A multi-mode shock tube for investigation of blast-induced traumatic brain injury.

Blast-induced mild traumatic brain injury (bTBI) has become increasingly common in recent military conflicts. The mechanisms by which non-impact blast exposure results in bTBI are incompletely understood. Current small animal bTBI models predominantly utilize compressed air-driven membrane rupture as their blast wave source, while large animal models use chemical explosives. The pressure-time signature of each blast mode is unique, making it difficult to evaluate the contributions of the different components of the blast wave to bTBI when using a single blast source. We utilized a multi-mode s

Early ultrastructural changes of myocardial endothelial cells in traumatic haemorrhagic shock in rats.

The aim of this study was to assess early traumatic haemorrhagic shock (THS)-induced changes in myocardial endothelial cells (mECs) both morphologically and morphometrically. The mECs of capillaries of the left ventricular myocardium were investigated by electron microscopy 15, 30, 60, and 120 min after a standardised THS and compared to a control group. With the use of a computer-assisted image analysis system, we measured the following morphometric parameters--height, area, circumference, cytoplasmic processes, and interendothelial junctions--and determined the number and distribution of cyt

Protective effects of CG-4203, a novel stable prostacyclin analog, in traumatic shock.

We studied the effects of CG-4203, a novel stable prostacyclin analog, in a severe model of traumatic shock in rats. Traumatic shock was produced by Noble Collip drum trauma and was characterized by marked hypotension, a 4- to 5-fold increase in plasma cathepsin D and myocardial depressant factor activities, and survival time of 95 +/- 15 minutes. Treatment with CG-4203 (100 ng/kg/min) significantly prolonged survival time to 194 +/- 20 min (p less than 0.002). Traumatized rats treated with CG-4203 exhibited significantly lower plasma activities of the lysosomal hydrolase cathepsin D (p less t

[Functional state of the cerebral cortex and midbrain reticular formation during traumatic shock].

Acute experiments on cats were carried out during traumatic shock to study evoked potentials in the sensorimotor cortex and reticular formation (RF) of the midbrain in response to electrodermal irritation of the contralateral paw. The cortical function was judged by thresholds of convulsant reaction while function of RF by thresholds of activation reaction in the cortex of the course of high-frequency electrical stimulation of the structures under study. It was shown that in traumatic shock, initial and pronounced depression of the cortical function paralleled long enough stability of RF funct

[Risk factors for acute respiratory distress syndrome in patients with traumatic hemorrhagic shock].

OBJECTIVE: To investigate the risk factors of acute respiratory distress syndrome (ARDS) after traumatic hemorrhagic shock. METHODS: This was a retrospective cohort study of 314 patients with traumatic hemorrhagic shock at Trauma Medicine Center, Peking University People's Hospital from December 2012 to August 2021, including 152 male patients and 162 female patients, with a median age of 63.00 (49.75-82.00) years. The demographic data, past medical history, injury assessment, vital signs, laboratory examination and other indicators of these patients during hospitalization were recorded. Thes

Construction of Standard Fast Medical Procedures for Traumatic Shock and Its Application Effects.

OBJECTIVE: To explore the construction of standard fast medical procedures for traumatic shock and its application effects. METHODS: 84 patients with traumatic shock were admitted to emergency department of the hospital between January 2018 and January 2020. Using random number table method, the patients were divided into the control group (was given emergency treatment by routine emergency rescue procedures) and the study group (was given emergency treatment by standard fast medical procedures) with 42 patients in each group. The treatment time (rescue time, consultation time in each departm

Blood glucose concentrations in prehospital trauma patients with traumatic shock: A retrospective analysis.

BACKGROUND: Deranged glucose metabolism after moderate to severe trauma with either high or low concentrations of blood glucose is associated with poorer outcome. Data on prehospital blood glucose concentrations and trauma are scarce. OBJECTIVES: The primary aim was to describe the relationship between traumatic shock and prehospital blood glucose concentrations. The secondary aim was to determine the additional predictive value of prehospital blood glucose concentration for traumatic shock when compared with vital parameters alone. DESIGN: Retrospective analysis of the predefined, observati

Prehospital Resuscitation of Traumatic Hemorrhagic Shock with Hypertonic Solutions Worsens Hypocoagulation and Hyperfibrinolysis.

Impaired hemostasis frequently occurs after traumatic shock and resuscitation. The prehospital fluid administered can exacerbate subsequent bleeding and coagulopathy. Hypertonic solutions are recommended as first-line treatment of traumatic shock; however, their effects on coagulation are unclear. This study explores the impact of resuscitation with various hypertonic solutions on early coagulopathy after trauma. We conducted a prospective observational subgroup analysis of large clinical trial on out-of-hospital single-bolus (250 mL) hypertonic fluid resuscitation of hemorrhagic shock trauma

Venoarterial extracorporeal life support in post-traumatic shock and cardiac arrest: lessons learned.

OBJECTIVES: Venoarterial extracorporeal life support (VA-ECLS) is an effective support of acute hemodynamic collapse caused by miscellaneous diseases. However, using VA-ECLS for post-traumatic shock is controversial and may induce a disastrous hemorrhage. To investigate the feasibility of using VA-ECLS to treat post-traumatic shock or cardiac arrest (CA), a single-center experience of VA-ECLS in traumatology was reported. MATERIALS AND METHODS: This retrospective study included nine patients [median age: 37 years, interquartile range (IQR): 26.5-46] with post-traumatic shock/CA who were treat

Hemorrhagic shock after experimental traumatic brain injury in mice: effect on neuronal death.

Traumatic brain injury (TBI) from blast injury is often complicated by hemorrhagic shock (HS) in victims of terrorist attacks. Most studies of HS after experimental TBI have focused on intracranial pressure; few have explored the effect of HS on neuronal death after TBI, and none have been done in mice. We hypothesized that neuronal death in CA1 hippocampus would be exacerbated by HS after experimental TBI. C57BL6J male mice were anesthetized with isoflurane, mean arterial blood pressure (MAP) was monitored, and controlled cortical impact (CCI) delivered to the left parietal cortex followed by
