Xylazine Complicating Opioid Ingestions in Young Children.

The authors of this report present 3 cases of synthetic opioid intoxication complicated by the concomitant presence of the additive xylazine, a nonopioid sedative analgesic and muscle relaxant used in veterinary medicine that potentiates respiratory depression associated with the opioid toxidrome. Three exposed children presented with severe signs and symptoms, 2 of whom experienced cardiac arrest, a need for continuous naloxone infusion or multiple naloxone doses, or mechanical ventilation to treat respiratory failure after their exposures. Additives were detected in urine studies only throug

Genes and molecules that can potentiate or attenuate psychostimulant dependence: relevance of data from animal models to human addiction.

Recent evidence suggests that a variety of molecule products play critical roles in the transitions from recreational drug use to drug abuse, and then to drug dependence. Elucidation of the roles of specific molecules in the development of drug dependence can come from preclinical animal models and/or from clinical data. Among animal models, behavioral sensitization, conditioned place preference, drug discrimination, drug self-administration, and extensions of these basic procedures have been widely used to identify molecule products that might be involved in psychostimulant dependence. Repeat…

A cluster-analytic study of substance problems and mental health among street youths.

Based on a cluster analysis of 211 street youths aged 13-24 years interviewed in 1992 in Toronto, Ontario, Canada, we describe the configuration of mental health and substance use outcomes. Eight clusters were suggested: Entrepreneurs (n = 19) were frequently involved in delinquent activity and were highly entrenched in the street lifestyle; Drifters (n = 35) had infrequent social contact, displayed lower than average family dysfunction, and were not highly entrenched in the street lifestyle; Partiers (n = 40) were distinguished by their recreational motivation for alcohol and drug use and the

Cannabis and the liver: Things you wanted to know but were afraid to ask.

Many Canadians use cannabis for medicinal and recreational purposes. We describe the current understandings of how cannabis is metabolized in the liver and its potential interactions with other common drugs. We also summarize how cannabis may exert various effects in chronic liver diseases (CLDs), especially in chronic hepatitis C virus (HCV) and fatty liver disease.

Cocaine-induced renal disease.

Cocaine has anaesthetic, vasoconstrictive and CNS stimulatory effects. Presently, it is used clinically as a local anaesthetic and abused as a recreational drug. It has been implicated in both acute and chronic renal failure and has been reported to affect every aspect of the nephron. This article will review the spectrum of cocaine-induced kidney disease and attempt to give insight into the pathophysiological mechanisms involved.

Eating attitudes, weight concerns and beliefs about drug effects in women who use ecstasy.

The drug ecstasy has appetite suppressant and exercise promoting effects that may appeal to young women who are concerned about weight and body image. This study therefore aimed to determine whether young women who use ecstasy differ from those who do not use this drug in concerns about eating and weight, and in beliefs about how these are affected by recreational drugs. One hundred and thirty young women, all cigarette smokers, were recruited; 73 who used ecstasy were compared with 57 who did not. All were assessed on Garner's (1991) Eating Disorder Inventory (EDI-2), body mass index (BMI), d

Forensic reconstruction of cause of death in a dismembered and partially burnt body: A case report.

BACKGROUND: Dismemberment and postmortem burning are uncommon forensic challenges that may obscure identification, injury interpretation and determination of cause of death. Systematic anatomical reconstruction and careful examination of dismemberment wounds, vital reaction of tissues and thermal alteration are essential for forensic reconstruction. Toxicological findings must be interpreted according to the substances and analytical categories actually examined. CASE PRESENTATION: Dismembered remains of an adult male were recovered from multiple locations and anatomically reassembled for med

Recreational nitrous oxide-related myeloneuropathy in adolescents: a multicenter retrospective case series and systematic literature review.

UNLABELLED: Nitrous oxide (N2O) is a common recreational drug. Chronic use inactivates vitamin B12 resulting in neurological damage. Most studies have focused on adults. This study aims to describe unique features of N2O-related myeloneuropathy in adolescents. Adolescents aged 14-18 years admitted with N2O-related myeloneuropathy to three tertiary medical centers in 2024 were enrolled. Their medical records were reviewed retrospectively. A systematic review was conducted for N2O-related neurologic complications in adolescents, using Embase, MEDLINE, Scopus and PubMed databases. Clinical data f

Substance-Related Decision-Making Trajectories in Fitness Contexts: A Person-Centered Longitudinal Study.

BACKGROUND: Preventing image- and performance-enhancing drug (IPED) use in recreational fitness settings remains a public health concern. Although theory-informed interventions have been associated with lower risk acceptance and substance-related behavior, heterogeneity in underlying decision-making processes remains insufficiently understood. Identifying distinct psychosocial configurations may inform more differentiated prevention research. METHODS: A three-wave cluster-randomized longitudinal field study was conducted across ten commercial fitness studios in Germany (N = 624 at baseline; n

Determinants of high rates of smoking among people with psychosis living in a socially disadvantaged region in South Australia.

OBJECTIVE: This study aimed to identify factors associated with the high rates of smoking amongst people with psychosis living in a disadvantaged region in Adelaide, South Australia. METHODS: Data were collected from 402 people with psychosis, aged 18-64 years, who lived in the northern suburbs of Adelaide. This area is disadvantaged on many measures of socioeconomic well-being and people living in this region have higher rates of smoking compared to the general Australian population. We hypothesised that whilst tobacco use by people with psychosis living in this region was primarily associat

Are Alcohol Anti-relapsing and Alcohol Withdrawal Drugs Useful in Cannabinoid Users?

Cannabinoids are still classified as illegal psychoactive drugs despite their broad and increasingly acknowledged therapeutic potential. These substances are most famous for their wide recreational use, particularly among young adults to either alter the state of consciousness, intensify pleasure induced by other psychoactive substances or as an alternative to the previously abused drugs. It is important to emphasize that cannabinoids are often taken together with a variety of medications intended for the treatment of alcohol use disorder (AUD) or alcohol withdrawal syndrome (AWS). These medic

A two-year review of cocaine findings in impaired driving investigations in Ontario, Canada.

Drug-impaired driving is an increasing public safety concern across Canada, particularly due to the demonstrated increase in use of recreational drugs such as cocaine. Cocaine is a central nervous system stimulant drug; however, it can impair an individual's driving ability in both the stimulant and crash phases. Despite the scientific consensus regarding cocaine's potential for driving impairment, there is relatively little information available regarding blood concentrations and associated observations of impairment in suspected impaired drivers. Retrospective data analysis was performed to

The phenomenon of low-frequency heroin injection among street-based urban poor: drug user strategies and contexts of use.

BACKGROUND: Dominant public health and medical discourse has relied on a pharmacocentric conception of heroin use-that is, the notion that heroin users inject compulsively to stave off physical and psychological withdrawal. Previous research disputes this claim suggesting that other patterns of heroin use, such as occasional, recreational, or controlled use are possible. In our previous cross-sectional epidemiological research, we identified the phenomenon of low frequency heroin injection (low-FHI), among street-based drug users. The goal of the current study was to qualitatively assess and c

Vitamin B12 deficiency-induced neuropathy secondary to prolonged recreational use of nitrous oxide.

A 24-year-old female, otherwise healthy, presented to the Emergency Department (ED) with difficulty walking and bilateral leg pain. The patient was a recreational nitrous oxide (NO2) user, also known as "whippets" or simply nitrous. Neurologic examination demonstrated an unsteady gait and positive Romberg sign along with normal deep tendon reflexes and normal muscle strength in upper and lower extremities. Laboratory results demonstrated macrocytic erythropoiesis, reduced B12, elevated homocysteine, and elevated methylmalonic acid. Outpatient MRI later demonstrated degeneration of the posterio

Gamma-hydroxybutyrate (GHB): a scoping review of pharmacology, toxicology, motives for use, and user groups.

Gamma hydroxybutyrate (GHB) is a central nervous system depressant with euphoric and relaxant effects. Documentation of GHB prevalence and the underreporting of abuse remains problematic, given the availability of GHB and its precursors γ-butyrolactone (GBL) and 1,4-butanediol (1,4-BD) and the ease of synthesis from kits available on the Internet. The continued abuse of and dependence on GHB, and associated fatalities, present an on-going public health problem. As the drug GHB remains an underresearched topic, a scoping review was chosen as a technique to map the available literature into a de

Neurobiological Underpinnings in Drug Addiction.

BACKGROUND: Drug addiction is a chronic biochemical drug use disorder that affects the human brain and behaviour, and leads to an uncontrollable use of a licit or illicit drug. Drug addiction can commence, usually in the young, with the use of a non-medical or a recreational drug in social gatherings, which becomes more frequent over a period of time. It is associated with incremental doses of the drug in order to achieve a state of euphoria. Addiction to drugs has been identified as a relevant social and health problem presenting a risk to public health, especially with regards to communicabl

Cocaine-associated Eustachian tube stenosis causing chronic 'glue ear': a rare cocaine-induced destructive lesion.

BACKGROUND: Cocaine is one of the most used recreational drugs. Whilst medical uses exist, chronic recreational nasal use of cocaine is associated with progressive destruction of the osseocartilaginous structures of the nose, sinuses and palate - termed cocaine-induced midline destructive lesions. CASE REPORT: A 43-year-old male with a history of chronic cocaine use, presented with conductive hearing loss and unilateral middle-ear effusion. Examination under anaesthesia revealed a completely stenosed left Eustachian tube orifice with intra-nasal adhesions. The adhesions were divided and the h

"Now drugs in Libya are much cheaper than food": A qualitative study on substance use among young Libyans in post-revolution Tripoli, Libya.

BACKGROUND: Libya is facing a rapidly growing epidemic of illicit drug use and HIV. This situation is fueled by a complex array of factors, mainly the consequences of the political and military turmoil of the Arab Spring. Although it is extensively documented in other settings that young people are one of the most vulnerable groups to both HIV and illicit drug use, no study has explored this issue among young people in Libya. The current study addresses this research gap. METHODS: This study is a qualitative study using in-depth interviews guided by a semi-structured questionnaire. We used a
