Predicting, preventing and managing persistent pain after breast cancer surgery: the importance of psychosocial factors.

Persistent pain after breast cancer surgery (PPBCS) is increasingly recognized as a potential problem facing a sizeable subset of the millions of women who undergo surgery as part of their treatment of breast cancer. Importantly, an increasing number of studies suggest that individual variation in psychosocial factors such as catastrophizing, anxiety, depression, somatization and sleep quality play an important role in shaping an individual's risk of developing PPBCS. This review presents evidence for the importance of these factors and puts them within the context of other surgical, medical,

Progressive cone dystrophy and sensorineural hearing loss.

A 39-year old man presented 13 years ago with a history of progressive loss of vision and photophobia. A full ophthalmological and ENT work-up during several years of follow-up, including psychophysical as well as electrophysiological tests, revealed a progressive cone dystrophy in combination with sensorineural hearing loss. His younger sister presented with very similar features and underwent the same work-up. A novel syndrome of progressive cone dystrophy and sensorineural hearing loss is described in both siblings. Both also suffered from non-ocular disease possibly related to ciliary dysf

On the diagnosis of hypoglycemia in car drivers--including a review of the literature.

Hypoglycemia may reach forensic relevance concerning the psychophysical ability of running a car and with respect to a possible imputability. Our retrospective study included anamnesis and clinical symptoms observed in drivers with impaired performance to point out correlations between biochemical parameters and the actual course of the disease and its sequelae on roadworthiness. Clinical-biochemical estimations were performed on glucose, lactic acid and ethanol including a toxicological screening. It is proposed to utilize the estimation of glucose and lactic acid (the so-called combined valu

[Convulsive seizure].

A formerly healthy 14-year old boy with difficulties at school was admitted after two generalized seizures. Blood samples taken in the emergency room revealed normal serum-values for glucose and magnesium, but low calcium and elevated phosphorus. First evaluations showed normal age-related psychophysical development, serum-PTH was elevated and serum-1,25(OH)2D was normal. A CT-scan disclosed symmetric intracerebral calcifications. Further investigations confirmed the diagnosis of pseudohypoparathyroidism type 1B. Adequate treatment with calcium and calcitriol normalized serum-calcium, phosphor

Analysis of the diagnostic process in sialography.

The diagnostic process in sialography was evaluated using the mouse parotid gland as a model. The relative significance of each of the three diagnostic phases, psychophysical, psychological and nosological, was determined by analysing observer accuracy in detecting sialoadenitis and sialodochitis on digital and analogue microsialograms. Automated diagnosis based on parametric values of the digitized image was compared with the human system using ROC analysis. The overall accuracy of automated diagnosis was superior to that of an observer with analogue images. It is concluded that if observers

Application of visually evoked response near the threshold of vision to objective measurement of dark adaptation.

The visually evoked response (VER) to dim lights with intensities within the scotopic or lower mesopic range increased in amplitude during the progressive dark adaptation. The VER amplitude vs. time curve resembled the psychophysical dark adaptation curve. The spectral sensitivity curve of the dark-adapted VER matched the C.I.E. scotopic sensitivity curve. The dark-adapted VER was abnormal in patients with retinitis pigmentosa and with congenital stationary night blindness and recordable in a patient with cone dysfunction. There was a close correlation between the VER threshold and subjective

Restoring Natural Forearm Rotation in Transradial Osseointegrated Amputees.

Osseointegrated transradial prostheses have the potential to preserve the natural range of wrist rotation, which improves the performance of activities of daily living and reduces compensatory movements that potentially lead to secondary health problems over time. This is possible by enabling the radius and the ulna bone to move with respect to each other, restoring the functionality of the original distal-radioulnar joint. In this paper, we report on psychophysics tests performed on an osseointegrated transradial amputee with the aim to understand the extent of mobility of the implants that i…

Allergic Contact Dermatitis: A Model of Inflammatory Itch and Pain in Human and Mouse.

This chapter is an overview of published observations from our laboratory on the psychophysics and neurobiology of the persistent itch and pain of allergic contact dermatitis (ACD). ACD is a clinically significant problem with many features characteristic of other pruritic disorders. Our approach was to produce ACD experimentally in humans and in the mouse. The goal was to use the mouse as an animal model for investigating the peripheral neural mechanisms of itch and pain of ACD in humans. Humans and mice were each sensitized by cutaneous topical application of squaric acid dibutyl ester, a ha…

Failure to detect meaning in RSVP at 27 ms per picture.

The human visual system has the remarkable ability to rapidly detect meaning from visual stimuli. Potter, Wyble, Hagmann, and McCourt (Attention, Perception, & Psychophysics, 76, 270-279, 2014) tested the minimum viewing time required to obtain meaning from a stream of pictures shown in a rapid serial visual presentation (RSVP) sequence containing either six or 12 pictures. They reported that observers could detect the presence of a target picture specified by name (e.g., smiling couple) even when the pictures in the sequence were presented for just 13 ms each. Potter et al. claimed that this …

Cortical hierarchies perform Bayesian causal inference in multisensory perception.

To form a veridical percept of the environment, the brain needs to integrate sensory signals from a common source but segregate those from independent sources. Thus, perception inherently relies on solving the "causal inference problem." Behaviorally, humans solve this problem optimally as predicted by Bayesian Causal Inference; yet, the underlying neural mechanisms are unexplored. Combining psychophysics, Bayesian modeling, functional magnetic resonance imaging (fMRI), and multivariate decoding in an audiovisual spatial localization task, we demonstrate that Bayesian Causal Inference is perfo…

Specific retinotopically based magnocellular impairment in a patient with medial visual dorsal stream damage.

We report here retinotopically based magnocellular deficits in a patient with a unilateral parieto-occipital lesion. We applied convergent methodologies to study his dorsal stream processing, using psychophysics as well as structural and functional imaging. Using standard perimetry we found deficits involving the periphery of the left inferior quadrant abutting the horizontal meridian, suggesting damage of dorsal retinotopic representations beyond V1. Retinotopic damage was much more extensive when probed with frequency-doubling based contrast sensitivity measurements, which isolate processing…

Biological motion processing in adolescents with early periventricular brain damage.

The developing brain is traditionally viewed to possess a great compensatory potential. Here we ask whether visual processing of point-light displays depicting human walking is compromised in adolescents who were born premature (between 27 and 33 gestation weeks) and suffer early bilateral damage to periventricular brain regions. Combining psychophysics with volumetric analysis of structural magnetic resonance imaging (MRI), we found that even relatively small periventricular parieto-occipital lesions may result in long-lasting breakdown of visual processing of point-light displays. Analysis o…

Temporal dynamics of early somatosensory processing in goal-directed actions.

Tactile sensitivity is temporally modulated on a moving limb. Yet the neural mechanisms underlying this modulation remain unclear. Using electroencephalography, we examined the temporal dynamics of early somatosensory processing during goal-directed reaching. Participants (self-reported gender: 22 female, 16 male) reached for their unseen left hand while a brief standard vibration of a fixed intensity was applied to the moving right index finger in different movement phases. After each reach, participants compared this standard with a second comparison vibration of varying intensity applied to

Recovery of Olfactory Function After Mepolizumab Treatment in Patients with Chronic Rhinosinusitis with Nasal Polyps: Influence of the Number of Surgeries.

Background: Chronic rhinosinusitis with nasal polyps (CRSwNP) is a predominantly type 2 inflammatory disease associated with significant olfactory dysfunction. The real-life effect of mepolizumab on smell recovery and the influence of prior surgery and peripheral eosinophilia remain unclear. Objective: Our objective was to evaluate olfactory outcomes after 12 months of mepolizumab in CRSwNP and analyze the impact of previous surgeries and baseline blood eosinophilia. Methods: This was a prospective observational study including 33 consecutive CRSwNP patients treated with mepolizumab. Olfactory

The association of gustatory dysfunction, olfactory dysfunction, and cognition in older adults.

BACKGROUND: The association of gustatory dysfunction (GD) with quality of life (QOL) and cognition in older adults is understudied. Our objective was to study the prevalence of GD in the community and explore impacts and associated factors. METHODS: A prospective, multi-institutional, pre-corona virus disease (COVID) cohort of adults aged 50 years and older had smell and taste testing using "Sniffin' Sticks" (TDI) and "Taste Strips." The impact of GD on mood, QOL, and social interaction was assessed through visual analog scales. Subjects completed the Questionnaire of Olfactory Disorders, Pat

An Exploration of the Influence of Non-Biomechanical Factors on Lifting-Related LBP.

Objective: The primary objective was to compare non-biomechanical factors between manual workers with and without a history of LBP related to lifting. A secondary objective was to investigate associations between the change in pain intensity during repeated lifting (termed pain ramp) and non-biomechanical factors tested in the LBP group. Methods: Manual workers currently in lifting occupations with and without a history of lifting-related LBP were recruited (21 LBP and 20 noLBP) and took part in a repeated (100) lift task. A series of non-biomechanical factors, including psychological, work-re

Vestibular Perceptual Thresholds in Older Adults With and Without Age-related Hearing Loss.

OBJECTIVES: Older adults with age-related hearing loss (ARHL) are at greater risk of falling and have greater mobility problems than older adults with normal hearing (NH). The underlying cause of these associations remains unclear. One possible reason is that age-related declines in the vestibular system could parallel those observed in the auditory system within the same individuals. Here, we compare the sensitivity of vestibular perceptual abilities (psychophysics), vestibular end-organ functioning (vestibular evoked myogenic potentials and video head impulse tests), and standing balance (po…
