Feasibility of "cold" triangle robotic pancreatoduodenectomy.

BACKGROUND: Triangle pancreatoduodenectomy adds to the conventional procedure the en bloc removal of the retroperitoneal lympho-neural tissue included in the triangular area bounded by the common hepatic artery (CHA), the superior mesenteric artery (SMA), and the superior mesenteric vein/portal vein. We herein aim to show the feasibility of "cold" triangle robotic pancreaticoduodenectomy (C-Tr-RPD) for pancreatic cancer (PDAC). METHODS: Cold dissection corresponds to sharp arterial divestment performed using only the tips of robotic scissors. After division of the gastroduodenal artery, trian

Robotic applications for intracardiac and endovascular procedures.

The large incisions and long recovery periods that accompany traditional cardiac surgery procedures along with the constant patient demand for minimally invasive procedures have motivated cardiac surgeons to implement the robotic technologies in their armamentarium. The robotic systems have been utilized successfully in various cardiac procedures including atrial septal defect repair, left atrial myxoma resection, MAZE procedure and left ventricular lead placement, yet coronary artery bypass and mitral valve repair still comprise the vast majority of them. This review analyzes the development

Robotic repair of non-midline hernias.

Non-midline abdominal wall hernias present unique anatomic challenges, making repair more complex. The constraints of the peritoneal cavity, pelvis, and costal margin limit the utility of intraperitoneal mesh repair, and extra-peritoneal repairs have traditionally been performed using open techniques, often resulting in higher wound morbidity. Advances in minimally invasive surgery make visualization and dissection of such complex cases feasible, with all the attendant benefits of a minimally invasive over an open approach. In this study, we examined the use of the robotic platform to repair n

A comparison of laparoscopic and robotic ergonomic risk.

INTRODUCTION: Work related injuries in minimally invasive surgery (MIS) are common because of the strains placed on the surgeon's or assistant's body. The objective of this study was to compare specific ergonomic risks among surgeons and surgical trainees performing robotic and laparoscopic procedures. MATERIALS AND METHODS: Ergonomic data and discomfort questionnaires were recorded from surgeons and trainees (fellows/residents) for both robotic and laparoscopic procedures. Perceived discomfort questionnaires were recorded pre/postoperatively. Intraoperatively, biomechanical loads were captur

Neuroenhancement of surgeons during robotic suturing.

BACKGROUND: The initial phases of robotic surgical skills acquisition are associated with poor technical performance, such as low knot-tensile strength (KTS). Transcranial direct-current stimulation (tDCS) can improve force and accuracy in motor tasks but research in surgery is limited to open and laparoscopic tasks in students. More recently, robotic surgery has gained traction and is now the most common approach for certain procedures (e.g. prostatectomy). Early-phase robotic suturing performance is dependent on prefrontal cortex (PFC) activation, and this study aimed to determine whether pe

Improved Integrated Robotic Intraocular Snake.

Retinal surgery can be performed only by surgeons possessing advanced surgical skills because of the small, confined intraocular space, and the restricted free motion of instruments in contact with the sclera. Snake-like robots could be essential for use in retinal surgery to overcome this problem. Such robots can approach from suitable directions and operate delicate tissues when performing retinal vein cannulation, epiretinal membrane peeling and so on. In this study, we propose an improved integrated robotic intraocular snake (I2RIS), which is a new version of our previous IRIS. This update

Falcon: A False Ceiling Inspection Robot.

Frequent inspections are essential for false ceilings to maintain the service infrastructures, such as mechanical, electrical, and plumbing, and the structure of false ceilings. Human-labor-based conventional inspection procedures for false ceilings suffer many shortcomings, including safety concerns. Thus, robot-aided solutions are demanded for false ceiling inspections similar to other building maintenance services. However, less work has been conducted on developing robot-aided solutions for false ceiling inspections. This paper proposes a novel design for a robot intended for false ceiling

History and Acceptance of Transoral Robotic Surgery.

The development and acceptance of transoral robotic surgery from an experimental procedure to widespread acceptance in the management of head and neck cancers and other disease states occurred over the course of about a decade, from 2005 to 2015. Transoral robotic surgery has cemented its' place in the treatment of pharyngeal and laryngeal cancer. Education and training was key to broad use and acceptance. This article traces the history and evolution of transoral robotic surgery to its current practice. The process of surgical innovation in this arena is followed from early cadaveric studies

Robotic resection of a large ovarian leiomyoma.

Uterine leiomyomata are the most frequent leiomyomata. However, this benign tumor can also arise in unusual locations (e.g. ovaries, urethra, bladder and vulva). The diagnostic process of ovarian leiomyomata can be difficult, and it is mostly incidental: the lesions are usually small and the clinical presentation is mainly asymptomatic. Since the first case described in 1862, less than 100 cases have been described. Some of these lesions have been treated laparoscopically. To our knowledge, this is the first worldwide case of an ovarian leiomyoma treated with a robotic approach (surgeon: Prof.

Specific complications and limitations of robotic esophagectomy.

Regardless of the approach to esophagectomy, it is an operation that may be associated with significant risk to the patient. Robotic-assisted minimally invasive esophagectomy (RAMIE) has the same potential for short- and long-term complications as does open and minimally invasive esophagectomy. These complications include among others, the risk for anastomotic leak, gastric tip necrosis, vocal cord palsy, and chylothorax. Moreover, there are additional risks that are unique to the robotic platform such as hardware or software malfunction. These risks are heavily influenced by numerous factors

[Robot-assisted Mediastinal Mass Resection].

In recent years, robot-assisted thoracic surgery is gaining more and widespread interest in Europe. Due to the narrow space and the complexity of anatomical structures, conventional minimally invasive mediastinal surgery may be challenging for the thoracic surgeon. Robot-assisted mediastinal surgery opens up new possibilities for minimally invasive surgery, as it permits greater dexterity, a three-dimensional view, and tremor adjustment, which allows the surgeon to perform complex procedures in small thoracic spaces. As robotic platforms continue to evolve, more complex mediastinal thoracic su

Robotic approach to a subcarinal functional paraganglioma.

INTRO: Functional mediastinal paragangliomas arise from extra-adrenal tissues and are rare. These cases create challenges related to diagnosis, peri-operative management, and surgical management. We present a case that demonstrates a planned robot-assisted thoracoscopic resection of a mediastinal paraganglioma that ultimately required a trans-sternal resection of the tumor off the left atrium. CASE REPORT: Our patient is a 42-year-old male with a prolonged history of refractory hypertension, palpitations, headaches, and diaphoresis, which led to the discovery of a subcarinal functional medias

ROBOTIC PARAGANGLIOMA RESECTION IN A PREGNANT PATIENT.

OBJECTIVE: Functional paragangliomas in pregnancy are rare; however, if not recognized and treated early, they can be life-threatening. New treatment approaches with robotic resection are promising. METHODS: We present a case of a pregnant female with a paraganglioma which was successfully resected during the second trimester using Da Vinci Xi robotic system. Paraganglioma was diagnosed with plasma and urinary catecholamines and metanephrines, a contrast computed tomography (CT) scan, and confirmed with surgical pathology. RESULTS: The patient was initially seen in the emergency room prior t

Robotic Lower Urinary Tract Reconstruction.

Lower urinary tract reconstruction has traditionally been approached in an open fashion but select complications or disease processes may be suitable for robotic reconstruction, including bladder neck contractures, proximal urethral strictures, and genitourinary fistulas. Here, the authors discuss the novel techniques used and the feasibility of robotic reconstruction for these conditions. The robotic approach is relatively novel, and more data and studies will be required to make definitive statements regarding success rate and complications from the procedure. Preliminary data suggest that t

Robotic Ureteral Reconstruction in the Pediatric Population.

Robot-assisted laparoscopic (RAL) surgery is a safe, minimally invasive technique that has become more widely used in pediatric urology over recent decades. With several advantages over standard laparoscopy, robotic surgery is particularly well-suited to reconstructive surgery involving delicate structures like the ureter. A robotic approach provides excellent access to and visualization of the ureter at all levels. Common applications include upper ureteral reconstruction (e.g., pyeloplasty, ureteropelvic junction polypectomy, ureterocalicostomy, and high uretero-ureterostomy in duplex system
