[Lung Volume Reduction Surgery for Severe Emphysema].

Lung volume reduction surgery( LVRS) is performed in patients with severe emphysema who do not respond to medical therapy. We report two cases of LVRS that resulted in favorable outcomes. We used a method in which the cyst wall is incised, the cyst base is covered with fibrin glue and polyglycolic acid (PGA) sheets, and the cyst wall is resected using an automatic suturing device. In the first case, respiratory failure was observed preoperatively, and home oxygen therapy was introduced. However, minimally invasive LVRS was performed using a thoracoscopic approach, resulting in a good outcome f

Unexpected case of pneumomediastinum and subcutaneous emphysema: primary or secondary aetiology?

A 77-year-old man was admitted with a relapse of antineutrophil cytoplasmic antibody-positive vasculitis with pulmonary involvement and acute kidney injury. There was a background of pulmonary fibrosis (non-specific interstitial pneumonia type pattern) and superadded pulmonary haemorrhage, acute pulmonary oedema and sepsis. The patient was intubated for 4 days and remained dependent on high flow oxygen and continuous positive airway pressure after extubation. A chest radiograph performed 2 weeks after extubation demonstrated unexpected, extensive pneumomediastinum and subcutaneous emphysema. T

Thymus and activation-regulated chemokine (TARC/CCL17) predicts decline of pulmonary function in patients with chronic obstructive pulmonary disease.

BACKGROUND: The deterioration of pulmonary function, such as FEV1-decline, is strongly associated with poor prognosis in patients with chronic obstructive pulmonary disease (COPD). However, few investigations shed light on useful biomarkers for predicting the decline of pulmonary function. We evaluated whether thymus and activation-regulated chemokine (TARC), a Th2 inflammation marker, could predict rapid FEV1-decline in COPD patients. METHODS: We recruited 161 patients with stable COPD and performed pulmonary function test once every six months. At the time of registration, blood tests, incl

Do survival and morbidity of very-low-birth-weight infants vary according to the primary pregnancy complication that results in preterm delivery?

OBJECTIVE: This retrospective study was conducted to determine whether predischarge survival and morbidity of very-low-birth weight infants varied according to the principal pregnancy complication that led to preterm delivery. STUDY DESIGN: The hospital records of 535 consecutive live-born singleton infants who weighed between 500 and 1499 gm were reviewed, and five primary complications that resulted in preterm delivery were identified: (1) premature rupture of membranes (n = 244, 46%), (2) idiopathic preterm labor (n = 97, 18%), (3) antepartum hemorrhage (n = 58, 11%), (4) pregnancy-induced

Radiographic evaluation of the potential lung volume reduction surgery candidate.

Delineating the extent and distribution of emphysema is an essential component of the evaluation of candidates for lung volume reduction surgery (LVRS). Imaging also may identify contraindications to LVRS, including bronchiectasis and pleural scarring. The chest X-ray is of limited utility in LVRS evaluation. Chest computed tomography (CT) scanning is an essential component of the evaluation, demonstrating the presence of emphysema and its amount and distribution. Clinical experience has shown that a substantial minority of chest CT scans will also demonstrate pulmonary nodules, some of which

Predicting the response to lung volume reduction surgery using scintigraphy.

This study was conducted to evaluate the use of quantitative scintigraphy with a newly designed marker to assess and predict the efficacy of lung volume reduction surgery in treating emphysema. In a series of 50 patients with severe emphysema who underwent the operation, ventilation/perfusion scintigraphy was performed and 2 markers of area ratio and lung uniformity were measured before and 6 months after surgery. The markers were correlated with the results of pulmonary function tests. The histopathological subtype of emphysema was also determined in the resected specimen and related to impro

Bronchoscopic endobronchial valves.

Bronchoscopic endobronchial valves (EBVs) have emerged as a minimally invasive alternative to surgical lung volume reduction for patients with severe emphysema. Emphysema, a progressive lung disease associated with COPD, leads to hyperinflation of the affected lung and reduces respiratory efficiency. EBVs function by limiting airflow to diseased lung segments, causing atelectasis of the emphysematous lung, and allowing healthier lung regions to expand, thereby improving overall pulmonary function. Two primary EBV systems, Zephyr and Spiration valves, have demonstrated efficacy in clinical tria

Monocyte-derived alveolar macrophages are key drivers of smoke-induced lung inflammation and tissue remodeling.

Smoking is a leading risk factor of chronic obstructive pulmonary disease (COPD), that is characterized by chronic lung inflammation, tissue remodeling and emphysema. Although inflammation is critical to COPD pathogenesis, the cellular and molecular basis underlying smoking-induced lung inflammation and pathology remains unclear. Using murine smoke models and single-cell RNA-sequencing, we show that smoking establishes a self-amplifying inflammatory loop characterized by an influx of molecularly heterogeneous neutrophil subsets and excessive recruitment of monocyte-derived alveolar macrophages

Severe vitamin D deficiency is associated with emphysema progression in male patients with COPD.

BACKGROUND: Patients with chronic obstructive pulmonary disease (COPD) have an increased risk of vitamin D deficiency. Vitamin D levels also correlate with lung function in patients with COPD. However, there are few reports on vitamin D deficiency and emphysema severity in COPD. This study aimed to investigate the effects of plasma 25-hydroxyvitamin D (25-OHD) level on emphysema severity in male COPD patients. METHODS: A total of 151 male subjects were selected from the Korean Obstructive Lung Disease (KOLD) cohort. Subjects were subdivided into four subgroups according to their baseline plas

Pulmonary hypertension associated with COPD: a phenotype analysis.

BACKGROUND: Pulmonary hypertension (PH) associated with COPD (PH-COPD) exhibits diverse phenotypes, challenging therapeutic management. This study aimed to describe the characteristics of COPD patients with distinct phenotypes, namely end-stage COPD with or without PH (group 1), other COPD patients with mild-to-moderate pre-capillary PH-COPD (group 2) and COPD patients with a pulmonary vascular phenotype (PVP) (group 3). METHODS: We performed a retrospective analysis of COPD patients who underwent right heart catheterisation from 2015 to 2022. RESULTS: 81 patients were included in group 1, 3

Incidental findings on chest CT imaging are associated with increased COPD exacerbations and mortality.

BACKGROUND: This study aimed to evaluate whether incidental CT findings of emphysema, airway thickening and bronchiectasis, as seen on CT scans performed for other non-pulmonary clinical indications, are associated with future acute exacerbations of COPD resulting in hospitalisation or death. METHODS: This multicentre prospective case-cohort study comprised 6406 subjects who underwent routine diagnostic chest CT for non-pulmonary indications. Using a case-cohort approach, we visually graded CT scans from cases and a random sample of ∼10% of the baseline cohort (n=704) for emphysema severity (

A case of Langerhans histiocytosis with HIV-like immunodeficiency.

A case of histiocytosis X (Langerhans type) associated with bullous pulmonary emphysema and acquired immune deficiency, regarding CD4 positive cells, is described. Previous history was remarkable for skin lesions which first appeared in 1981 and progressively worsened, diabetes insipidus diagnosed in 1986, and bullous pulmonary emphysema detected in 1988. Biopsy results of skin lesions were consistent with histiocytosis X. Thyroid gland involvement was found by means of cytological examination. The search for HIV infection (also performed by means of immunoblotting and PCR) was negative. To ou…

[Bronchopulmonary dysplasia].

Bronchopulmonary dysplasia is a most frequent contemporary lesion of the lung in early childhood. It is characterized by clinical symptoms (neonatal respiratory distress syndrome) and by X-ray picture reflecting progressive morphological changes in the respiratory tract, i.e. in trachea, bronchi, bronchioles, and pulmonary acini, followed by interstitial pulmonary lesion. As usual, bronchopulmonary dysplasia is forerun by hyaline membranes and may be associated with or followed by interstitial emphysema. Pathogenetic participants are toxicity of highly concentrated and long administered oxygen

Quantitatively Assessed Emphysema Severity on HRCT Independently Predicts Coronary Artery Disease in COPD: A Retrospective Cohort Study.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is associated with an increased risk of coronary artery disease (CAD). However, the role of emphysema, which represents an important structural subtype of COPD, in the development of CAD remains insufficiently clarified. This study aimed to evaluate whether quantitatively assessed emphysema on high-resolution computed tomography (HRCT) independently predicts CAD in COPD patients. METHODS: This retrospective cohort study included 392 COPD patients with no prior history of CAD between 2015 and 2020. All participants underwent HRCT for aut

[Selective bronchial occlusion in the treatment of unilateral interstitial emphysema in respiratory distress in premature infants].

A case of right unilateral pulmonary interstitial emphysema in a compromised premature infant was managed by right selective bronchial occlusion with a favorable outcome. Occlusion with a Rashkind catheter occurred on day 8 and was well-tolerated. The balloon was deflated 10 hours later and no recurrence was observed. Right occlusion is easier than left selective intubation and seems to be the best method for treatment of unilateral interstitial emphysema in infants in extremely poor conditions.
