Atypical intradermal smooth muscle neoplasms: clinicopathologic analysis of 84 cases and a reappraisal of cutaneous "leiomyosarcoma".

Atypical or mitotically active dermal smooth muscle neoplasms are uncommon lesions, which are most often termed "cutaneous leiomyosarcoma," although preexisting-mostly small-series suggest a low risk of aggressive behavior. To further investigate these tumors, 84 cases from consultation and institutional files were analyzed for pathologic and clinical characteristics. There was a striking male-to-female preponderance (4.3:1), with a mean age of 56 years (range, 6 to 82y). Nine patients had a history of malignancies (6 of the skin). Tumors measured 1.3 cm on average and were predominantly locat

Hepatic PEComa, a rare cause of a liver mass.

Perivascular epithelioid cell tumors (PEComas) are rare mesenchymal neoplasms characterized by the coexistence of melanocytic and smooth muscle differentiation, with hepatic involvement being exceptionally uncommon. We present the case of a 60-year-old man with no relevant past medical history, who presented with right upper quadrant pain, without constitutional symptoms or significant laboratory abnormalities. Abdominal ultrasound revealed a hepatic mass in the right lobe. Further evaluation with contrast-enhanced computed tomography and dynamic liver magnetic resonance imaging demonstrated a

Provoking myofibroblast death: Strategies to resolve fibrosis and remodel tumor microenvironment.

Fibrosis is marked by the excessive accumulation of extracellular matrix (ECM) components, leading to tissue scarring and progressive loss of organ function. Myofibroblasts, which emerge during tissue repair, are specialized contractile cells exhibiting features of both fibroblasts and smooth muscle cells. Their expression of α-smooth muscle actin facilitates contractile activity, while their persistent activation and overproduction of ECM components contribute significantly to pathological wound contraction and fibrotic progression. Beyond ECM production, myofibroblasts play a significant rol

Epithelioid Leiomyoma of the Uterus: A Rare Tumor Mimicking Malignancy.

The majority of the smooth muscle tumors of the uterine tract are leiomyomas. A thorough histopathological study of these common lesions is mandatory despite a negligent tendency to omit thorough gross examination. Among the various histopathological subtypes of leiomyomas, epithelioid leiomyoma comprises an uncommon variant that does not display the classic spindle cell morphology of a conventional leiomyoma and could be confused histopathologically with smooth muscle tumor of uncertain malignant potential (STUMP), perivascular epithelioid cell tumor (PEComa), leiomyosarcoma, and endometrial …

Primary leiomyosarcoma of the adrenal; a case report.

INTRODUCTION AND IMPORTANCE: Primary adrenal leiomyosarcoma (PAL) is an extremely rare neoplasm that usually arises from the smooth muscle cells of the adrenal or adjacent vascular structures. The tumor is asymptomatic until it grows up and develops a mass effect in the retroperitoneal region. Although there are about 50 reported valid cases, surgical intervention is mandatory in the majority of patients. CASE PRESENTATION: Herein, we report the case of a 32-year-old healthy woman with a chief complaint of vague abdominal pain. After initial clinical and radiological examinations, we found a

Leiomyomatosis peritonealis disseminata: A case report and review of the literature.

Leiomyomatosis peritonealis disseminata (LPD) is a rare smooth muscle tumor, which is characterized by the dissemination of multiple smooth muscle-like nodules throughout the omental and peritoneal surfaces. The present report describes the case of a 33-year-old woman who had previously undergone laparoscopic myomectomy due to a bizarre leiomyoma. Five years after the initial operation, the patient was referred to our hospital with irregular episodes of right abdominal pain. As the presentation was highly suspicious of malignant uterine tumor metastasis or LPD, the patient subsequently underwe…

CT-guided aspiration core needle biopsy of gastrointestinal wall lesions.

PURPOSE: The purpose of this work was to examine the accuracy and safety of CT-guided core biopsy of gastrointestinal wall lesions. METHOD: We reviewed over 1,200 CT-guided abdominal core biopsy results from 1989 through 1998. Forty-five were from gastrointestinal wall lesions (16 gastric, 3 duodenal, 7 small intestine, and 19 colon). RESULTS: A definitive histologic diagnosis was obtained from the core biopsy in 41 patients (91%) without complication. The gastric lesion diagnoses were mesenchymal tumor of smooth muscle origin (eight), lymphoma (one), adenocarcinoma (three), and normal (one)

Diffuse leiomyomatosis with circumferential thickening of the gastrointestinal wall, resembling human diffuse leiomyomatosis, in a young miniature dachshund.

Leiomyoma is the most common mesenchymal tumor in the gastrointestinal (GI) tract. Leiomyomas usually have a single or multinodular mass of various sizes, and affected animals can develop alimentary symptoms depending on the location and size. A 3-year old female miniature dachshund died after a history of refractory rectal prolapse, esophagectasis and aspiration pneumonia. At necropsy, the GI wall at the gastroesophageal and anorectal junctions was circumferentially thickened. Histologically, both GI lesions were composed of bundles of well-differentiated smooth muscles without mass formation

Subcutaneous symplastic haemangioma after radiotherapy: A case report.

Symplastic haemangioma is a rare vascular tumor presented with regressive and degenerative atypia in stromal cells. Its morphology represents a challenge in classification of vascular tumors, regarding their biological behaviour in particular. We present a case report of a 47-years-old female with a history of left-sided breast adenocarcinoma treated by resection followed by adjuvant chemotherapy and radiotherapy. Three years after the primary diagnosis a tumorous mass appeared in the region of upper margin of left scapula, in subcutaneous tissues and the trapezius muscle. Histologically, the

Pulmonary Benign Metastasizing Leiomyoma in a Patient With a Concurrent Breast Tumor: A Diagnostic Challenge.

Pulmonary benign metastasizing leiomyoma (PBML) is a rare condition in which histologically benign smooth muscle tumors occur in distant organs, most often the lungs. Although typically discovered incidentally, differentiating BML from metastatic malignancy can be challenging in real-world clinical settings. A 53-year-old woman with a history of hysterectomy for uterine leiomyoma presented with multiple pulmonary nodules and a cystic left breast lesion containing an irregular solid component. Fluorine-18 fluorodeoxyglucose positron emission tomography/computed tomography demonstrated no signif…

Angiomyolipoma of the kidney--an immunohistochemical study.

The study included 25 patients with angiomyolipoma of the kidney. None of them had any signs of tuberous sclerosis. Serious clinical complications such as massive hemorrhage (2/25) and hydronephrosis (2/25) were documented in four patients. Concomitant adenoma of the adrenal gland and renal cell carcinoma were found in one patient each. All tumors consisted of a differing mixture of mature adipose tissue, smooth muscle cells and blood vessels, with thickened walls devoid of elastic lamina. Immunohistochemically, the smooth muscle cells stained strongly with antibodies against vimentin, desmin

Adrenal leiomyoma: a case report and review of the literature.

We describe a case of a 65-year-old Caucasian woman found to have an enlarging mass of the left adrenal gland. Laboratory examination revealed the mass to be nonfunctional. The patient underwent an uneventful left adrenalectomy. Pathological examination revealed the mass to be a leiomyoma. These tumors are benign and develop from smooth muscle cells. They can occur in any part of the body where smooth muscular layers exist, but occur frequently in the uterus and gastrointestinal tract. This case report presents a review of the recent literature on this rare entity.

Intracranial Leiomyoma Associated with Epstein-Barr Virus: A Cerebellopontine Angle Mass Presenting with Trigeminal Neuralgia.

BACKGROUND: Primary intracranial leiomyoma is a rare smooth muscle tumor often associated with Epstein-Barr virus (EBV), with <30 cases reported worldwide. These tumors commonly occur in patients with immunocompromised status, especially those with human immunodeficiency virus. In the present report, we have described the case of an EBV-associated leiomyoma at the cerebellopontine angle. The patient had presented with trigeminal neuralgia, which, to the best of our knowledge, is the first reported anatomical location and presentation for this tumor type. CASE DESCRIPTION: A 41-year-old male p…

Fibromyxoid variant of endometrial stromal sarcoma with atypical bizarre nuclei.

Endometrial stromal sarcoma (ESS) is the second most common malignant uterine mesenchymal tumor. It affects women primarily in the perimenopausal age group. ESSs are morphologically heterogeneous. The distinction between uterine smooth muscle tumors such as cellular leiomyoma and myxoid leiomyosarcoma and low-grade ESS can be problematic when stromal sarcomas show prominent smooth muscle differentiation and abundant myxoid stroma, respectively. We herein present a rare case of fibromyxoid variant of ESS, which was misdiagnosed as hydropic leiomyoma on intraoperative frozen section examination.…

The promoter methylation and expression of the O6-methylguanine-DNA methyltransferase gene in uterine sarcoma and carcinosarcoma.

O6-methylguanine-DNA methyltransferase (MGMT) gene promoter hypermethylation is observed in a number of solid tumors and is correlated with the silencing of MGMT expression. In glioblastoma patients treated with the alkylating agent temozolomide, MGMT gene methylation status was shown to have predictive value in terms of prolonged overall survival. Recently, temozolomide has demonstrated promising activity in the treatment of soft tissue sarcomas, including those of the uterus. The tissue specimens involving tumor samples and normal uterine fragments were obtained from nine patients with smoot
