OBJECTIVE: By presentation of cases of spontaneous (nontraumatic) ruptures of previously intact uteri, we sought to emphasize important aspects of this rare and dangerous event.

A case of recurrent rupture of the uterus is presented. The patient had a history of left cornual uterine rupture, which was repaired, and she was considered to be suitable for a subsequent pregnancy. During the subsequent pregnancy, attempts were made to evaluate the condition of the uterine scar by ultrasound in order to anticipate threatening rupture of the scar. The authors evaluated the ability of ultrasound (US) in detection of silicone implant ruptures and compared US detection with that of mammography and physical examination in 22 women with 29 sites of implant leakage. A 41-year-old woman developed hypovolaemic shock following instrumental delivery. This was ultimately discovered to be due to uterine rupture. Management involved massive transfusion of crystalloids, colloids and blood products and subtotal hysterectomy and left salpingo-oopherectomy progressing later to bilateral ligation of internal iliac arteries. Resuscitation was complicated by the development of dilutional coagulopathy, cardiac tamponade and cardiac arrest. The patient survived and was discharged from hospital 28 days after the primary event.

A young man with an infectious mononucleosis underwent an emergency splenectomy for a spontaneous splenic rupture. This is a rather rare complication of mononucleosis caused on the one hand by a change in the histological structure of the spleen and on the other hand by a splenomegaly. A case of spontaneous rupture of the colon in a previously asymptomatic patient is herein reported. At laparotomy, apart from the tear in the sigmoid colon, no other pathological problem was present. An awareness of the existence of this condition will lead to early diagnosis and management with improved morbidity and mortality.

A case is presented of spontaneous rupture of splenic tissue occurring 14 years after a splenectomy was carried out for trauma. Spontaneous rupture of a splenotic nodule has not previously been described and it may be added to the list of causes of spontaneous haemoperitoneum. The incidence and function of residual splenic tissue are briefly discussed and other causes of splenic rupture are outlined.

The clinical, endoscopic, and radiological features of seven patients with an uncommon oesophageal injury characterised by long lacerations of the oesophageal mucosa with haematoma formation but without perforation are reported.
