Bradycardic responses to vagally mediated bedside maneuvers in healthy volunteers.

PURPOSE: To determine the relative potency in healthy individuals of the vagally mediated reflexes used clinically to inhibit sinoatrial and atrioventricular node function. SUBJECTS AND METHODS: Twenty healthy volunteers with no history of heart disease performed face immersion in cold water and the Valsalva maneuver twice, to maximum endurance and to the subjective point of first discomfort, and face immersion in warm water and the Müller maneuver to maximum endurance only. Right and left carotid massage and left, right, and bilateral eyeball compression were each performed for 15 seconds. C

Using vestibular evoked myogenic potentials to localise brainstem lesions. A preliminary report.

BACKGROUND: Vestibular Evoked Myogenic Potentials (VEMPs) are saccular responses to acoustic stimuli. They can be recorded from the sternocleidomastoid muscle ipsilaterally to the stimulated ear. Their reflex arc includes the ipsilateral vestibular nuclei. OBJECTIVE: To determine the usefulness of VEMPs in localising brainstem lesions. METHODS: We used VEMPs, Blink Reflex (BR) and Brainstem Auditory Evoked Responses (BAERs) to evaluate six patients presenting with acute ischaemic or haemorrhagic brainstem lesions, or basilar dolichoectasia. RESULTS: MRI in patient one revealed a dorsolatera

[Unilateral apraxia of eyelid closure in ischemic stroke: role of the right hemisphere in the emotional gesture communication].

INTRODUCTION: Apraxia of eyelid closure (AEC) is an infrequent disorder that is characterised by the inability to close the eyelids on command, although spontaneous blinking and reflex shutting of the eyes is preserved. Very few cases of unilateral AEC have been reported and no long-term follow-ups have been carried out. We report the case of a patient with unilateral AEC that was followed up over a 3-year period and also discuss the role played by the right hemisphere in this disorder. CASE REPORT" We examined the case of a 67-year-old right-handed male who was admitted because of a parietote

Unusual neutrophil infiltration under the soft contact lens in a patient with Behçet's disease.

BACKGROUND: Behçet's disease (BD) characterized by multisystemic disorders has many kinds of ocular involvement. We report one case of BD with unusual neutrophil infiltration under a soft contact lens (SCL). CASE: A 31-year-old woman diagnosed as having BD exhibited bilateral recurrent phlyctenular keratitis and finally developed corneal perforation in the cornea of her right eye. OBSERVATIONS: A therapeutic SCL was immediately fitted on the right cornea as a bandage and another SCL on the left, as prophylaxis. White gelatinous membrane suddenly appeared between the bandage SCL and the corne

Intraoperative Detection of a Lateralized S2 Nerve Root Conduction Block Using Hemisphincter Bulbocavernosus Reflex Monitoring.

Sacral reflex monitoring using bulbocavernosus reflex (BCR) and external urethral sphincter reflex (EUSR) is used to assess neural pathways responsible for bowel and bladder function during spine surgery. However, these techniques are generally considered limited in their ability to determine laterality due to bilateral afferent activation and shared sacral reflex circuitry. An 85-year-old male with progressive lower extremity weakness and worsening urinary incontinence underwent resection of a thoracic epidural tumor and separate sacral tumor involving the left S2 nerve root. BCR responses we

The Usefulness of Deep Tendon Reflexes in the Diagnosis of Lumbar Spine Diseases: A Narrative Review.

The deep tendon reflex (DTR) is a more objective indicator than sensory and muscle assessments for lumbar spine disorders. Further, unlike sensory and muscle assessments that require patient cooperation, the DTR can be assessed even in patients with impaired consciousness or cognition. Therefore, DTR assessment with a hammer is an essential neurological test for lumbar spinal diseases. However, despite the usefulness of DTR assessment, few reports have described the significance of increased, diminished, or absent deep lower extremity reflexes in lumbar spine diseases. This review outlines the

Autonomic asymmetry in migraine: augmented parasympathetic activation in left unilateral migraineurs.

Brain autonomic control is asymmetrical, the left hemisphere affecting predominantly parasympathetic function and the right hemisphere affecting predominantly sympathetic function. It is not known whether the extent of autonomic activation is altered in migraine, although the fact that some migraineurs express parasympathetic features such as facial flushing, lacrimation and rhinorrhoea might suggest increased parasympathetic activation. We instilled diluted soapy eyedrops and measured (i) the trigemino-parasympathetic reflex by the vasodilator response of forehead skin bilaterally using photo

[Pathological jaw opening reflex after whiplash injury].

The brainstem mediated inhibitory reflex of the m. temporalis was investigated on average 3 days after the accident in 61 patients (f = 36; m = 25; mean age = 28 years +/- 9 SD) who presented an acute cervico-cephalic syndrome with posttraumatic headache and neck pain following acute whiplash injury but without neurological deficits, bone injury of the cervical spine or combined direct head trauma. We postulated a disturbed reflexive temporalis muscle inhibition in patients with 'whiplash cephalalgia', as has previously been reported for classical muscle tension headache. Latencies and duratio

The effect of hand dominance on results in the Functional Head Impulse Test.

OBJECTIVE: The vestibulo-ocular reflex is essential for maintaining visual stability during head movements. Individual neurophysiological characteristics, such as hand dominance, may influence vestibular responses through hemispheric lateralisation. This study aimed to investigate the effect of hand dominance on functional vestibulo-ocular reflex performance using the Functional Head Impulse Test. METHODS: Fifty-eight healthy volunteers were classified as right- or left-handed using the Edinburgh Handedness Inventory. Functional Head Impulse Test reading scores were obtained during passive ri

Positive ice test in a patient with a lymphomatous infiltration of the levator palpebrae superioris.

The ice test is known to be specific for the diagnosis of ocular myasthenia gravis in patients with ptosis, with reported specificities of 97%-100%. We report a patient with diffuse large B-cell lymphoma who showed a positive result to the ice test. A 53-year-old woman presented with left eyelid ptosis of 3 weeks' duration. She had finished chemotherapy and radiotherapy for diffuse large B-cell lymphoma a year previously. On ophthalmological examination, her best-corrected visual acuity was 20/20 in each eye. Marginal reflex distances were +4 in the right eye and -1 in the left eye. Levator pa

Isolated Medial Longitudinal Fasciculus Midbrain Infarction Mimicking Medial Rectus Paralysis.

INTRODUCTION: Medial longitudinal fasciculus infarction is rare in clinical practice and generally accompanied by brain tissue damage around the medial longitudinal fasciculus. Isolated medial longitudinal fasciculus midbrain infarction was seldom reported. CASE REPORT: An 81-year-old man with hypertension was admitted to our hospital because of sudden onset diplopia. A neurological examination revealed right adduction paresis without abducting nystagmus in the left eye, whereas the convergence reflex was normal. Diffusion-weighted magnetic resonance imaging demonstrated a small acute lacunar

Cervical vestibular-evoked myogenic potentials in patients with essential tremor.

Although different neuroanatomical structures and pathways are emphasized as possible explanations for essential tremor (ET), there is still an ongoing debate. This study aimed to assess the role of brainstem and reflex pathways with cervical vestibular-evoked myogenic potentials (VEMP) in patients with ET. This prospective study included 34 patients with ET and 25 healthy controls. Cervical VEMP was performed in both groups and latencies, inter-peak latency intervals, peak-to-peak amplitudes and asymmetry ratios were recorded. There was statistically no significant difference between the grou

[A case of HNPP presenting with late onset of symptoms, chronic progressive course and sensory impairment in face and trunk].

A 69-year-old man was admitted with a chief complaint of progressive muscle weakness and tingling sensation in the distal portion of all extremities since the age of 55. On neurological examination, sensory impairment of all modalities was noted in the right side of face, segmental areas of the right Th4-10 and the distal portion of four extremities. Symmetrical muscle weakness and atrophy was also found over the distal portion of all extremities. All deep tendon reflexes except biceps brachii reflexes were absent. Neurophysiological studies, however, rather indicated mononeuritis multiplex in

Asymmetry in jaw-jerk reflex latency in craniomandibular dysfunction patients with unilateral masseter pain.

The purpose of the present study was to elucidate the effects of unilateral masseter muscle pain on the jaw-jerk reflex. The latency and peak-to-peak amplitude of bilateral electromyographic activity recorded at the masseter muscles during the jaw-jerk reflex were measured in 18 patients with craniomandibular dysfunction (CMD) with strictly unilateral masseter pain or tenderness and 10 control subjects using a computerized recording and analysis system. The reflex was elicited, at the mandibular rest position, by tapping the centre of the chin downwards with a reflex hammer incorporating a mic

Spatiotemporal aspects of sympathetic C-fiber afferent activity in pressor reflex during abdominal ischemia.

Activation of abdominal sympathetic visceral afferents during ischemia elicits excitatory cardiovascular reflexes. The present study examined the time course and discharge patterns of activation of ischemically sensitive sympathetic C-fiber afferents, and then the relationship between summated afferent activity and the pressor reflex induced by prolonged abdominal ischemia was determined. Single-unit activity of abdominal C-fiber afferents was recorded from the right thoracic sympathetic chain of anesthetized cats during 30 min of ischemia. The reflex pressor response to abdominal ischemia was

Skin and not dorsal root stimulation reduces hypertonus in thoracic motor complete spinal cord injury: a single case report.

On demand and localized treatment for excessive muscle tone after spinal cord injury (SCI) is currently not available. Here, we examine the reduction in leg hypertonus in a person with mid-thoracic, motor complete SCI using a commercial transcutaneous electrical stimulator (TES) applied at 50 or 150 Hz to the lower back and the possible mechanisms producing this bilateral reduction in leg tone. Hypertonus of knee extensors without and during TES, with both cathode (T11-L2) and anode (L3-L5) placed over the spinal column (midline, MID) or 10 cm to the left of midline (lateral, LAT) to only acti
