Obstetrical Brachial Plexus Palsy.

Obstetrical brachial plexus palsy is a disorder of the peripheral nervous system and occurs in as many as 0.4% of infants born. It is associated with shoulder dystocia, use of mechanical extraction, and macrosomia; it occurs more frequently in infants born by vaginal delivery. The unilateral injury to the brachial plexus complex occurs during the delivery phase as lateral traction is applied to the head to permit shoulder clearance. The infant typically presents in the delivery room with decreased active movements of the affected arm and asymmetrical primitive reflex responses. The severity of

Bilateral mini-thoracotomy off-pump Jarvik 2000 implantation in regional asymmetric paravertebral analgesia.

We describe the surgical technique and treatment of a 59-year-old male with cardiogenic shock, who underwent a minimally invasive off-pump ventricular assist device (VAD) implantation with the aid of paravertebral regional analgesia in bilateral mini-thoracotomies as first procedure described in the literature. He was extubated soon after the procedure, in the operating room, with the aim to reduce the right ventricle impairment. These issues are particularly true for patients suffering from pulmonary hypertension and disease, in whom the shortest time of postoperative intubation is fundamenta

Alternatives to packing in septorhinoplasty.

Nasal packing is considered routine by most physicians and patients at the completion of nasal and septal surgery. Yet the rationale for this maneuver is not clearly defined by reported investigation or logical analysis. We discuss 75 consecutive nasal surgical procedures completed without packing. There were two postoperative episodes of bleeding, both from pyriform aperture incisions for lateral osteotomy and both managed in the recovery room with an absorbable gelatin sponge. Technical refinements such as scrupulous preoperative history taking, through-and-through suturing of the entire sep…

Extraordinary growth of giant cell reparative granuloma during pregnancy.

A 25-year-old woman had a giant cell reparative granuloma (GCRG) in the nasal fossa during the fifth month of pregnancy. This was treated with a partial excision, but the lesion recurred three months later. By this time, there was extension into the anterior fossa. Because of rapidly decreasing visual acuity, headache, and facial pain, a combined craniofacial approach was undertaken. The lesion encroached on the optic chiasm and a piecemeal, but incomplete, removal of the lesion was performed. The patient spontaneously gave birth to a normal child while in the recovery room. She received no ad…

Remote Cerebellar Hematoma Following Supratentorial Craniotomy: A Case Report.

Remote cerebellar hemorrhage (RCH) is a rare complication following supratentorial craniotomies with unclear pathophysiology, predisposing factors, and clinical outcomes. This is a case of a 46-year-old female who presented to the emergency room with a complaint of severe headache associated with nausea. MRI studies demonstrated right frontal lesions consistent with low-grade glioma. She underwent a right frontal craniotomy, and the tumor was resected successfully. She developed a severe headache on postoperative day five, and CT scans showed ipsilateral cerebellar hematoma. She was managed co

Concussion in the Athletic Training Room: a Team Physician Narrative.

Concussion evaluation and management has changed significantly. Understanding proper recognition, evaluation, and management allows for improved provision of care to patients. This paper will approach this topic from a sideline to training room management versus the traditional clinic evaluation RECENT FINDINGS: Research is continuing to refine and examine tools to assist in proper concussion evaluation. Concussion recovery protocols are becoming more conservative as patients are taking longer to recover than previously thought. Treatment of concussion is becoming more sophisticated and patien

Simple and Efficient Room-Temperature Release of Biotinylated Nucleic Acids from Streptavidin and Its Application to Selective Molecular Detection.

The biotin-streptavidin bond is the strongest noncovalent bond in nature and is thus used extensively in biotechnology applications. However, the difficulty of releasing the bond without high temperatures or corrosive solutions can be a barrier to applications involving nucleic acids and other delicate substrates. Here, room-temperature phenol is employed to release biotin-tagged DNA constructs from streptavidin rapidly and efficiently. It is demonstrated that synthetic biotinylated DNA can be recovered at yields approaching 100% from both solution-phase and bead-bound streptavidin with as lit

Skull roentgenography in the evaluation of head injury.

The role of skull roentgenograms in determining choice of therapy, hospital admission, and length of hospitalization was evaluated in 106 consecutive patients with head injury. Thirty patients were discharged from the emergency room after initial evaluation and had uneventful recovery. Seventy-six were admitted and discharged without operative treatment one to ten days later. Only five had pathologic skull findings roentgenographically. Of these, one was discharged from the emergency department; the remaining four were admitted because of abnormal neurologic findings. All five recovered uneven

Pseudomonas putida and Pseudomonas fluorescens Species Group Recovery from Human Homes Varies Seasonally and by Environment.

By shedding light on variation in time as well as in space, long-term biogeographic studies can help us define organisms' distribution patterns and understand their underlying drivers. Here we examine distributions of Pseudomonas in and around 15 human homes, focusing on the P. putida and P. fluorescens species groups. We describe recovery from 10,941 samples collected during up to 8 visits per home, occurring on average 2.6 times per year. We collected a mean of 141 samples per visit, from sites in most rooms of the house, from the surrounding yards, and from human and pet occupants. We recov

Risk evaluation for staphylococcal food poisoning in processed milk produced with skim milk powder.

The growth of S. aureus and the production of staphylococcal enterotoxin A (SEA) in skim milk concentrates stored at inappropriate temperatures in a recovery milk tank (tank for excess concentrated skim milk) used in the manufacture of skimmed milk powder were investigated. Also, it was estimated if a possible outbreak of food poisoning would occur if the contaminated skimmed milk powder was used in the manufacture of processed milk. Skim milk concentrates with milk solid content of 15, 25, and 35% were inoculated with S. aureus at 1-2 log CFU/ml and incubated at 15, 25, or 35 degrees C for 0

Room Temperature Sensing of Volatile Organic Compounds Using Hybrid Layered SnO Mesoflowers and Laser-Induced Graphitic Carbon Devices.

In this work, we demonstrate chemiresistive volatile organic compound (VOC) sensors prepared by drop-cast assembly of layered tin monoxide mesoflowers (SnO-MFs) on additively produced laser-induced graphene-like carbon (LIG). The SnO-MFs were synthesized below 100 °C at ambient pressure and offer a low fabrication energy alternative route to typical furnace-prepared metal-oxide materials. The additive dropcast assembly of room-temperature operating metal oxide active material allows the substitution of LIG for metal current collectors and glass for alumina, reducing the environmental footprint

Intravenous indoxacarb toxicity: A rare case of unusual exposure with clinical implications.

A 25-year-old male farmer presented to the emergency department with confusion, cyanosis, and low oxygen saturation (SpO₂) after an intentional intravenous injection of indoxacarb. Initial laboratory tests revealed severe methemoglobinemia (40.8 %), hypokalemia, hyponatremia, leukocytosis, and coagulation abnormalities. Despite supplemental oxygen, SpO₂ remained low, prompting Intensive Care Unit (ICU) admission. Methylene blue (2 mg/kg IV loading dose, followed by 1 mg/kg after one hour) was administered to reduce methemoglobin levels, which decreased to 2 % within six hours, leading to signi

Boerhaave's syndrome in an ultra-distance runner.

A runner competing in a 100 mile trail race presented with severe lower chest pain and right upper abdominal pain. His pain started immediately after he took an over the counter non steroidal anti-inflammatory pill a few hundred metres after leaving the aid station. When he took the pill, he immediately had to vomit and spit out the pill. On arriving back at the aid station, he was noted to have severe left-sided chest pain that worsened with reclining. He also had profound dyspnoea. Initial vital signs were unremarkable. The runner was immediately transported to an emergency room and eventual

Administering caudal anesthesia at completion of clubfoot surgery does not affect postoperative use of narcotics.

In the pediatric population, control of postoperative pain is a challenging and important issue. We conducted this retrospective study to determine whether single-dose caudal anesthesia administered after club-foot surgery helps to decrease postoperative use of narcotics. Fifty-one patients given an injection of caudal anesthesia (bupivacaine) at completion of clubfoot surgery were compared with 41 patients who did not receive a caudal block. Postoperative pain control was assessed by recording how much narcotic was used by each patient during time in the recovery room and during the first 8 h…
