Pain Management Protocols During Uterine Fibroid Embolisation: A Systematic Review of the Evidence.

BACKGROUND: Uterine fibroid embolisation (UFE) is an effective treatment for fibroids. There are varying analgesia protocols published to control procedure associated pain. We aimed to assess what protocols are most effective in controlling post-procedural pain. MATERIALS AND METHODS: A systematic review of the Embase and Medline databases was conducted according to PRISMA guidelines. Studies regarding analgesia protocols post-uterine fibroid embolisation with Visual Analogue Scale or Numerical Rating Scale pain scores were included. The mean maximal pain scores of patients post-procedure wer…

Psychosocial Determinants of Pain in Muscle Biopsy.

INTRODUCTION/AIMS: Open muscle biopsy (OMB) is a key diagnostic tool for neuromuscular diseases, though patients often worry about risks and … eristics. Univariate and multivariate linear regression analyses identified factors associated with procedural pain. Follow-up phone calls were performed at 15 and 30 days postbiopsy. RESULTS: Procedural pain was mild (median NRS = 2) with moderate and weak positive correlations with preprocedural anxiety (r = 0.54, p < 0.01) and Clinical Frailty Scale scores (r …

The Role of Medical Hypnosis in Alleviating Procedural Anxiety in Pediatric Interventional Radiology: A Pilot Study.

Pediatric interventional radiology procedures often use general anesthesia to manage pain and anxiety, but general anesthesia carries risks. Medical hypnosis, a noninvasive technique, has shown potential, though its application in pediatric interventional radiology is underexplored. This pilot study compared medical hypnosis and general anesthesia in pediatric interventional radiology, focusing on pre- and post-procedural anxiety and overall patient experience. The mean age of the medical hypnosis group was higher than the general anesthesia group. Post-procedural anxiety was significantly low

Interventional pain management in cancer patients-a scoping review.

BACKGROUND: Pain is the most prevalent symptom in cancer patients. To improve pain care, World Health Organization (WHO) Pain ladder was introduced in 1986 as a template for choosing pain medications in oncological settings. Since then, advancements in oncological treatments have improved the survival of cancer patients, requiring prolonged analgesia in various treatment stages. Additionally, there have been newer challenges in pain management with opioid epidemic and associated opioid use disorders. This has shifted the focus from WHO Pain Ladder and brought new importance to the rapidly evol

Transversus Abdominis Plane Block in the Emergency Department for Paracentesis: A Case Report.

Abdominal paracentesis is a key diagnostic and therapeutic intervention in patients with cirrhosis and ascites, yet it can be painful, especially in the presence of peritonitis. While local anesthetic infiltration is the conventional method for pain management, regional anesthesia techniques such as the ultrasound-guided transversus abdominis plane (TAP) block offer an effective, opioid-sparing alternative. We present the case of a 33-year-old woman with cirrhosis and suspected spontaneous bacterial peritonitis who underwent a large-volume paracentesis in the emergency department following a T

Lumbar Sympathetic Neurolysis: How to and When to Use?

Lumbar sympathectomy was historically a mainstay of treatment for arterial occlusive disease and other vasospastic disorders, before the development of contemporary arterial reconstructive procedures either by surgical or endovascular means. Today, percutaneous methods of sympathetic blockade are possible using chemical neurolytic or ablative modalities. Lumbar sympathetic neurolysis is generally reserved for those patients with ischemic rest pain in the setting of nonreconstructable arterial occlusive disease, although patients with complex regional pain syndrome, peripheral neuralgia, vasosp

A Narrative Review of Remimazolam in Procedural Sedation.

Remimazolam is a recently approved benzodiazepine for procedural sedation in Taiwan. It is a new type of short-acting γ-aminobutyric acid receptor agonist with the characteristics of non-organ-dependent metabolism, no injection pain, and inactive metabolites. Remimazolam has a mild cardiopulmonary suppressive effect, showing good effectiveness and safety in clinical applications, especially in the elderly, critically ill patients, or patients with hepatic and renal insufficiency. This review aims to provide an overview of the specific basic and clinical pharmacology of remimazolam and provide

Ultrasound-Guided Truncal Blocks for Tube Thoracostomy Analgesia in the Emergency Department.

BACKGROUND: Tube thoracostomy is a common yet exceedingly painful emergency department (ED) procedure requiring potent analgesia for safe performance. Traditionally, emergency physicians provide premedication or procedural sedation in combination with local anesthetic infiltration of the insertion tract prior to tube thoracostomy. However, more advanced pain management strategies, such as ultrasound-guided truncal blocks, may offer a targeted and effective analgesic alternative. OBJECTIVE: Herein, the authors discuss the thoracic paravertebral, erector spinae, and serratus plane blocks, and t
