Changes in hyperpolarization-activated cyclic nucleotide-gated channel expression and activity in bladder interstitial cells of Cajal from rats with detrusor overactivity.

INTRODUCTION AND HYPOTHESIS: To investigate changes in the expression and function of the hyperpolarization-activated cyclic nucleotide-gated (HCN) channels, interstitial cells of Cajal (ICCs) were isolated from detrusor overactivity (DO) rats induced with partial bladder outlet obstruction (PBOO). We hypothesized that increased expression of HCN channels in ICCs would enhance the excitability of ICCs and bladder in DO rats. METHODS: Forty adult female Sprague-Dawley rats were randomly assigned to control and DO groups. The expression of HCN isoforms in the rat bladders was detected using rev

ELIOS: A Multicenter, Molecular Profiling Study of Patients with EGFR-Mutant Advanced Non-Small Cell Lung Cancer Treated with First-Line Osimertinib.

UNLABELLED: ELIOS (NCT03239340) prospectively compared tumor biopsies obtained pre-treatment and post-progression to characterize acquired resistance mechanisms to first-line osimertinib in epidermal growth factor receptor (EGFR)-mutant advanced non-small cell lung cancer (NSCLC). Of 154 patients enrolled, 52 patients had next-generation sequencing (NGS) results from paired tissue biopsies. The most common acquired alterations at progression were MET amplification (17%), deletion of CDKN2A/CDKN2B (15%) and MTAP (13%), and EGFR C797S (13%). Proteogenomic analysis (n = 32 at baseline and n = 18

Prevalence and therapeutic response of EGFR T790I mutation in NSCLC patients with acquired resistance to EGFR-TKIs: a retrospective cohort study.

BACKGROUND: The EGFR T790I mutation is a potential resistance mechanism against EGFR tyrosine kinase inhibitors (TKIs) in non-small cell lung cancer (NSCLC). However, its prevalence and clinical significance remain unexplored. We aim to delineate the mutational spectrum of EGFR T790I in NSCLC patients and investigate its association with clinical outcomes and resistance to EGFR-targeted therapies. METHODS: We retrospectively reviewed the clinico-genomic databases of Chinese NSCLC patients who underwent next-generation sequencing (NGS) at Geneseeq from January 2017 to June 2024. The clinicopath

Metabolite monitoring to guide thiopurine therapy in systemic autoimmune diseases.

6-Thioguanine nucleotide (6-TGN) is the active metabolite of thiopurine drugs azathioprine and 6-mercaptopurine. 6-Methylmercaptopurine (6-MMP) is an inactive and potentially hepatotoxic metabolite. A subgroup of patients (shunters) preferentially produce 6-MMP instead of 6-TGN, therefore displaying thiopurine resistance and risk for hepatotoxicity. Outside inflammatory bowel disease literature, few data exist regarding individualized thiopurine therapy based on metabolite monitoring. This study sought to describe metabolite monitoring in patients receiving weight-based thiopurine for systemic

Overview: A New Era of Cancer Genomics in Lymphoid Malignancies.

Novel technologies including next-generation sequencing have not only delineated the molecular pathogenesis of lymphoid malignancies but also identified novel biomarkers predicting the outcome of specific therapies. In addition, many actionable genetic alterations, which can be targeted by either specific therapeutic compounds or monoclonal antibodies, have been discovered. An appropriate selection of the patients enrolled in clinical trials using novel drugs targeting specific mutations will usher in a new era of personalized medicine in clinical practice of lymphoid malignancies.

Stabilization of disease after targeted therapy in a thymic carcinoma with KIT mutation detected by clinical next-generation sequencing.

Precision medicine uses individually determined genomic information to guide treatment in cancer and other diseases. We have implemented a clinical genomics assay that uses targeted next-generation sequencing of 25 cancer-related genes to guide the use of targeted therapies in diverse malignancies. We report the case of a 55-year-old woman with a poorly differentiated squamous cell carcinoma of thymic origin, with disease progression after standard treatment. Targeted tumor sequencing revealed the presence of a KIT codon 579 deletion (p.D579del). This specific mutation has not previously been

Enzymatic analysis of isomeric trithymidylates containing ultraviolet light-induced cyclobutane pyrimidine dimers. I. Nuclease P1-mediated hydrolysis of the intradimer phosphodiester linkage.

Our recent findings suggest that enzymatic hydrolysis of the intradimer phosphodiester bond may constitute the initial step in the repair of UV light-induced cyclobutane pyrimidine dimers in human cells. To examine the susceptibility of this phosphodiester linkage to enzyme-mediated hydrolysis, the trinucleotide d-Tp-TpT was UV-irradiated and the two isomeric compounds containing a cis-syn-cyclobutane dimer were isolated by high performance liquid chromatography and treated with various deoxyribonucleases. Snake venom phosphodiesterase hydrolyzed only the 3'-phosphodiester group in the 5'-isom

Non-immobilized graphene oxide-based screening of aptamers for non-small cell lung cancer (NSCLC) drug resistance biomarkers and development of a nano-detection platform.

This study aims to establish a technology for the dynamic monitoring and early identification of resistance to epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) in patients with non-small cell lung cancer (NSCLC). To achieve this, we first developed an osimertinib-resistant NSCLC cell model and employed a graphene oxide-based SELEX (GO-SELEX) strategy to screen for high-affinity single-stranded DNA aptamers. After multiple rounds of enrichment and validation, the selected Osi-1 aptamer exhibited optimal binding affinity to the supernatant of resistant cells. Subsequent pu

Candidate pharmacogenetic signals at CCL2 and ITPA associated with JAK inhibitor response in Taiwanese rheumatoid arthritis.

OBJECTIVES: Rheumatoid arthritis (RA) is a chronic autoimmune disease characterised by persistent synovial inflammation. Janus kinase inhibitors (JAKi) are effective targeted therapies for RA, yet clinical responses vary. We aimed to identify genetic predictors of JAKi effectiveness in Taiwanese patients with RA. METHODS: In this retrospective study, 275 RA patients treated with tofacitinib, baricitinib, or upadacitinib in the Taiwan Precision Medicine Initiative (TPMI) were recruited and classified as Effective or Non-effective. Fifty-eight candidate single-nucleotide polymorphisms (SNPs) we

Identifying the genomic landscape of EGFR-mutant lung cancers with central nervous system metastases.

BACKGROUND: Despite the intracranial efficacy of osimertinib, central nervous system (CNS) metastases remain a major cause of morbidity and mortality in EGFR-mutant non-small-cell lung cancer (NSCLC). The genomic drivers of CNS dissemination are poorly understood. PATIENTS AND METHODS: We analyzed the clinicogenomic features of patients with EGFR-mutant NSCLC receiving first-line osimertinib with extracranial next generation sequencing (NGS) (n = 262) and individuals with intracranial NGS (n = 81). Paired extra- and intracranial NGS was available for 14 patients. Time-to-event analyses were c

Relapsed CLL: sequencing, combinations, and novel agents.

Although the therapy of chronic lymphocytic leukemia (CLL) has changed rapidly over the last 5 years, the key considerations in selecting a therapy for a previously treated patient with CLL continue to include the nature of the prior therapy and the duration of prior remission to that therapy, the prognostic features of the disease, and the health and comorbidities of the patient in question. For patients treated initially with chemoimmunotherapy, randomized trials have demonstrated the benefit of targeted therapy. Retrospective data suggest that ibrutinib is preferred as a first kinase inhibi

Landscape of Genomic Mechanisms of Resistance to Selective RET Inhibitors in RET-Altered Solid Tumors: Analysis of the RETgistry Global Consortium.

PURPOSE: Rearranged during transfection (RET) alterations are oncogenic drivers across solid tumors. Selective RET inhibitors (SRI) selpercatinib and pralsetinib have transformed outcomes for patients with RET-altered malignancies. Limited knowledge exists on genomic mechanisms of resistance to SRI. EXPERIMENTAL DESIGN: We established "RETgistry," a global consortium of patients with advanced RET-altered solid tumors who received SRI and underwent postprogression tissue or plasma biopsies assessed by next-generation sequencing. Frequencies of secondary RET resistance mutations and acquired no

Ocular and systemic vascular endothelial growth factor ligand inhibitor use and nephrotoxicity: an update.

Tumor growth is intricately linked to the process of angiogenesis, with a key role played by vascular endothelial growth factor (VEGF) and its associated signaling pathways. Notably, these pathways also play a pivotal "housekeeping" role in renal physiology. Over the past decade, the utilization of VEGF signaling inhibitors has seen a substantial rise in the treatment of diverse solid organ tumors, diabetic retinopathy, age-related macular degeneration, and various ocular diseases. However, this increased use of such agents has led to a higher frequency of encountering renal adverse effects in

Nucleotide salvage deficiencies, DNA damage and neurodegeneration.

Nucleotide balance is critically important not only in replicating cells but also in quiescent cells. This is especially true in the nervous system, where there is a high demand for adenosine triphosphate (ATP) produced from mitochondria. Mitochondria are particularly prone to oxidative stress-associated DNA damage because nucleotide imbalance can lead to mitochondrial depletion due to low replication fidelity. Failure to maintain nucleotide balance due to genetic defects can result in infantile death; however there is great variability in clinical presentation for particular diseases. This re
