Beyond adenotonsillectomy: outcomes of sleep endoscopy-directed treatments in pediatric obstructive sleep apnea.

OBJECTIVES: In this study we determine the subjective and objective outcomes of pediatric patients with refractory OSA undergoing drug-induced sleep endoscopy (DISE)-directed surgical treatment. METHODS: 31 consecutive children with OSA following TA underwent DISE. 26 completed DISE-directed operative management of the level(s) of ongoing upper airway obstruction. Pre- and postoperative OSA were assessed through a detailed history (of nighttime symptoms (NS) and daytime symptoms (DS)), physical examination, and polysomnography. RESULTS: Age ranged 5-18 years (mean 9.7 ± 3.4). Fourteen of 26

Magnetic resonance sleep studies in the evaluation of children with obstructive sleep apnea.

Magnetic resonance (MR) sleep studies have been shown to be a useful tool in the evaluation and surgical planning of children with persistent obstructive sleep apnea. MR sleep studies include sequences to evaluate both static anatomic and well as dynamic motion information about the airway. The combination of the information about anatomic abnormalties and patterns of collapse can help in surgical decision making. The most common identified entities include recurrent enlargement of the adenoid tonsils, enlargement of the lingual tonsils, abnormal soft palate, glossoptosis, and hypopharyngeal c

Effects of sildenafil on autonomic nervous function during sleep in obstructive sleep apnea.

OBJECTIVE: To evaluate the effects of sildenafil on the autonomic nervous system in patients with severe obstructive sleep apnea. METHODS: Thirteen male patients with severe obstructive sleep apnea (mean age 43+/-10 years with a mean body mass index of 26.7+/-1.9 kg/m(2)) received a single 50-mg dose of sildenafil or a placebo at bedtime. All-night polysomnography and heart rate variability were recorded. Frequency domain analysis of heart rate variability was performed for the central five-minute sample of the longest uninterrupted interval of slow wave and rapid eye movement sleep, as well

Obstructive sleep apnea-hypopnea and neurocognitive functioning in the Sleep Heart Health Study.

BACKGROUND AND PURPOSE: Obstructive sleep apnea-hypopnea (OSAH) is associated with sleep fragmentation and nocturnal hypoxemia. In clinical samples, patients with OSAH frequently are found to have deficits in neuropsychological function. However, the nature and severity of these abnormalities in non-clinical populations is less well defined. PATIENTS AND METHODS: One hundred and forty-one participants from the Tucson, AZ and New York, NY field centers of the Sleep Heart Health Study completed a battery of neuropsychological tests for 9-40 months (mean=24 months, SD=7 months) after an unattend

The association of tongue scalloping with obstructive sleep apnea and related sleep pathology.

OBJECTIVE: The association between OSAS and patient history and physical exam findings is previously established; however, to our knowledge there are no studies that evaluate the role of tongue scalloping as a reliable clinical indicator for OSA, snoring, or the presence of other sleep pathology as evidenced by polysomnography. This study evaluates the hypothesis that such an association exists. SUBJECTS AND METHODS: Sixty-one otolaryngology clinic patients were evaluated by history and physical exam for the presence and degree of tongue scalloping, snoring, and other previously established c

Is sedation safe during dynamic sleep fluoroscopy of children with obstructive sleep apnea?

OBJECTIVE: Airway compromise is considered a relative contraindication for pediatric sedation. This contraindication presents a paradoxical problem when patients require sedation in preparation for imaging performed to evaluate the cause of airway obstruction. We use dynamic sleep fluoroscopy in the evaluation of children who have obstructive sleep apnea. The purpose of this study was to evaluate the success and safety of a structured sedation program for dynamic sleep fluoroscopy. MATERIALS AND METHODS: Eighty consecutive dynamic sleep fluoroscopic studies were evaluated. The type of sedatio

[Clinical relevance of periodic limb movements during sleep in obstructive sleep apnea patients].

INTRODUCTION: The periodic limb movements disorder (PLMD) is frequently associated with the obstructive sleep apnea syndrome (OSAS), but the prevalence and clinical relevance of this association have not been studied in detail. OBJECTIVE: The objectives were to make a prospective study on the prevalence of PLMD in patients with OSAS, and correlate this association with clinical and respiratory parameters. PATIENTS AND METHODS: Forty-two patients diagnosed with OSAS, without clinical suspicion of PLMD, underwent a polysomnographic study. Clinical symptoms and signs were evaluated with an stru

Subjective estimates of sleep differ from polysomnographic measurements in obstructive sleep apnea patients.

It is well established that, as a group, insomnia patients overestimate sleep onset latency (SOL) and underestimate total sleep time (TST) when compared to objective polysomnographic (PSG) findings. Whether a similar phenomenon occurs with other sleep disorders is not fully established. We compared the PSG sleep of 84 patients with suspected sleep apnea (SA) to their subjective experience of sleep reported on a sleep diary the morning after PSG testing. Both patients with SA (SA+) and those without (SA-) tended to overestimate SOL, but the SA+ group (n = 50) made larger overestimations (p < 0.

Localization of upper airway collapse during sleep in patients with obstructive sleep apnea.

The present study was conducted to determine the effects of body position and sleep state, as well as the effect of uvulopalatopharyngoplasty (UPPP) on the regions over which the upper airway (UA) collapses during sleep. To accomplish this goal, 18 male patients with obstructive sleep apnea (OSA) underwent overnight polysomnography with simultaneous monitoring of pressures in the posterior nasopharynx, oropharynx, hypopharynx, and esophagus. From the profile of pressures recorded in the UA and esophagus, the regions over which the UA collapses during apneas could be determined. The patients we

[Life-threatening apnea after midazolam administration in a patient with obstructive sleep apnea syndrome].

In a 27-year-old patient with a severe obstructive sleep apnea syndrome documented by a polysomnographic study, an oral dose of 15 mg midazolam caused life-threatening obstructive apnea and excessive sedation. Therapy with nasal CPAP eliminated the pharyngeal obstruction, but marked central hypoventilation persisted. Benzodiazepines are known to aggravate the obstructive sleep apnea syndrome. Dangerous central hypoventilation occurring after this medication during treatment with nasal CPAP has not yet been reported. Excessive susceptibility to depression of chemoresponsiveness to pCO2 is suspe

A crosstalk between the immune system and obstructive sleep apnea.

Obstructive sleep apnea (OSA) is characterized by recurrent upper airway obstruction during sleep. Its hallmark pathophysiological features, intermittent hypoxia (IH) and sleep fragmentation (SF), trigger cellular and molecular responses that disrupt immune homeostasis. Recurrent hypoxia-reoxygenation promotes oxidative stress and activates hypoxia-inducible factor-1α (HIF-1α), nuclear factor-κB (NF-κB), and the NLRP3 inflammasome, contributing to sustained inflammatory signaling, endothelial dysfunction, and alterations in circulating and tissue-resident immune cells. This review integrates e

Piezoelectric Ceramic Sensor Array Based Obstructive Sleep Apnea Event Detection.

Obstructive sleep apnea (OSA) is one of the major sleep disorders, which has been demonstrated to be a high-risk factor for cardiovascular disease, hypertension, and motor vehicle accidents. Pressure sensors in a contactless manner are a promising way to monitor sleep conditions outside of the hospital. However, previous studies mainly based on limited sensors are often subjected to noise contamination and constrained by the sleeper position to pressure sensors. The acquired pressure signals are of poor quality or even lost, which are not appropriate for the downstream task of OSA event detect

[Multifaceted health benefits of proactive intervention for obstructive sleep apnea].

Obstructive sleep apnea (OSA) is a highly prevalent disease worldwide and is closely associated with cardiovascular disease, metabolic disorders, and traffic accidents. Current OSA prevention and management face three major challenges: low public awareness, insufficient screening vigilance among non-specialist physicians, and a high proportion of asymptomatic patients who remain undiagnosed. Proactive intervention is crucial for reducing the public health burden and improving patient prognosis. This review summarizes the benefits of proactive intervention in OSA patients and outlines future st

Phenogroup-based stratification of cardiovascular risk in obstructive sleep apnea.

BACKGROUND: Obstructive sleep apnea (OSA) exhibits substantial phenotypic heterogeneity, but its prognostic relevance in patients with coronary artery disease (CAD) undergoing revascularization remains uncertain. METHODS: We pooled data from two prospective cohorts of CAD patients undergoing percutaneous coronary intervention or coronary artery bypass grafting (n = 2318). Major adverse cardiac and cerebrovascular events (MACCE; cardiovascular death, non-fatal myocardial infarction, non-fatal stroke, and unplanned revascularization) were assessed over a mean follow-up of 1.9 ± 1.1 years. Mechanistic interactions between inflammatory bowel disease and obstructive sleep apnea.

Inflammatory bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), is increasingly recognized as being associated with obstructive sleep apnea (OSA). Recent cohort studies and meta-analyses identify IBD as an independent risk factor for OSA, supporting the need for OSA screening in patients with IBD and management of modifiable risk factors. However, these associations come primarily from observational studies with virtually no confirmatory mechanistic studies. Further, the available clinical and experimental evidence of an association between OSA and IBD is highly h
