Effect of Nitrous Oxide-Oxygen Inhalation on Pain and Physiological Responses During Supragingival Ultrasonic Scaling: A Prospective Cohort Study.

BACKGROUND: Nitrous oxide-oxygen (N2O-O2) inhalation is widely utilized for anxiolysis in dentistry; however, its clinical efficacy and physiological impact during routine supragingival scaling remain insufficiently assessed. METHODS: This prospective observational cohort study enrolled 100 adult patients. Participants were allocated to either the N2O-O2 sedation group (n = 50) or a non-sedated control group (n = 50) based on treatment preference. Vital signs, including heart rate (HR), systolic blood pressure (SBP), and diastolic blood pressure (DBP), were monitored at baseline (T0), intraop

Determination of sedative and amnestic doses of lorazepam in children.

An open-label, dose-escalation study was conducted to determine doses of lorazepam required to induce anterograde amnesia and sedation in children without producing excessive toxicity. Oncology patients 4 to 17 years of age undergoing lumbar puncture or bone marrow aspiration were eligible; a patient could be entered in the study for a second procedure at a different lorazepam dose. A single oral dose of lorazepam was administered 45-60 minutes before the procedure. Starting with 0.02 mg/kg, the same dose was given to three patients; if no dose-limiting effects occurred, dose was increased by

Ultrasound-Guided Truncal Blocks for Tube Thoracostomy Analgesia in the Emergency Department.

BACKGROUND: Tube thoracostomy is a common yet exceedingly painful emergency department (ED) procedure requiring potent analgesia for safe performance. Traditionally, emergency physicians provide premedication or procedural sedation in combination with local anesthetic infiltration of the insertion tract prior to tube thoracostomy. However, more advanced pain management strategies, such as ultrasound-guided truncal blocks, may offer a targeted and effective analgesic alternative. OBJECTIVE: Herein, the authors discuss the thoracic paravertebral, erector spinae, and serratus plane blocks, and t…

Boerhaave's Syndrome Post-colonoscopy: When Emesis Conceals an Emergency.

Boerhaave syndrome, a rare yet life-threatening spontaneous esophageal rupture, is classically associated with forceful emesis. We present a case of a 48-year-old male with no prior esophageal pathology who developed vomiting, epigastric pain and dyspnea after undergoing an elective colonoscopy. Notably, the patient had experienced emesis following alcohol consumption the day prior to the colonoscopy procedure but did not disclose this history during the pre-procedural assessment. Initial assessments misattributed his symptoms to sedation-related nausea and post-procedural ileus, thereby delay

Review of the Physiology and Anesthetic Considerations for Pleuroscopy/Medical Thoracoscopy.

Pleuroscopy or medical thoracoscopy is the second most common utilized procedure after bronchoscopy in the promising field of interventional pulmonology. Its main application is for the diagnosis and management of benign or malignant pleural effusions. Entry into the hemithorax is associated with pain and patient discomfort, whereas concurrently, notable pathophysiologic alterations occur. Therefore, frequently procedural sedation and analgesia is needed, not only to alleviate the patient's emotional stress and discomfort by mitigating the anxiety and minimizing the pain but also for yielding …
