Medial border of the perirenal space: CT and anatomic correlation.

PURPOSE: To explore the mode of spread of disease between the perirenal space and the perivascular central retroperitoneum and to determine the anatomy along the medial border of the perirenal space. MATERIALS AND METHODS: Anatomic dissection, injection of latex, and observation of cross sections of the abdomen were performed in nine cadavers. Attention was paid to the juncture of the central prevertebral, perivascular, and extraperitoneal regions, and the perirenal space. Anatomic findings were correlated with observations made at computed tomography (CT) in 82 patients with retroperitoneal

Paraganglioma associated with hypoglycemia.

A 63-year-old female had frequent attacks of palpitations with sweating and occasional loss of consciousness for 7 years before admission. These symptoms and documented hypoglycemia (blood glucose: 46 mg/dl) suggested insulinoma, but computed tomography demonstrated a retroperitoneal tumor which was diagnosed as a paraganglioma by histological examination of the resected specimen.

Sinus histiocytosis with massive lymphadenopathy. Occurrence in identical twins with retroperitoneal disease.

Sinus histiocytosis with massive lymphadenopathy (SHML) was originally defined as a relatively specific benign pseudolymphomatous disorder. Although the etiology remains unknown, the spectrum of SHML has been expanded to include predominance of extranodal disease in some patients, clinically significant immunologic abnormalities in 10% of patients, and fatal outcome in 7% of patients. We report the rare occurrence of SHML in identical twins; to our knowledge, SHML in identical twins has been reported only once previously. The two patients described are also unusual because of the predominance

Computed tomography-guided percutaneous biopsy: a combined approach to the retroperitoneum.

When computed tomography (CT) is used to evaluate the lumbar spine in patients with low back pain, it is important to look at the adjacent retroperitoneum. CT is ideally suited to visualize the retroperitoneum and provide access for percutaneous needle biopsy.

Retroperitoneal fat necrosis producing ureteral obstruction.

We present 3 cases of a rare syndrome of extrinsic ureteral obstruction produced by retroperitoneal fat necrosis and compare the medical records and histopathology to 19 cases of classic idiopathic retroperitoneal fibrosis. Clinical similarities include average age of onset in the fourth and fifth decades, male preponderance, abdominal or back pain as the presenting symptom, elevation of the blood urea concentration prior to diagnosis and tendency toward bilateral involvement. Histologically, the 2 conditions were different. Retroperitoneal fat necrosis is characterized by coalescence of fat c

An interdisciplinary approach for laparoscopic removal of a large retroperitoneal pelvic schwannoma attached to vital vessels: A case report.

RATIONALE: Retroperitoneal schwannomas are very rare and may grow very close to major abdominal vessels. Since the surgical approach to the retroperitoneal space may be complex due to surrounding vital organs, including major vessels, laparoscopic surgery is challenging and has only been recently adopted. Here, we report a case of laparoscopic resection of a large retroperitoneal schwannoma attached to large vital vessels. PATIENT CONCERNS: A 62-year-old woman presented with a chief complaint of pain in the lower right limb with consequent claudication, which had lasted for approximately 1 ye…

Imaging features and metastatic pattern of non-IVC retroperitoneal leiomyosarcomas: are they different from IVC leiomyosarcomas?

PURPOSE: The purposes of this study were to describe the imaging features and metastatic pattern of non-inferior vena cava (IVC) retroperitoneal leiomyosarcomas (non-IVC LMS) and to compare them with those of IVC leiomyosarcomas (IVC LMS) to assess any differences between the 2 groups. MATERIALS AND METHODS: In this institutional review board-approved, Health Insurance Portability and Accountability Act-compliant retrospective study, all 56 patients with pathologically confirmed primary retroperitoneal leiomyosarcoma (34 non-IVC LMS and 22 IVC LMS) seen at our tertiary cancer center during a

Recurrent liposarcomas of the abdomen and retroperitoneum: three case reports.

We report three cases of patients with liposarcomas of the abdomen who had been treated during the last 13 years (1980-1993). Two patients were men, aged 29 and 51 years, with tumors of the retroperitoneal space and the third patient was a woman aged 64 years with a tumor in the peritoneal cavity. Therapeutic treatment was as aggressive as possible excision of the tumor. In the case of the first male patient, the histological examination revealed a retroperitoneal myxoid liposarcoma which recurred 5 times within 13 years. In the second male patient, it revealed a well differentiated retroperit…

Cullen's sign and haemoglobinuria as presenting signs of retroperitoneal haemorrhage in a dog.

Haemoglobinuria and periumbilical discoloration (also known as Cullen's sign) are clinical signs uncommonly reported in veterinary patients. … ut the anaesthetic period the dog was hypertensive and a large haematoma originating from the right retroperitoneal space was identified at surgery. The cause of the haemorrhage was uncertain but a ruptured phaeochromocytoma was thought possible on the basis of the persistent hypertension and the location of the haemorrhage.

Video-assisted retroperitoneal debridement for infected retroperitoneal fat necrosis following laparoscopic distal pancreatectomy.

Video-assisted retroperitoneal debridement (VARD) is an established and minimally invasive treatment option for infected necrotizing pancrea … fluid collection around the pancreatic stump with rapid extension of inflammatory changes into the retroperitoneal space. Conservative management with fasting and antibiotic therapy resulted in temporary improvement in inflammatory markers. However, extensive infected retroperitoneal fat necrosis developed on POD 38. On POD 43, CT-guided percutaneous catheter drainage …

Surgical management of retroperitoneal schwannoma complicated with severe hydronephrosis: A case report.

RATIONALE: Schwannomas are usually benign tumors arising from well-differentiated schwann cells, which rarely occur in the retroperitoneal space. The lack of specific signs and radiologic imaging characteristics makes preoperative diagnosis rather difficult. Most retroperitoneal schwannomas are benign and the primary treatment choice for retroperitoneal schwannomas is surgical excision, however, the involvement of the urinary system is scarcely reported. PATIENT CONCERNS: A 34-year-old woman presented with progressive left abdominal pain and rebound abdominal mass at the left lower quadrant f…

Retroperitoneal bile leak after laparoscopic cholecystectomy.

Bile duct injury (BDI) is a well-recognised complication of laparoscopic cholecystectomy (LC). Following a BDI, bile usually leaks into the peritoneal space and causes biliary peritonitis. This manifests as non-specific abdominal pain and fever occurring several days after the surgery. It can be managed by laparoscopic washout with or without bile duct repair. We present a rare case of retroperitoneal bile leak post-LC. The mechanism of injury here was likely partial avulsion from excessive traction of the cystic duct during intraoperative cholangiogram. Diagnosing retroperitoneal bile leak ca

[Soft tissue emphysema as manifestation of perforating diverticulitis].

Pronounced soft-tissue emphysema expressing disseminated fecal abscess formation in perforated colonic diverticulitis is described. Cervical, mediastinal, and thoracic dissemination and involvement of the scrotal space and the thigh reveal the original retroperitoneal localization as well as the unusual but potential dissemination. The diagnostic methods are described.

[Retroperitoneal fibrosis--possible cause of hypertension].

The purpose of this study is to show the case of idiopathic retroperitoneal fibrosis, which clinically manifests itself as hypertension. The property of this disease described for the first time by Ormond is pathological deposition of connective tissue in retroperitoneal space. In this particular case hypertension develops as the result of renal hypoperfusion and subsequent activation of the renin-angiotensin-aldosterone system, caused by hydronephrosis and compression of renal vessels by pathologic tissue. The early diagnosis based on computer tomography and magnetic resonance provided the po…
