Massive rotator cuff tear associated with acute traumatic posterior shoulder dislocation: report of two cases and literature review.

A massive rotator cuff tear in association with acute traumatic posterior glenohumeral dislocation is rare. To our knowledge, only four documented cases have been reported in the literature. We present two additional cases of such injury secondary to the traffic accident. The first patient had an unsuccessful closed reduction due to the posterior instability while the second developed the profound shoulder weakness following the reduction. From the findings of our cases together with the previous reports, every patient had a unique injury mechanism of high-energy directed axial loading on an o

A Novel Methodology for Establishing Best Values for MCID, SCB, and PASS Thresholds for Rotator Cuff Repair.

BACKGROUND: The heterogeneity of threshold values for the minimal clinically important difference (MCID), substantial clinical benefit (SCB), and Patient Acceptable Symptom State (PASS) as reported in the rotator cuff repair literature undermines the utility of these concepts. This systematic review identifies studies with published threshold values and proposes a methodologic framework for determining which values should be used for rotator cuff repair moving forward. PURPOSE: To provide recommendations for the MCID, SCB, and PASS thresholds of commonly utilized patient-reported outcome meas

Anterior Capsular Reconstruction with Human Dermal Allograft for Irreparable Subscapularis Tears.

Anterior capsular reconstruction (ACR) using human dermal allograft (HDA) is an innovative surgical technique for managing irreparable subscapularis (SSc) tears. This procedure begins with patient positioning in the beach-chair configuration under general anesthesia, combined with an interscalene block for postoperative pain control. A deltopectoral approach is utilized, with an 8-10 cm incision extending from the coracoid tip to the deltoid tuberosity. After identifying and retracting the deltopectoral interval, the anterior glenoid is prepared by decorticating the bone surface to promote gra

Editorial Commentary: Arthroscopically Assisted Posterior Latissimus Dorsi and Teres Major Transfers Show Improvement for Irreparable Posterosuperior Rotator Cuff Tears but Are Rarely the Best Choice.

Tendon transfers have shown good clinical results in a numerous reviews of treatment options for unrepairable posterosuperior rotator cuff tears. Combined latissimus dorsi and teres major transfer have been described as an option to improve external rotation strength. Although outcomes have been acceptable, other options for treatment of this patient population may be preferable in terms of outcomes and perioperative morbidity.

Techniques for Lower Trapezius Transfer Using Achilles Allograft in Irreparable Posterosuperior Rotator Cuff Tears.

The management of irreparable rotator cuff tears presents significant challenges, particularly in active individuals experiencing functional limitations, such as reduced forward elevation and deficits in both external and internal rotation. Traditional latissimus dorsi (LD) tendon transfer has shown effectiveness in reducing pain associated with posterosuperior cuff tears but often yields inconsistent functional outcomes. This is largely due to the LD's primary role as an internal rotator, which limits its capacity to restore normal shoulder biomechanics. To address these limitations, the lowe

Editorial Commentary: Multiple Factors, Including Multimodal Anesthesia, Can Reduce Opioid Requirements After Rotator Cuff Repair.

Rotator cuff repair often results in significant pain and discomfort early after surgery, whereas longer-term outcomes generally result in substantial improvement. Efforts to decrease pain levels and opioid use after arthroscopic rotator cuff repair have led to the expansion of multimodal analgesia techniques. An interscalene brachial plexus block (ISBPB)-"plus" technique adds a subacromial space injection and "blind" (i.e., without imaging) suprascapular and axillary nerve blocks. Recent research has shown that the ISBPB-plus technique results in less opioid consumption at 48 hours compared w

Subacromial Surgery for Irreparable Posterosuperior Rotator Cuff Tears.

The irreparable posterosuperior rotator cuff tear describes a tear of the supraspinatus and/or infraspinatus tendon that is massive, contracted, and immobile in both the anterior-posterior and medial-lateral directions. Patients with an intact subscapularis and preserved forward elevation are challenging to treat because there is not a consensus treatment algorithm. For low-demand, elderly patients, several subacromial surgical options are available that can provide pain relief without the risks or burden of rehabilitation posed by reverse total shoulder arthroplasty or a complex soft-tissue r

A comparative probabilistic analysis of human and chimpanzee rotator cuff functional capacity.

Computational musculoskeletal modeling represents a valuable approach to examining biological systems in physical anthropology. Probabilistic modeling builds on computational musculoskeletal models by associating mathematical distributions of specific musculoskeletal features within known ranges of biological variability with functional outcomes. The purpose of this study was to determine if overlap in rotator cuff muscle force predictions would occur between species during the performance of an evolutionarily relevant horizontal bimanual arm suspension task. This necessitated creating novel p

Imaging and histological evaluation of the long head of the biceps tendon in the presence of different types of rotator cuff tears.

BACKGROUND: A comparison of changes in the long head of the biceps tendon for different types of rotator cuff tears has not been previously performed. Furthermore, the correlation between the thickening and degeneration of the long head of the biceps tendon and the cause of these changes have not been fully clarified. We evaluated the relationship between degenerative changes in the long head of the biceps tendon and rotator cuff tears in a rat model using imaging and histology. METHODS: Ninety-six 12-week-old Sprague-Dawley rats were divided into anterior (subscapularis tear), anterosuperior

Biceps-based 3-layer reconstruction of the irreparable rotator cuff: a technical note on biceps tendon as a common local graft for in-situ superior capsular reconstruction, partial cuff repair, and middle trapezius tendon transfer.

BACKGROUND: For irreparable rotator cuff tears, 3-layer tendon reconstruction (in which in-situ superior capsular reconstruction-reinforced partial rotator cuff repair was augmented with hamstring-sheet-lengthened middle trapezius tendon transfer) was recently reported to achieve satisfactory postoperative outcomes. To avoid hamstring graft-related drawbacks, the current note describes a technical modification of that reconstruct; wherein long head of biceps tendon is used as a cornerstone structure for simultaneously reconstructing the superior capsule; lengthening the transferred middle trap

MRI-based 3-dimensional volumetric assessment of fatty infiltration and muscle atrophy in rotator cuff tears.

BACKGROUND: The Goutallier and Warner Classification systems are useful in determining rotator cuff reparability. Data are limited on how accurately the scapular-Y view used in both systems reflects the 3-dimensional (3-D) changes in fatty infiltration (FI) and muscle atrophy (MA). Tendon retraction in the setting of a cuff tear may also influence the perception of these changes. This study's objectives were to (1) measure the 3-D volume of the supraspinatus muscle in intact rotator cuffs, and with varying magnitudes of retraction; (2) measure the 3-D volume of FI in the supraspinatus muscle i

Superior Capsular Reconstruction: Proposed Biomechanical Advantages.

The intact rotator cuff provides dynamic stability for the glenohumeral joint through range of motion by compressing the humeral head to the center of the glenoid. Maintenance of articular congruity provides a stable fulcrum for the more forceful muscles of the shoulder girdle. Massive rotator cuff tears disrupt the concavity-compression mechanism and lead to unopposed superior pull of the deltoid. As a result, superior migration of the humerus, abutment of the acromion, and, in some cases, progression to cuff tear arthropathy occur. Arthroscopic superior capsular reconstruction has emerged as

The Plug-Type Patch Results in Immediate and Postoperative Advantages in Graft-to-Bone Integration for Bridging Massive Rotator Cuff Tears in a Chronic Rabbit Model.

BACKGROUND: Various patches have been used to bridge massive rotator cuff tears (MRCTs) by reconnecting the cuff tendons to the humeral head, but the outcomes continue to be suboptimal. Notably, the graft-bone junction is a vulnerable site for failure, which requires optimization in patch design and techniques to enhance initial and postoperative fixation strength at the graft-bone interface. HYPOTHESIS: The plug-type patch (Plug-Pat) through intratunnel fixation would optimize mechanical characteristics in initial graft-to-bone fixation and subsequently improve postoperative biomechanical an

Incidence and risk factors for early postoperative stiffness after arthroscopic rotator cuff repair in patients without preoperative stiffness.

The purpose of this study was to investigate the incidence and risk factors of early postoperative stiffness in patients without preoperative stiffness undergoing isolated arthroscopic rotator cuff repair (ARCR). Two hundred seventy-four patients who underwent primary ARCR were included. At 3 months after surgery, criteria for shoulder stiffness was set as follows: (1) passive forward flexion < 120˚, or (2) external rotation at side < 30˚. Patients with preoperative stiffness or who underwent additional procedures were excluded. Patients-related, radiological (muscle atrophy and fatty infiltra
