect of Mechanical Pressure on the Skin from the Cellular Perspective.

Since human skin is the primary interface responding to external mechanical stimuli, extrinsic forces can disrupt its balanced microenvironment and lead to cutaneous lesions. We performed this review to delve into the pathological effects of mechanical pressure on skin from the cellular perspective. Fibroblasts of different subsets act as heterogeneous responders to mechanical load and express diverse functionalities. Keratinocytes relay mechanical signals through mechanosensitive receptors and the ensuing neurochemical cascades to work collaboratively with other cells and molecules in respons…

Ultrasound guided paralaryngeal pressure versus cricoid pressure on the occlusion of esophagus: a crossover study.

The primary objective of this study is to compare the effectiveness of cricoid pressure (CP) and paralaryngeal pressure (PLP) on occlusion of eccentric esophagus in patients under general anesthesia (GA). Secondary objectives include the prevalence of patients with central or eccentric esophagus both before and after GA, and the success rate of CP in occluding centrally located esophagus in patients post GA. Fifty-one ASA physical status I and II patients, undergoing GA for elective surgery were enrolled in this study. Ultrasonography imaging were performed to determine the position of the eso…

Sacral and Ischial Pressure Ulcer Management With Negative-Pressure Wound Therapy With Instillation and Dwell.

BACKGROUND: The addition of topical fluid instillation, a programmable "dwell" time and a novel foam-wound interface to the established wound healing benefits of negative-pressure wound therapy (NPWT) works synergistically to benefit patients with complex wounds. This engineering breakthrough for wound care has been termed NPWT with instillation and dwell (NPWTi-d), and the new foam dressings are reticulated open cell foam dressings specifically designed for use with NPWTi-d. This combined technology has shown promise in chronic, complex wounds and has potential for the management of sacral an…

Negative-pressure hydrocephalus in the course of a complex postoperative intracranial pressure disturbance: illustrative case.

BACKGROUND: Negative-pressure hydrocephalus (NePH) is a rare clinical entity that presents on the background of ventriculomegaly with atypical symptoms. Its diagnosis is difficult, and some patients experience several shunt revisions until the proper solution is found. OBSERVATIONS: The authors present a patient who developed acute deterioration due to iatrogenic NePH after surgery for a vertebral artery thrombosed giant aneurysm. The deterioration occurred after the insertion of a lumbar drain by which the authors intended to reduce a postoperative subcutaneous cerebrospinal fluid (CSF) coll…

Mixed factors affecting plantar pressures and center of pressure in obese children: Obesity and flatfoot.

BACKGROUND: Flatfoot has a very high incidence of obese children. Functional parameters such as plantar pressures and center of pressure (COP) are sensitive to foot type. However, previous foot biomechanical studies of obese children rarely excluded the flatfoot as a prerequisite of the participants involved. RESEARCH QUESTION: This study aimed to determine whether it is essential to define flatfoot as a subject screening criterion in the foot biomechanical study for obese children. METHODS: Foot types were classified by arch index (AI). Totally 21 obese children with flatfoot (OF group) alo…

Effects of the magnitude of pressure on the severity of injury and capillary closure in rat experimental pressure ulcers.

Experimental pressure ulcers were successfully produced in the rat abdominal wall at 100 mmHg in our previous study. We hypothesized that injury is less severe when pressures are lower than 100 mmHg and explored a critical pressure in the production of pressure ulcers. At 70 and 60 mmHg, repeated compressions for 4 h daily for 5 consecutive days resulted in partial skin necrosis and eschar formation in the majority of rats, whereas skin injuries were absent or very mild in most of the rats at 50 mmHg. The extent of ischemia was also examined by visualization of capillary blood flow using intra…
