An ultrastructural study was done on 15 mixed growth hormone (GH) and prolactin (PRL)-secreting pituitary adenomas surgically removed from acromegalic patients with hyper-prolactinaemia, in order to see whether the 2 hormones were present in the same cell or in different cells. Serum prolactin (PRL) concentrations at baseline and after TRH stimulation were determined in 15 healthy women and in 51 premenopausal patients suffering from Gross Cystic Breast Disease. The level of pituitary prolactin reserve (PPR) was studied in 56 patients with chronic respiratory failure (CRF) in the acute stage after injections of metoclopramide and Thyrotropin Releasing Hormone (TRH). PPR was low in 90% of the subjects in both groups so that the pathogenic mechanism is more likely to be hypophyseal than hypothalamic and due to the reduced synthesis of PRL by the pituitary gland as a result of inadequate ATP synthesis and/or functional alterations to the receptors or pituitary cells following hypoxaemia and hypercapnia.

An attempt was made to correlate the serum concentration of PRL, GH and cortisol with anxiety experienced before elective surgery. A significant direct correlation was observed between cortisol and anxiety (P = 0.028), and between GH and anxiety (P = 0.001) although the latter correlation may have reflected the confounding effects of sex and age on GH levels. No correlation was observed between anxiety and PRL and we conclude that psychosocial stress cannot be invoked as an occasional cause of hyperprolactinaemia.

Seventeen women with prolactin levels of 100 ng/ml and above suspected of harboring prolactin-secreting pituitary adenoma, from the basis of this study. Ten patients had radiological signs of an adenoma while in 7 the radiological criteria for such a diagnosis were not fulfilled. Twenty-eight women with hyperprolactinaemia and amenorrhoea received bromocriptine treatment which resulted in 31 term pregnancies. Bromocriptine treatment was stopped as soon as pregnancy was established. Nineteen of the women had radiological signs of a pituitary tumour. Subfractional activities and some properties of beta-glucuronidase were studied at different ontogenetic stages of the human placenta. Maximum activity was localized in normal pregnancies rose from 13.58 ng/ml in the 6th week to 52.46 ng/ml in the 15th week of gestation with marked individual variability.
