Progressive chronic kidney disease in primary care: modifiable risk factors and predictive model.

OBJECTIVE: To determine the incidence and prevalence of CKD and of progressive GFR decline, to identify modifiable risk factors of and to develop a predictive risk model for progressive GFR decline in high risk primary care patients. METHODS: Retrospective observational study of 803 patients with or at high risk of CKD exclusively managed in primary care. Baseline data was collected in 2003, follow-up data in 2006. High risk was defined as inadequately controlled hypertension or diabetes, and GFR<60, progressive GFR decline as annual GFR decline >2.5mlmin(-1) 1.73m(-2). RESULTS: CKD was pres

Preserving expertise in vitamin K antagonists in the era of direct oral anticoagulants: avoiding therapeutic segregation.

The rapid uptake of direct oral anticoagulants has sharply reduced the prescribing of vitamin K antagonists (VKAs), giving rise to an underappreciated risk: the progressive loss of the clinical expertise required to manage them. Yet, several populations remain dependent on VKAs, including patients with mechanical heart valves, high-risk antiphospholipid syndrome, and advanced renal disease, as well as patients in resource-limited settings. These patients face both the intrinsic complexity of VKA therapy and a health care environment in which practical experience with these agents is becoming s

Megacystis-microcolon-intestinal hypoperistalsis syndrome (MMIHS) due to a de novo ACTG2 mutation: A neonatal case report.

BACKGROUND: Megacystis-microcolon-intestinal hypoperistalsis syndrome (MMIHS) is a rare congenital visceral myopathy characterized by severe gastrointestinal dysmotility and bladder dysfunction, most commonly associated with ACTG2-mutations. CASE REPORT: We report a preterm neonate with prenatally detected megacystis and a de novo heterozygous ACTG2 variant (c.593G>T; p.Gly198Val). Postnatally, the patient developed progressive abdominal distension and feeding intolerance. Imaging demonstrated dilated bowel loops and microcolon. Exploratory laparotomy on day 5 confirmed dilated ileum, microco

Haemothorax as an Unusual Presentation of Osteochondroma: A case report and brief literature review.

Exostosis, or osteochondroma, is an irregular bone growth commonly occurring near the femur, scapula, humerus and ribs. It can be a single isolated lesion or multiple lesions as in hereditary multiple exostoses (HME) which is a genetic autosomal dominant disorder. Although often asymptomatic, complications such as haemothorax are rare and have serious implications if not promptly recognised. We report a 15-year-old male patient who presented to a tertiary care hospital in Muscat, Oman, in 2023 who presented with progressive chest pain and dyspnoea. The patient had a family history of HME. Vide

Framboesiform Facial Lesions and Chorioretinopathy as Presenting Signs of Syphilis in an Immunocompetent Patient.

Syphilis has long been considered the "great masquerader," notorious for its varying presentations and ability to affect most organ systems in the body. We report the case of a 41-year-old immunocompetent man who presented to ophthalmology with rapidly progressive visual complaints from bilateral panuveitis and concomitant verrucous facial lesions initially disregarded by the patient as acne. Serum testing for syphilis was positive, and he was admitted for 14 days of intravenous (IV) penicillin with multiservice care from dermatology, ophthalmology, and infectious disease. We present photograp

Liver echinococcosis in a renal transplant patient: a particularly intriguing case report.

A renal transplant patient was admitted to his local town hospital with suspected acute pancreatitis (jaundice, abdominal pain and fever). Despite intensive medical care (including endoscopic retrograde cholangiopancreatography) and several negative radiological exams (including computed tomography and nuclear magnetic resonance spectroscopy), no certain diagnosis could be made. After 1 month, the patient was transferred to our facility, where a new sonography was performed, as a prelude to a liver biopsy, which showed the presence of multiple fluid-filled cysts (up to 8 cm), strongly suggesti

Ethical challenges in treatment-goal transitions in invasively ventilated ALS: a case-based topical review.

INTRODUCTION: In advanced amyotrophic lateral sclerosis (ALS), eye movements often represent the last channel for intentional communication. While oculomotor function has traditionally been considered relatively preserved, emerging evidence indicates progressive impairment in long-term survivors on tracheostomy-invasive ventilation (TIV). As a result, eye-based communication may become increasingly unreliable before complete loss, challenging clinical decision-making and advance care planning (ACP). METHODS: We conducted a case-based topical review integrating clinical observation and literat

Living with Transthyretin amyloid cardiomyopathy from a patient perspective.

BACKGROUND: Transthyretin amyloid cardiomyopathy (ATTR-CM) is a progressive condition for which disease-modifying therapies are increasingly available. However, limited research has explored the patient experience with ATTR-CM and its impact on quality of life. METHODS: In this single-center qualitative study, we conducted semi-structured interviews with ten participants (either with ATTR-CM or at-risk family members). At-risk family members were defined as asymptomatic individuals with a clinically actionable TTR variant that were identified through cascade genetic testing. Interviews were r

Nutritional status and interventions in hospice: physician assessment of cancer patients.

BACKGROUND: Cancer cachexia is a multifactorial syndrome characterised by a progressive loss of skeletal muscle mass. It adversely influences quality of life, treatment response and survival. Early identification and multimodal interventions can potentially treat cancer cachexia. However, healthcare professionals demonstrate a lack of understanding and the ability to identify cancer cachexia early. The present study aimed to evaluate the assessment by physicians of nutritional status in cancer patients admitted to hospice. METHODS: A retrospective medical record review was conducted on all ca

The effect of a multidisciplinary outpatient team approach on outcomes in diabetic foot care: a single center study.

BACKGROUND: Recent studies showed no reduction in major amputation rates after introduction of a multidisciplinary team (MDT) approach for the treatment of diabetic foot ulcer. The efficacy of MDTs in the current standard of care is being questioned. This retrospective single-center study evaluated the efficacy of an outpatient MDT approach on limb salvage and ulcer healing in treating diabetic foot ulcers. METHODS: Patients with a diabetic foot ulcer treated before (2015) and after (2017) implementation of an MDT in a single center were compared. The MDT met weekly and consisted of a vascula

Rhabdomyolysis and Multisystem Organ Failure Due to Fulminant Ehrlichiosis Infection.

A previously healthy 51-y-old male presented to his local emergency department with subjective fevers, myalgias, dyspnea, and generalized weakness that had been progressive for several weeks. He was initially diagnosed with bilateral pneumonia, septic shock, and rhabdomyolysis requiring transfer to a tertiary care facility. He was treated for sepsis with broad-spectrum antibiotics, steroids, and a fluid bolus before transfer. Once he arrived at the tertiary care facility, he developed respiratory failure requiring intubation and ventilatory support. Ceftriaxone and metronidazole were started i

[Pharmaceutical care of heart failure: theory and clinical practice].

Heart failure is a serious, progressive and chronic illness that affects an ever-growing part of the population. Heart failure has a bad prognosis: only 50% of patients are still alive five years after the diagnosis. It is important to improve this outcome and the pharmaceutical care of these patients is therefore a priority for pharmacists. In the present study we investigated the differences between theory and clinical practice in order to identify opportunities to improve the pharmaceutical care of the heart failure patient. We showed that the knowledge of the pharmacist concerning heart fa

A narrative review of clinical and non-clinical determinants of health-related quality of life in advanced Parkinson's disease.

Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized by a wide range of motor and non-motor symptoms that lead to substantial disability over time, particularly in advanced stages, with a significant impact on patients' quality of life (QoL). Given the chronic and currently incurable nature of the disease, symptom management and the preservation of health-related QoL (HRQoL) are central therapeutic priorities from a patient-centered perspective. In this context, there has been a growing emphasis on the use of patient-reported outcome measures (PROMs) to capture th

Rehabilitation interventions in bronchiectasis: expanding evidence, personalization, and clinical practice.

Bronchiectasis is a chronic and heterogeneous respiratory disease characterized by irreversible airway dilation, recurrent infection, persistent inflammation, and progressive functional impairment. Pulmonary rehabilitation is consistently recommended in international guidelines as a core non-pharmacological intervention; however, its implementation in clinical practice remains inconsistent and access to comprehensive rehabilitation programmes remains limited despite strong guideline endorsement. Recent advances in disease conceptualization, particularly the treatable traits framework, together

Quality standards for the diagnosis and management of ATTR and AL amyloidosis: a nominal group technique study.

BACKGROUND: Transthyretin (ATTR) and immunoglobulin light-chain (AL) amyloidosis are progressive conditions that can have debilitating effects on people. Delayed diagnosis and variable access to high-quality care continue to affect quality of life and prognosis. Clear and ambitious quality standards could drive consistent care and improve outcomes. METHODS: A modified nominal group technique was used to develop consensus-based quality standards for the diagnosis and management of ATTR and AL amyloidosis. Eighteen experts participated, including patient representatives and clinicians from high

Pneumomediastinum, Pneumopericardium, and Pneumothorax: An Uncommon Triad Induced by Cocaine Use.

We present a rare case of a clinically stable 24-year-old male patient who developed acute central chest pain shortly after intranasal cocaine use. Initial chest imaging revealed pneumomediastinum, a small apical pneumothorax, and pneumopericardium-three uncommon yet potentially serious findings. Remarkably, the patient was hemodynamically stable with unremarkable clinical examination and vital signs. High-resolution computed tomography (CT) of the thorax and abdomen with oral and intravenous contrast effectively excluded esophageal rupture, a critical differential diagnosis requiring surgical

Severe parapharyngeal abscess that developed significant complications: management during the COVID-19 pandemic.

A 50-year-old Caucasian man presented to the emergency department during the early stages of the COVID-19 pandemic with a rapidly progressive facial swelling, fever, malaise and myalgia. The patient had recently travelled to a COVID-19-prevalent European country and was therefore treated as COVID-19 suspect. The day before, the patient sustained a burn to his left forearm after falling unconscious next to a radiator. A CT neck and thorax showed a parapharyngeal abscess, which was surgically drained, and the patient was discharged following an intensive care admission. He then developed mediast

Constipation: take it seriously.

A case report of a young, healthy female who presented as an emergency with large bowel obstruction secondary to a rectosigmoid tumour. Acute presentation was preceded by a progressive history of change in bowel habit managed medically in the community with laxative treatment. The family history of note included breast cancer of an identical triplet aged 41 years. An emergency Hartman's procedure was successfully performed the same day as presentation due to the high risk of perforation. The patient was discharged home within a week.
