Sclerosing skin diseases comprise a group of distinct dermatological conditions characterized by fibrotic changes that may severely impair patients' quality of life. These conditions often present with cutaneous manifestations and, in some cases, may extend to extracutaneous tissues, potentially resulting in significant morbidity and mortality. This review explores the current understanding of the four most prevalent sclerosing skin diseases - localized scleroderma (morphea), systemic sclerosis (SSc), scleromyxedema, and scleredema adultorum of Buschke - as well as sclerotic conditions induced

Excimer light is a subtype of NB-UVB that emits a 308 nm wavelength, and can provide targeted phototherapy treatment. The absorption of 308 nm light by skin cells leads to therapeutic response in various common and ultraviolet-responsive skin diseases, such as psoriasis and vitiligo, and photo-resistant skin diseases such as prurigo nodularis, localized scleroderma, genital lichen sclerosis, and granuloma annulare, cutaneous T-cell lymphomas, among others. Excimer light has few adverse reactions and overall is well tolerated by patients, furthermore, it can be performed in places that are diff

Dear Editor, Localized scleroderma (LSc; morphea) is a rare fibrosing disease of the skin and underlying tissues, which is different from systemic sclerosis (SSc). LSc is classified based on clinical presentations: circumscribed, linear, generalized, pansclerotic, and mixed morphea. Generalized morphea (GM), which is characterized by widespread skin lesions with multiple indurated plaques or hyperpigmentation, is a relatively rare subtype occurring in 7-9% of patients with LSc. Previous studies have shown that SSc can be complicated by GM, separately from GM-like SSc. Here, we present a rare c

BACKGROUND: Segmental vitiligo, segmental morphea, and lichen striatus are distinct dermatological conditions that may share pathogenic mechanisms involving genetic mosaicism and immune dysregulation. Their simultaneous occurrence is extremely rare and may offer insights into their shared etiology.
CASE PRESENTATION: A 24-year-old female presented with a 6-year history of progressive indurated and discolored skin lesions. Dermatological examination revealed dermatomal depigmented macules consistent with segmental vitiligo on the right trunk, a violaceous Blaschkoid macule compatible with lich

A 72-year-old white man presented to the clinic with a tender, pruritic lesion on the upper part of his left arm that had progressively worsened over 4 months. Physical examination revealed an erythematous to violaceous, indurated, and sclerotic plaque with multiple foci of crusting and erosions (Figure 1). The patient denied any recent trauma, travel, fever, chills, weight loss, or constitutional symptoms. Before presentation, he had undergone treatment with cephalexin, prednisone, and doxycycline without reported improvement. Laboratory studies were negative for antinuclear antibody and SCL7

A 63-year-old woman had a sclerotic plaque with a bulla under her left breast. Histopathological examination of a biopsy confirmed the diagnosis of bullous morphea. Bullae are rarely seen in morphea. The patient was successfully treated with mometasone ointment 0.1%. To our knowledge, the pathogenesis of bullous morphea is still under debate.

Eosinophilic fasciitis is a rare inflammatory disease of unknown etiology, described for the first time by Shulman in 1974. The disease presents with induration of the skin, connective tissue and the underlying muscle fascia, sometimes accompanied by myalgia, most commonly in the lower extremities. Unlike scleroderma, it presents with absence of visceral organ involvement and Raynaud's phenomenon. Hypergammaglobulinemia and eosinophilia have been reported. Eosinophilic fasciitis is often associated with hematological disorders--there are reports of combinations with other autoimmune diseases s

Periocular linear scleroderma coup de sabre has been seen in association with blepharoptosis. The authors describe a 4-year-old girl who developed left blepharoptosis simultaneously with a remote scalp lesion consistent with linear scleroderma coup de sabre. This case is unique in that the scalp lesion was distant from the affected eye.

A 50-year-old woman presented with a three-month history of violaceous, non-tender, indurated plaques on the chest, abdomen, breasts, and proximal portions of the arms and legs. An incisional biopsy specimen showed changes consistent with a diagnosis of inflammatory morphea. Over the course of one year, the patient began to develop signs and symptoms suggestive of a diagnosis of eosinophilic fasciitis, which included the characteristic groove sign on the upper extremities. Although our patient did not exhibit peripheral or histopathologic evidence of eosinophilia, the diagnosis of eosinophilic

Nailfold capillaroscopy (NVC) is a simple, non-invasive, inexpensive and useful method for the analysis of microvascular abnormalities found in several rheumatic disorders. The well-known Raynaud's phenomenon is a clinical condition that should promptly lead to a microvascular analysis, in order to distinguish its primary form (functional, not disease associated) from the secondary Raynaud's phenomenon (disease associated). NVC has an exceptional predictive value in this early distinction, and this may be the best advantage this technique can offer. Microvascular damage is a typical feature of

Secondary mucinosis is a common finding in connective tissue diseases, especially in lupus erythematosus and dermatomyositis, but is seen only rarely in morphea. We report the case of a 9-year-old boy who presented with linearly arranged, flesh-colored to erythematous, indurated, very tender plaques on his right arm. He had similar lesions on his midchest and upper back. Histopathology revealed the characteristic findings of morphea and mucin deposition between thickened collagen bundles. This is an unusual case of linear morphea with hyaluronic acid deposition.

A 37-year old man was suffered from Raynaud's phenomenon, sclerodactyly, and polyarthritis involving knees, shoulders, and hands. Recurrent skin ulcers were present in finger tips. Laboratory studies showed positive RA test, antinuclear antibody (nucleolar type), and anti-Scl 70 antibody. So diagnosis of progressive systemic sclerosis (PSS) was made. Progressed destructive arthritis with rheumatoid nodules developed in the patient. The joint destruction was severe and he was satisfied with the criteria of the American Rheumatism Association (ARA). This patient appears to be an overlapping case

The contributions of anti-Topoisomerase 1 (Top1) autoantibodies to the pathophysiology of diffuse cutaneous systemic sclerosis (dcSSc), the most aggressive scleroderma subtype, are unknown. Top1 catalyzes DNA relaxation and unwinding in cell nuclei, a site previously considered inaccessible to antibodies. The discovery of autoantibodies in systemic lupus erythematosus that penetrate nuclei and inhibit DNA repair raised the possibility that nuclear-penetrating autoantibodies contribute to mechanisms of autoimmunity. Here we show that an anti-Top1 autoantibody produced by a single B cell clone f
