Diaphragmatic Palsy Due to a Paraneoplastic Autoimmune Syndrome Revealed by Checkpoint Inhibitors.

Background and Clinical Significance: Immune checkpoint inhibitors (ICIs) have revolutionized cancer treatment but may underlie diverse and potentially life-threatening immune-related adverse events (irAEs). They may cause various conditions leading to respiratory failure, including myasthenic syndromes and myositis. However, diaphragmatic paralysis (DP) has rarely been reported. To describe patients with diaphragmatic paralysis in a pharmacovigilance registry, we searched the prospective REISAMIC registry at the Gustave Roussy Cancer Center (Villejuif, France) for cases of diaphragmatic palsy

Diaphragmatic Nerve Paralysis After Redo Aortic Valve Replacement That Improved Over Time and Led to Successful Ventilator Weaning: A Case Report.

We report a 75-year-old female with a history of two heart operations: aortic valve replacement (St. Jude MedicalTM 21 mm) at the age of 44 years for severe rheumatic aortic stenosis and mitral valve replacement (CarbomedicsTM 29 mm) at the age of 51 years for rheumatic mitral regurgitation. Decades later, she presented with exertional dyspnea. Echocardiography revealed aortic stenosis with an effective orifice area of 0.79 cm². Coronary angiography showed #6 75% stenosis and a limited mechanical valve opening. After a thorough discussion, the patient agreed to undergo redo surgery. The surger

Recurrent laryngeal nerve transection in mice results in translational upper airway dysfunction.

The recurrent laryngeal nerve (RLN) is responsible for normal vocal-fold (VF) movement, and is at risk for iatrogenic injury during anterior neck surgical procedures in human patients. Injury, resulting in VF paralysis, may contribute to subsequent swallowing, voice, and respiratory dysfunction. Unfortunately, treatment for RLN injury does little to restore physiologic function of the VFs. Thus, we sought to create a mouse model with translational functional outcomes to further investigate RLN regeneration and potential therapeutic interventions. To do so, we performed ventral neck surgery in

Acute segmental poliomyelitis-like flaccid paralysis in an adult in the UK, associated with enterovirus D68.

Enterovirus D68 has been associated with a poliomyelitis-like illness, notably during an outbreak in 2014, and particularly affecting children in the USA. We report a case of acute segmental flaccid paralysis with respiratory involvement in an adult in the UK, with enterovirus D68 detected in a sputum sample. MR imaging of cervical spinal cord showed a longitudinally extensive T2 hyperintensity in the anterior cord. Cerebrospinal fluid showed an elevated white cell count, predominantly lymphocytic, with otherwise normal constituents and negative viral PCRs. His respiratory function improved af

Periodic paralysis: An unusual presentation of drug-induced hyperkalemia.

Hyperkalemia is a life-threatening electrolyte abnormality. The most common cause of hyperkalemia includes renal disease and ingestion of medications. Drug-induced hyperkalemia may develop in patients with underlying renal impairment, disturbed cellular uptake of potassium load, excessive ingestion or infusion of potassium-containing substances. We report a case of "drug-induced severe hyperkalemia" presenting as periodic paralysis. A 67-year-old diabetic and hypertensive woman presented to emergency department with the complaint of intermittent episode of inability to walk for the past 5 days

Kaposi sarcoma with laryngeal, tracheal, and parenchymal involvement.

The exclusive respiratory involvement by Kaposi sarcoma is rare, especially in the upper airway. When present, it could lead to severe airway compromise. We present the case of a human immunodeficiency virus infection-positive male patient who presents with dyspnea and stridor, and was found to have bilateral vocal cord paralysis and stenosis of the upper airway due to laryngeal and tracheal involvement by Kaposi sarcoma. The patient underwent bronchoscopy for both diagnostic and therapeutic intentions.

Inspiratory pressure threshold training in a case of congenital bilateral abductor vocal fold paralysis.

We present a non-surgical treatment option to decrease symptoms of dyspnea in a 6 year-old child with congenital bilateral abductor vocal fold paralysis. A respiratory muscle strength-training program was used to strengthen her inspiratory muscles for 8 months, 3 to 5 days per week. Inspiratory muscle strength increased over the course of training, resulting in reported decreases in dyspnea by both the child and parents during speech and exercise.

Laryngeal Surgery as It Pertains to the Thoracic Surgeon.

A surgeon managing complex airway disease benefits from knowledge of laryngeal physiology and pathology. The Larynx provides phonation and protection for the lower airway via closure and cough reflex. Disruption of these functions or development of paralysis or stenosis can lead to respiratory failure. Management includes a variety of endoscopic and open management techniques. Understanding of laryngeal physiology can also limit laryngeal functional impairment through careful preservation of motor nerves and early recognition of injuries.

Puffer fish poisoning: a potentially life-threatening condition.

Puffer fish poisoning has been documented rarely in Australia. It results from ingesting tetrodoxtoxin found in the liver, ovaries, intestines and skin of the fish. Over a recent 16-month period, 11 cases of puffer fish poisoning were reported to the NSW Poisons Information Centre. Symptoms of poisoning may include paralysis, respiratory failure, numbness, paraesthesia, nausea and ataxia. Health professionals should be aware of the condition so as to institute early and appropriate management.

[Surgical treatment of the bilateral abductor paralysis].

27 patients with bilateral vocal cord paralysis were treated in the First Clinical Medical College of Beijing Medical University, and all of them were followed up at least one year after treatment. According to the length of the vocal cord and the degree of the anterior angle of the thyroid cartilage, one of the three different surgical procedures would be selected to alleviate the symptoms of bilateral abductor paralysis. The width of glottic chink after treatment is the paramount index. The average estimated size of the posterior glottic chink or point of maximum width of glottic chink shoul

Effects of interpleural bupivacaine (0.5%) on canine diaphragmatic function.

Several authors have questioned the potential for phrenic nerve paralysis with interpleural analgesia. This study was designed to examine the potential for phrenic nerve paralysis with the use of interpleural bupivacaine in dogs. Seven dogs were anesthetized, tracheally intubated, and allowed to breathe spontaneously with halothane/oxygen while in the supine position. After a midline laparotomy, two wires were inserted into the costal portion of each hemidiaphragm for measurement of electromyographic (EMG) signals. A balloon catheter was placed in the abdominal cavity to measure abdominal pres

Diaphragmatic paralysis caused by cervical spondylosis. Case report.

The authors describe a rare case of diaphragmatic paralysis caused by cervical spondylosis. A 64-year-old man presented with dyspnea as well as cervical radicular pain and left-sided upper-extremity motor weakness. Chest radiography revealed elevation of both sides of the diaphragm. All symptoms were ameliorated immediately after cervical laminoplasty, and spirometry revealed improvement of ventilatory function 6 months after surgery. Cervical spondylosis should be considered a factor that can cause respiratory dysfunction.

Xylazine-induced attenuation of dorsal displacement of the soft palate associated with epiglottic dysfunction in a horse.

An 18-month-old Quarter Horse filly was evaluated because of dorsal displacement of the soft palate associated with epiglottic dysfunction that caused exercise intolerance and an abnormal respiratory noise. The abnormality of the epiglottis was corrected by use of a sedative dose of xylazine hydrochloride. There was no familial predisposition to hyperkalemic periodic paralysis, which may cause similar laryngopharyngeal signs, and the horse did not respond to administration of acetazolamide. There was no known history of trauma that could have caused neuritis, which has also been suggested as a

A cluster of acute flaccid paralysis and cranial nerve dysfunction temporally associated with an outbreak of enterovirus D68 in children in Colorado, USA.

BACKGROUND: Clusters of acute flaccid paralysis or cranial nerve dysfunction in children are uncommon. We aimed to assess a cluster of children with acute flaccid paralysis and cranial nerve dysfunction geographically and temporally associated with an outbreak of enterovirus-D68 respiratory disease. METHODS: We defined a case of neurological disease as any child admitted to Children's Hospital Colorado (Aurora, CO, USA) with acute flaccid paralysis with spinal-cord lesions involving mainly grey matter on imaging, or acute cranial nerve dysfunction with brainstem lesions on imaging, who had on

[Utility of heliox during treatment of upper airway obstruction secondary to bilateral vocal cord paralysis after thyroidectomy].

Helium is a noble gas whose low density decreases airway resistance. This property is utilized when a mixture of helium and oxygen (heliox) is employed in certain clinical situations, particularly in the context of airway obstruction. We report the case of a woman with severe upper airway obstruction due to bilateral vocal cord paralysis after thyroidectomy. Heliox was used temporarily to reduce respiratory effort and avoid the need for tracheal intubation while the obstruction was being treated with antiinflammatory drugs.

Respiratory failure and lethal hypotension due to blue-ringed octopus and tetrodotoxin envenomation observed and counteracted in animal models.

The effects of crude blue-ringed octopus venom gland extract and tetrodotoxin (TTX) on anaesthetised rats and rabbits were studied. Paralysis of the respiratory musculature causing anoxia and cyanosis was overcome with positive, artificial respiration. The second lethal mechanism of the toxins: rapid and severe hypotension, had to be counteracted peripherally, since neural transmission had been drastically reduced by the toxins. Noradrenaline, d-amphetamine, phenylephrine and methoxamine, agonists acting on vascular adrenergic a-receptors, were tested.
