Cineradiographic study of wrist motion after fracture of the distal radius.

Residual dorsal tilt is one of the main deformative factors in the malunion of fractures of the distal radius. We investigated the effect of such tilt on the range of wrist motion and on carpal alignment in a cineradiographic study of wrist motion in 30 patients with extra-articular Colles' fracture. With increased dorsal tilt, the range of wrist motion became more restricted and abnormalities of carpal alignment during wrist motion became apparent. When the dorsal tilt was less than 10 degrees, the effects on the range of motion and on the dynamic carpal alignment were small.

Effects of distal radius fracture malunion on wrist joint mechanics.

An experimental model using a static positioning frame, pressure-sensitive film (Fuji), and a microcomputer-based videodigitizing system was used to measure contact areas and pressures in the wrist. Contact areas and pressures were compared in a group of wrists between the normal state and with simulated distal radius fracture malunions of varying degrees. In simulated malunions, radial shortening to any degree slightly increased the total contact area in the lunate fossa, and was significant at 2 mm of shortening. By angulating the distal radius more than 20 degrees either palmar or dorsal, t

[Avulsion fractures of the distal part of the radius with anterior displacement in children].

The authors report 7 cases of fractures of the distal part of the radius, with a frontal displacement with children. This fracture is rare in pediatric traumatology. There is no particular type of mechanism that will provoke that fracture; in five cases the child was falling down on the palm of his hand and in two cases in a forced flexion. The problem about this fracture is the stability of the reduction. A proper function of the wrist is impossible with an imperfect result. The orthopedic treatment is often unable to stabilize the fracture. The authors need a surgical management using a plat

The prevention and treatment of complications from fractures of the distal radius and ulna.

Almost one third of fractures of the distal radius are associated with some complication, either bony or soft tissue. Proper reduction and appropriate stabilization of the fracture are necessary to prevent many of these complications, particularly those involving the articular end of the radius. Although closed treatment is usually satisfactory, an open reduction and internal fixation of the fracture often is indicated in younger patients.

[Comminuted fractures of the lower end of the radius. Treatment by traction-suspension].

Treatment of comminuted fractures of the distal radius bone by the traction-suspension technique is both simple and innocuous. The authors describe this old and satisfactory technique, slightly modified: a transmetacarpal pin is introduced and suspended to a giving set stand, and a counter-traction of 3 to 4 kg is exerted by means of a transulnar pin. After remodelling of the bone under general anaesthesia and radiographic control, the pins are sunk in plaster to be removed 6 to 8 weeks later. Excellent, often unexpected clinical and radiological results can be obtained, and complex traumatic

A modification of the use of extraskeletal immobilization for comminuted fractures of the distal radius.

The patients selected for this modification of the Roger Anderson device are few, because the method is intended for unstable fractures of the distal radius. Extensive disabling stiffening of the wrist and hand encountered following prolonged immobilization by rigid distraction methods can be avoided or greatly reduced by this simple modification of the method. This modification of the extraskeletal fixation should prove to be superior to rigid distraction when early decompression of the carpal tunnel is indicated.

Pins and plaster treatment of comminuted fractures of the distal end of the radius.

The technique used in the treatment of seventy-five patients with severely comminuted, often intra-articular fractures near the wrist was as follows- Pins were inserted through the metacarpals and proximal part of the ulna, reduction was done, and the pins were incorporated in a cast from elbow to knuckles. This allowed movement of the fingers and the elbow. The main complications - loss of radial length and pintract infection - are avoided by attention to technical details. Of the patients with adequate follow-up, 86 per cent had good or excellent results.

Pronator quadratus-sparing volar plating of distal radius fractures: A prospective study.

PURPOSE: Volar locked plates for the distal radius fractures (DRFs) are applied through the Henry approach, and its modification entails a routine step to disinsert the pronator quadratus from its radial and distal attachments. This muscle insertion is mostly fleshy with minimal tendinous tissue, and surgeons find it difficult to reattach at the end of the surgery, the hardware comes in direct contact with the flexor tendons. METHODS: The study was performed as a single-center prospective case series to investigate the possibility of preserving the pronator quadratus and, at the same time, ac…

Management of pediatric distal radius fractures - A systematic review and meta-analysis.

BACKGROUND: Pediatric distal radius fractures are the most common fractures in children. Management of displaced injuries remains controversial because remodeling capacity varies with skeletal maturity, fracture stability, and fracture subtype. Metaphyseal and physeal fractures differ biologically: metaphyseal injuries are primarily threatened by redisplacement, whereas physeal injuries carry risks of growth disturbance and iatrogenic physeal injury. An evidence-based, maturity- and stability-guided framework is required. METHODS: A PRISMA-compliant systematic review and Meta-analysis was per…

Arthroscopic Management of Intraarticular Distal Radius Fractures: A Step-by-Step Approach.

The traditional management of distal radius fractures involves the use of fluoroscopy or capsulotomy to guide articular reduction. Arthroscopy is a tool that can aid in achieving articular congruity as well as the management of concomitant injuries, where indicated. In this article, we present a practical arthroscopic step-by-step guide in the treatment of distal radius fractures.

Ballistic Distal Radius Fractures: A Single-Center Experience in Management and Outcomes.

PURPOSE: Gunshot wounds (GSWs) to the upper extremities can be devastating and often require surgical treatment. Ballistic injuries to the distal radius in particular present a complex challenge for hand surgeons because of concerns of bony comminution, neurovascular injury, and injury to surrounding structures. This study compares the demographics, management, and complications of nonballistic versus ballistic distal radius fractures (DRFs). METHODS: In this retrospective study, patients over the age of 16 years with radiographically confirmed DRFs were reviewed from 2018 to 2023. Demographi…

[Palmar Displacement in Distal Radius Fractures following Extension Injuries of the Wrist].

BACKGROUND: Fractures of the distal radius have been traditionally classified based on the suspected mechanism of injury. Among clinicians, … The aim of this study was to analyse the mechanisms of injury that lead to various types of distal radius fractures. Particular emphasis was placed on "Smith" fractures and on the biomechanical, physiological and anatomical characteristics that contribute to their occurrence. To assess these relationships, data of fractures were collected following a life-like fr …

Automatic Segmentation and Radiologic Measurement of Distal Radius Fractures Using Deep Learning.

BACKGROUND: Recently, deep learning techniques have been used in medical imaging studies. We present an algorithm that measures radiologic parameters of distal radius fractures using a deep learning technique and compares the predicted parameters with those measured by an orthopedic hand surgeon. METHODS: We collected anteroposterior (AP) and lateral X-ray images of 634 wrists in 624 patients with distal radius fractures treated conservatively with a follow-up of at least 2 months. We allocated 507 AP and 507 lateral images to the training set (80% of the images were used to train the model, …

Dorsal wrist plating for the management of intra-articular distal radius fractures.

The aim of the study was to evaluate the results of Dorsal Wrist Plating in intra-articular distal radius fractures with a dorsal displacement. In this prospective study, a single surgeon treated 20 patients with a (partially) intra-articular distal radius fracture with a dorsal rim avulsion or a dorsal Barton's type fragment. They all underwent an open reduction and internal fixation by Dorsal Wrist Plating. A total of 17 patients had a follow-up period of at least 12 months (mean follow- up of 17 months) and these patients were included in the study. Both functional and radiological outcome …
