The choice of therapy in medically intractable trigeminal neuralgia.

Trigeminal neuralgia stands out as one of the few severe neuropathic pain states for which successful treatment options are available. Up to one-half of patients become refractory to medical therapy over time. At this point, the question of which intervention is appropriate becomes an important issue. A personal series of 150 patients treated between 1988 and 1995 is reviewed. Patients under 65 years and in good general health underwent surgery for microvascular decompression of the trigeminal root; in the remaining 120 patients glycerol was injected percutaneously into the trigeminal cistern.

Endoscopic Microvascular Decompression for Vagoglossopharyngeal Neuralgia.

Glossopharyngeal neuralgia (GN) is a nerve compression syndrome that presents with episodes of unilateral sharp, stabbing pain in the distribution of the ninth cranial nerve. This syndrome may present with cardiac and autonomic manifestations - a condition termed vagoglossopharyngeal neuralgia (VGPN). Most cases of VGPN arise from neurovascular insult at the cerebellopontine angle. Conservative treatment for VGPN includes antiepileptic medications. Surgical treatments include trigeminal tractotomy-nucleotomy, Gamma Knife® stereotactic radiosurgery, radiofrequency thermocoagulation, rhizotomy, …

Cerebral Palsy: An Overview.

Cerebral palsy, which occurs in two to three out of 1,000 live births, has multiple etiologies resulting in brain injury that affects moveme … clude intramuscular onabotulinumtoxinA, systemic and intrathecal muscle relaxants, selective dorsal rhizotomy, and physical and occupational therapies. Patients with cerebral palsy often also experience problems unrelated to movement that need to be managed into adulthood, including cognitive dysfunction, seizures, pressure ulcers, osteoporosis, behavioral …

Comprehensive study of diagnosis and treatment of trigeminal neuralgia secondary to tumors.

Among 5,058 patients seen at the Mayo Clinic from 1976 through 1990 for face pain, we diagnosed trigeminal neuralgia in 2,972. Tumors were c … wever, temporarily or permanently blocking afferent impulses with radiofrequency ablation, glycerol rhizotomy, or alcohol blocks was a good alternative to craniotomy.

EMG changes in rat hind limb muscles following bilateral deafferentation.

Bilateral section of dorsal roots was performed in 9 adult rats in order to ascertain whether the tendency to extension, the appearance of s … MG response to stretch of the deafferented SOL frequently appeared 1-2 weeks after bilateral dorsal rhizotomy. The performance of movement was highly atactic after bilateral deafferentation. However, the basic locomotor EMG pattern persisted, although simultaneous activation of homonymous muscles was also occasionally recorded.

Volume maximised glycerol rhizolysis for trigeminal neuralgia: a single centre analysis of outcomes.

OBJECTIVES: Glycerol rhizotomy is an established treatment for medically refractory trigeminal neuralgia in select cases where microvascular decompression is contraindicated or not preferred. The standard approach is to inject a fixed volume of glycerol using Hartel's technique into Meckel's cave. We discuss a 'volume-maximised' technique of measuring the volume of Meckel's cave using intra-operative fluoroscopy and injecting an equivalent volume of glycerol such that every patient receives a tailored quantity of glycerol dependent on the volume of Meckel's cave. The safety and efficacy of thi…

Other surgical interventions.

Optimal management of patients with chronic neuropathic pain requires a multidisciplinary approach that may include surgery. Yet despite the fact that lumbosacral spinal surgery, for example, is performed in thousands of patients every year, there is very little controlled clinical data to support its use or that of other surgical techniques in the treatment of chronic nonmalignant pain, especially neuropathic pain. Nevertheless, there is evidence of some success for ablative techniques such as dorsal root entry zone lesioning for phantom limb pain and girdle-zone neuropathic pain, and sympath

[Sacral deafferentation and neurostimulation of anterior spinal roots in the treatment of neurogenic bladder in patients with complete transverse spinal lesions--initial clinical experience].

Of those patients who survived the first years after traumatic severing of the spinal cord 85% die from urological complications, their life span being significantly reduced unless they are adequately treated. Sacral deafferentation and electrostimulation of the urinary bladder is in recent years one of the most effective therapeutic methods of neuropathic urinary bladder caused by suprasacral transverse spinal lesions: rhizotomy permanently increases the compliance of the bladder, eliminates hyperreflexion of the detrusor and detrusor-sphincteric dyssynergy--the main causes of nephropathy and…

Surgical and anatomical nuances in trigeminal neuralgia due to cerebellopontine angle lipoma: illustrative case.

BACKGROUND: Trigeminal neuralgia (TN) is a highly debilitating condition often caused by neurovascular compression. Less commonly, cerebellopontine angle (CPA) tumors can be the culprit, while intracranial lipomas are an exceedingly rare etiology. Treatment options include medical management and surgical interventions ranging from neurovascular decompression alone to mechanical rhizotomy and tumor resection. OBSERVATIONS: The authors present the case of a 30-year-old woman with medically refractory TN found to have a CPA lipoma abutting the root entry zone of the trigeminal nerve. A decision …

Role of cranial and spinal virtual and augmented reality simulation using immersive touch modules in neurosurgical training.

Recent studies have shown that mental script-based rehearsal and simulation-based training improve the transfer of surgical skills in variou … actual procedure. Modules available include ventriculostomy, bone drilling, percutaneous trigeminal rhizotomy, and simulated spinal modules such as pedicle screw placement, vertebroplasty, and lumbar puncture. We present our experience with the development of such augmented reality neurosurgical modules and the feedback from neurosurgical residents.

Use of a self-designed bladder controller for restoring bladder function in paraplegic dogs.

OBJECTIVE: To evaluate the results of a self-designed bladder controller for restoring micturition function in paraplegic dogs. METHODS: The spinal cords of 4 dogs were transected above the cone. Electrodes were implanted in S2 bilaterally and connected to the subcutaneous receivers for external activation. Microsurgical technique was employed to perform dorsal rhizotomy of S1-3 intradurally. The dogs were stimulated daily to observe micturition. Urodynamic testing and vesicography were performed. RESULTS: All the dogs acquired micturition under the control of electric stimulation, with urin…

Neurosurgical treatment of spasticity: a potential return to the cerebellum.

OBJECTIVE: Neurosurgical targeting of the cerebellar dentate nucleus via ablative dentatotomy and stimulation of the dentate nucleus was historically used for effective treatment of spasticity. Yet for decades, neurosurgical treatment of spasticity targeting the cerebellum was bypassed in favor of alternative treatments such as intrathecal baclofen pumps and selective dorsal rhizotomies. Cerebellar neuromodulation has recently reemerged as a promising and effective therapy for spasticity and related movement disorders. METHODS: In this narrative review, the authors contextualize the historica

The postoperative course was uneventful and pain was relieved in the first case and decreased in the second. In VGPN, a tethered arachnoid may play a role in causing the neuralgia, either alone or associated with a neuro-vascular conf …
