Leydig Cell Tumor-Induced Gonadotropin-Independent Precocious Puberty Progressing to Gonadotropin-Dependent Precocious Puberty Post Orchiectomy: Out of the Frying Pan Into the Fire.

Many pathologies can cause gonadotropin-independent precocious puberty (GIPP) in prepubertal boys. Leydig cell tumor is one rare cause of this presentation. Here we present a six-year-old boy with features of isosexual precocious puberty, high testosterone levels, low gonadotropin levels, and bone age advancement. Testicular USG revealed a left-sided testicular tumor. The left testis was removed surgically, and the Leydig cell tumor was confirmed on histopathology. Post orchiectomy, the boy had elevated testosterone levels with raised luteinizing hormone (LH) levels. A diagnosis of gonadotropi

Serum Metabolomic Analysis of Healthy and Central Precocious Puberty Girls.

BACKGROUND: The incidence of precocious puberty (PP) has been on the rise in recent years. Based on different control mechanisms, childhood PP is divided into central precocious puberty (CPP) and peripheral precocious puberty (PPP). CPP accounts for 80% of all PP cases. Metabolomics is considered a link between genomics and phenotypes, providing a direct reflection of intricate biological traits. However, studies on serum metabolomic changes in CPP are very limited. METHODS: In this study, non-targeted metabolomics analysis of serum from healthy controls and CPP groups was performed. Serum sa

Gut microbiota alterations modulate high-fat diet-induced precocious puberty.

The link between gut microbiota and precocious puberty has garnered increasing attention; however, the underlying mechanisms remain poorly understood, and most existing studies are cross-sectional, making it difficult to capture the longitudinal changes in gut microbiota throughout precocious puberty. Here, female rats were fed high-fat diets supplemented with antibiotics to establish a model of precocious puberty with gut microbiota perturbation. Gut microbiota dynamics were characterized based on stool samples collected at postnatal day (PND) 21, 25, 29, and 33. Serum metabolomic profiles on

Genetic, neuropeptidergic, and cardiometabolic interplay in female central precocious puberty.

Central precocious puberty (CPP) results from premature reactivation of the hypothalamic-pituitary-gonadal axis and is increasingly recognized as a systemic condition linked to cardiometabolic health. Genetic mutations, particularly in imprinted genes such as MKRN3 and DLK1, are major monogenic causes of familial CPP, while rare activating variants in KISS1 and KISS1R highlight the pivotal role of kisspeptin signaling. Neuropeptides, including kisspeptin and neurokinin B, are central to pubertal regulation. Advances in clinical assessment, biochemical markers, pelvic ultrasound, and genetic te

Gonadotropin-Dependent Precocious Puberty: Single-Center Experience From Western India.

OBJECTIVE: To describe the characteristics of gonadotropin-dependent precocious puberty (GDPP) in Indian children. METHODS: Clinical profiles of GDPP (n=78, 61 females) and premature thelarche (n=12) from a single center in Western India were retrospectively studied. RESULTS: Pubertal onset was earlier in boys than girls (29 vs 75 months, respec-tively; P=0.008). The basal luteinizing hormone (LH) was ≥0.3 mIU/mL, except 18% of GDPP girls. At 60 minutes after GnRHa-stimulation, all patients (except one girl) had LH ≥5 mIU/mL. The GnRHa-stimulated LH/FSH ratio was ≥0.34 at 60 minutes in girls

Central precocious puberty - etiology with particular consideration of neurological causes.

Central precocious puberty (CPP) is a rare disease of poorly understood etiology. The cause is mostly idiopathic. However, congenital and acquired structural changes in the central nervous system are also involved. Prevalent organic changes in the central nervous system, especially among boys, prompt imaging assessment in each case of CPP. In addition, genetic and environmental factors have been reported. Safe and effective treatment is available. The time of treatment implementation is crucial to the successful outcome. Proper diagnosis and treatment make it possible to avoid a number of comp

Growing Up Fast: Managing Autism Spectrum Disorder and Precocious Puberty.

CASE: John is a 4-year-old boy with autism spectrum disorder (ASD) and developmental delay who presented with concerns about increasing aggressive behavior at a follow-up visit with his developmental-behavioral pediatrician. Diagnosis of ASD was made via Diagnostic and Statistical Manual of Mental Disorders, 5th version criteria at initial evaluation at 34 months. Medical history at that time was pertinent for rapid linear growth since the age of 1 and recent pubic hair growth and penile enlargement. Family history was significant for early puberty in a maternal uncle and 4 distant maternal re

Malocclusion and maxillofacial characteristics of young girls having precocious puberty.

AIM: This study aimed to evaluate the dentofacial characteristics and need for orthodontic treatment in young girls having precocious puberty (PP). DESIGN: It was a cross-sectional study that included 39 girls, age ranging from 6 to 11 years old, with confirmed diagnosis of PP. The Dental Aesthetic Index (DAI) and the Index of Orthodontic Treatment Need (IOTN) were used to assess malocclusion and the need for orthodontic treatment, and cephalometric analysis was used for the diagnosis of facial growth abnormalities. Data analysis included descriptive statistics and one-sample t-test. RESULTS

Paternally Inherited DLK1 Deletion Associated With Familial Central Precocious Puberty.

CONTEXT: Central precocious puberty (CPP) results from premature activation of the hypothalamic-pituitary-gonadal axis. Few genetic causes of CPP have been identified, with the most common being mutations in the paternally expressed imprinted gene MKRN3. OBJECTIVE: To identify the genetic etiology of CPP in a large multigenerational family. DESIGN: Linkage analysis followed by whole-genome sequencing was performed in a family with five female members with nonsyndromic CPP. Detailed phenotyping was performed at the time of initial diagnosis and long-term follow-up, and circulating levels of D

Virilizing adrenocortical carcinoma advancing to central precocious puberty after surgery.

Adrenocortical carcinoma (ACC) in pediatric and adolescent patients is rare, and it is associated with various clinical symptoms. We introduce the case of an 8-year-old boy with ACC who presented with peripheral precocious puberty at his first visit. He displayed penis enlargement with pubic hair and facial acne. His serum adrenal androgen levels were elevated, and abdominal computed tomography revealed a right suprarenal mass. After complete surgical resection, the histological diagnosis was ACC. Two months after surgical removal of the mass, he subsequently developed central precocious puber

KISS1 gene polymorphisms in Korean girls with central precocious puberty.

Kisspeptin/G-protein couple receptor-54 (GPR54) system plays a key role in the activation of the gonadotropic axis at puberty. Central precocious puberty (CPP) is caused by the premature activation of hypothalamic gonadotropin-releasing hormone secretion. This study was aimed to identify KISS1 gene variations and to investigate the associations between KISS1 gene variations and CPP in Korean girls. All coding exons of KISS1 gene were sequenced in Korean girls with CPP (n = 143) and their healthy controls (n = 101). Nine polymorphisms were identified in KISS1 gene. A novel single-nucleotide pol

Sylvian cistern arachnoid cyst--a rare cause of precocious puberty.

Precocious puberty still remains an elusive diagnosis in the majority of patients. Infrequently, lesions of the central nervous system are associated with sexual precocity. Depending on their location, these cysts may affect many systems, however, there is little information concerning their involvement in endocrinological disorders. We report a case of a sylvian cistern arachnoid cyst presenting with precocious puberty in a 3-year-old girl. The child recovered following a cystoperitoneal shunt. The mass effect of the arachnoid cyst upon the hypothalamus was, at least in part, responsible for

From the radiologic pathology archives: precocious puberty: radiologic-pathologic correlation.

Precocious puberty represents a unique diagnostic problem in which imaging plays an important role. Development of secondary sex characteristics may result from inappropriate activation of the hypothalamic-pituitary axis with release of gonadotropin, or from gonadotropin-independent secretion of sex steroids by the adrenal glands or gonads. A variety of lesions can manifest with precocious puberty, including various central nervous system (CNS) lesions, adrenal lesions, and sex cord-stromal tumors of the testis or ovary. CNS lesions causing precocious puberty are much more common in boys than

Serum kisspeptin levels in Korean girls with central precocious puberty.

Central precocious puberty (CPP) is caused by premature activation of hypothalamic gonadotropin-releasing hormone (GnRH) secretion. Kisspeptin and G-protein coupled receptor-54 system is the essential gatekeeper of the reproductive system, playing a key role in the activation of the gonadotropic axis at puberty. We aimed to determine whether serum kisspeptin may function as a marker for CPP by investigating serum kisspeptin levels in Korean girls with CPP and their prepubertal controls. Serum kisspeptin levels of Korean girls with CPP (n = 30) and age-matched healthy prepubertal controls (n =
