Former substance users working as counselors. A dual relationship.

All helping professionals risk participation in "dual relationships." But in the case of former substance users working as counselors, specific dilemmas and problems are accentuated. A qualitative analysis highlights some of the ethical and personal dilemmas faced by these counselors. The data are derived from an interview study initiated in 2000 in Denmark on former substance users with 4-8 years of abstinence. Through an analysis of interview data from a larger group of former substance users, it became evident that those working as counselors experienced specific dilemmas and problems. The

Changing from oral to enteral feeding: impact on families of children with disabilities.

The number of children in the U.K. who are being fed enterally at home is increasing. Feeding by nasogastric or gastrostomy tube may not seem particularly abnormal to nurses who care for sick children. However, the initiation of tube feeding can have a significant impact on both the child and the family. Research related to children with disabilities who have feeding problems is focused around the views of their mothers because they are often the main carers of these children. However, eating is a social activity in our society and tube feeding will have an effect on other members of the famil

Scapegoating and the unpopular nurse.

This paper considers the theories of scapegoating and their application to nursing. It reviews the literature on scapegoating drawing particular attention to those studies which have emphasised on the instrumental uses of scapegoating in organisational contexts. The paper draws on data from a qualitative study of the identification and management of the 'problem' nurse conducted in three healthcare Trusts. It describes a number of illustrative cases in which different types of scapegoating occurred. The paper argues that scapegoating is an ethical challenge which nurses must face.

Nurses' views of factors that help and hinder their intrapartum care.

OBJECTIVE: To explore labor and delivery nurses' views of intrapartum care, particularly factors that help or hinder their efforts to provide professional labor support. DESIGN: Content analysis of narrative comments that nurses wrote on questionnaires during a two-part research study on professional labor support in 2001. PARTICIPANTS: Intrapartum registered nurses. RESULTS: Six themes emerged under the category of factors that hinder nurses' intrapartum care: (a) hastening, controlling, and mechanizing birth; (b) facility culture and resources; (c) mothers' knowledge, language, and medica

Staff Confidence and Barriers to Complex Clinical Communication in Nursing Homes: A Convergent Mixed-Methods Study.

AIM: To describe all nursing home staff members' confidence in engaging in complex clinical communication with residents and family carers, and to explore factors affecting their attitudes. METHODS: A convergent mixed-methods study used questionnaires (n = 288) and 15 group-based discussions involving 278 professionals across nine nursing homes between April and May 2025. Quantitative data were analysed descriptively, and qualitative data underwent inductive thematic analysis. Findings from the two strands were compared and integrated to develop mixed-methods inferences, with qualitative data

Above the threshold, beyond the trip: the role of the 5-HT2A receptor in psychedelic-induced neuroplasticity and antidepressant effects.

Serotonergic psychedelics, including the recreationally used psilocybin and LSD, have become promising therapeutic agents for the treatment of treatment-resistant depression. While it is generally agreed that they exhibit their antidepressant effects by inducing rapid and sustained neuroplasticity, the molecular mechanisms responsible are widely debated. In particular, the role of the serotonin 5-HT2A receptor, known to mediate the hallucinogenic effects of psychedelics, is under scrutiny. However, many studies remain in conflict on whether action at the receptor is also required for neuroplas

Role of Antiplatelet Therapy in Stroke Prevention in Patients With Atrial Fibrillation.

Patients with atrial fibrillation are at increased risk of having a cardioembolic stroke. The use of oral anticoagulation is now well established to prevent strokes in patients with atrial fibrillation and a CHA2DS2-VASc (congestive heart failure, hypertension, age ≥75 years [2 points], diabetes mellitus, prior stroke/transient ischemic attack or thromboembolism [2 points], vascular disease, age 65 to 74 years, and sex category) score of greater than 1, beyond sex. However, the role of antiplatelet therapy, specifically aspirin in low-risk patients or as an alternative to oral anticoagulation,

Vagueness and Ambiguity in Communication of Case Management: A Content Analysis in the Australian National Disability Insurance Scheme.

Case management (CM) is an integrated care strategy, characterised by a set of actions to support person-centred planning, coordination of health and social services. Decades of CM, organisational psychology and occupational research highlight how vagueness and ambiguity in role communication can create role conflict and job stress, negatively impacts staff turnover, intra-organisational collaboration, job performance, and that poor communication of CM impedes policy, quality analysis service development and practice. We conducted a detailed top-down hierarchical, quality analysis of communica…

Member Perspectives on the Role of BDSM Communities.

Bondage-discipline/Dominance-submission/sadomasochism (BDSM) is an often misunderstood and misrepresented social phenomenon warranting further discourse and study. Community-based research that engages member perspective can assist in understanding socially marginalized experiences. The current study examined the role, meaning, and function of BDSM communities from the perspective of self-identified members. Seven nominal group technique workshops were conducted representing a variety of practitioner experiences and identities. Workshops involved 48 participants and resulted in the generation

The power to terrify: eclampsia in 19th-century American practice.

Throughout the 19th century, eclampsia was among the most dreaded complications of pregnancy. Conflicts arose over proposed etiologies and therapeutic modalities. Bloodletting and other harsh therapies reflected the prevailing humoral, neurovascular, and toxicologic theories. The role of untrained and professional nurses in the lying-in room of an eclamptic woman emerges from the literature of the time. An appreciation of the history of eclampsia provides context for the modern obstetric nurse.

Interdisciplinary team for the treatment of eating disorders.

Eating disorders are pathologies with a complex, multifactorial etiopathogenesis. Their treatment requires an interdisciplinary team. This paper discusses some characteristics and the most common difficulties encountered by such a team (multiple transfers, concealed alliances, role inversion, dissociation). Since their task that is carried out with patients in a critical situation and conflicting families, the team members are exposed to stressing conditions (illustrated by their dreams), and faced with possibilities of withdrawal and/or burn-out.

Magnesium supplementation and bone turnover.

Two-thirds of total body magnesium content is located in the skeleton. Recently, there have been reports that high dietary magnesium intakes are associated with higher bone mineral density. The effect of magnesium supplementation on bone turnover has recently been investigated in young adults. However, the findings from these studies are conflicting. More studies are necessary to better elucidate the role of magnesium in bone health.

Family functions and children's postdivorce adjustment.

Data on school-age children of divorced parents were examined to determine which dimensions of family dynamics were most associated with the children's socioemotional adjustment. Those factors found to be most significant were family roles, behavior control, and affective involvement, as well as children's reaction to and insight into the divorce, and conflict in the home after the divorce. Implications for parent education and early intervention are discussed.

Shiga toxin associated hemolytic uremic syndrome.

Shiga toxin associated hemolytic uremic syndrome (Stx HUS), a thrombotic microangiopathy, is the most common cause of pediatric acute kidney injury but has no direct treatment. A better understanding of disease pathogenesis may help identify new therapeutic targets. For this reason, the role of complement is being actively studied while eculizumab, the C5 monoclonal antibody, is being used to treat Stx HUS but with conflicting results. A randomized controlled trial would help properly evaluate its use in Stx HUS while more research is required to fully evaluate the role complement plays in the

Renin-angiotensin-aldosterone blockade for cardiovascular disease prevention.

The renin-angiotensin-aldosterone system (RAAS) plays a significant role in pathophysiology of multiple disease states. RAAS blockade is beneficial in patients with hypertension, acute myocardial infarction, chronic heart failure, stroke, and diabetic renal disease. RAAS blockade with the combination angiotensin-converting enzyme inhibitors (ACEIs) and angiotensin receptor blockers (ARBs) has demonstrated conflicting results in recent clinical trials. This article reviews the latest evidence of isolated ACEI or ARB use, their combination, and the role of aldosterone blockers and direct renin i

The role of cytokines in Guillain-Barré syndrome.

Cytokines play an important role in the pathogenesis of autoimmune diseases including Guillain-Barré syndrome (GBS) and its animal model experimental autoimmune neuritis (EAN). In this article, we reviewed the current knowledge of the role of cytokines such as TNF-α, IFN-γ, IL-1β, IL-6, IL-12, IL-18, IL-23, IL-17, IL-10, IL-4 and chemokines in GBS and EAN as unraveled by studies both in the clinic and the laboratory. However, these studies occasionally yield conflicting results, highlighting the complex role that cytokines play in the disease process. Efforts to modulate cytokine function in G

Blastocystis: past pitfalls and future perspectives.

Blastocystis is a genetically heterogeneous protist found in the intestinal tract (IT) of many vertebrates, and although it is implicated in a variety of human intestinal disorders, data regarding the clinical relevance of Blastocystis is at best speculative. Several research issues, including a lack of standardization across studies, the potential for intrasubtype variation in pathogenicity, and difficulties associated with diagnostics for many idiopathic disorders of the human IT have led to conflicting reports in support of a role for Blastocystis pathogenicity. Here, several research areas
