[Human hand bite causing soft tissue infection and finger amputation: a case report].

A 50-year-old female patient presented to our institution with a human bite wound on her left hand. Her complaints started 15 days ago after her husband bit her hand. Oral antibiotics and local wound treatment had been given by another medical center to which she first presented. The treatment was changed to intramuscular antibiotics after her complaints worsened. The patient was neither hospitalized nor immobilized. She then presented to our clinic with deep wound infection. After hospitalization, intravenous (IV) antibiotics were given and open debridement was performed. F b hemolytic strept

Scedosporium apiospermum soft-tissue infection: a case report and review of kidney transplant literature.

Scedosporium apiospermum is an environmental mould. Human infections caused by this organism have been observed; however, only a few case reports show its role as a telluric contaminant to kidney recipients. We have reported here a case of a dramatic soft tissue infection by S. apiospermum in a kidney-transplanted man. Surgical drainage together with voriconazole systemic therapy was successful.

A simple model to help distinguish necrotizing fasciitis from nonnecrotizing soft tissue infection.

BACKGROUND: Necrotizing fasciitis (NF) has been associated with certain "hard" clinical signs (hypotension, crepitance, skin necrosis, bullae, and gas on x-ray), but these may not always be present. Using results of a previous study, we developed a simple model to serve as an adjunctive tool in diagnosing NF (admission WBC > 15.4 x 10(9)/L or serum sodium [Na] < 135 mmol/L) and determined its ability to distinguish between patients with NF and nonnecrotizing soft tissue infection (non-NF). STUDY DESIGN: A retrospective review was conducted of consecutive NF (n=31) and non-NF patients (n= 328)

Necrotizing soft tissue infection masquerading as cutaneous abcess following illicit drug injection.

OBJECTIVE: To assess factors that might predict serious necrotizing soft tissue infections following illicit drug injection. DESIGN: A retrospective review of a consecutive case series. SETTING: An urban municipal hospital. PATIENTS: Thirty patients presenting with cutaneous abscesses resulting from illicit drug injections during a 5-year period. All cases presented clinically with fluctuance, erythema, or induration but required extensive debridement at the time of incision and drainage. INTERVENTIONS: Operative treatment employed wide incision, routine subfascial examination, and aggress…

Acute soft tissue infection in intravenous drug abusers: its differential diagnosis by ultrasonography.

Portable, real-time, B-mode ultrasonography was used by surgeons to diagnose acute soft tissue infections in intravenous drug abusers. It was helpful in differentiating simple cellulitis from abscess and in detecting pseudoaneurysm and thrombophlebitis. The authors conclude that ultrasonography as an initial diagnostic method for these types of lesions provides both early diagnosis and early treatment, thereby shortening hospital stay and reducing complications.

An Unusual Case of Genital Herpes Simplex Virus (HSV) Infection With Severe Non-necrotizing Soft Tissue Infection.

Genital herpes simplex virus (HSV) infection is a prevalent condition with symptoms of genital ulcers, vesicles, or pustules, often along with malaise, fever, and inguinal lymphadenopathy. Although inguinal lymphadenopathy is quite common in genital HSV infection, it is uncommon to present without anogenital symptoms. This report highlights its clinical significance with a report of an immunocompetent 65-year-old male involved in high-risk sexual activities overseas who initially presented with only persistent inguinal lymphadenopathy. A perianal vesicular rash was identified on examination wi

Real-world experience with dalbavancin therapy in gram-positive skin and soft tissue infection, bone and joint infection.

PURPOSE: Dalbavancin is a novel lipoglycopeptide with potent activity against several gram-positive pathogens, an excellent safety profile and a long elimination half-life. METHODS: In this case series observed at the University Hospital of Vienna between 2015 and 2017, all adult patients with gram-positive infections who received at least one dosage of dalbavancin were screened (n = 118). A total of 72 patients were included in the final analysis. The number of included patients stratified by the source of infection was: skin and soft tissue infection (SSTI) (n = 26), osteomyelitis (n = 20),

An Ineffective Differential Diagnosis of Infective Endocarditis and Rheumatic Heart Disease after Streptococcal Skin and Soft Tissue Infection.

We herein report the case of a 68-year-old woman with a skin and soft tissue infection at her extremities. The blood culture results were positive for Streptococcus pyogenes, and we started treatment using ampicillin and clindamycin, although subsequent auscultation revealed a new-onset heart murmur. We therefore suspected rheumatic heart disease and infective endocarditis. The case met both the Jones criteria and the modified Duke criteria. Transesophageal echocardiography revealed vegetation on the aortic valve, although the pathological findings were also compatible with both rheumatic hear

The Silent Infection: Cytokine Blockade, Blunted Sepsis Physiology, and Soft Tissue Infection With NLRP12-Associated Autoinflammatory Syndrome.

NLRP12-associated autoinflammatory syndrome is a rare monogenic autoinflammatory disease characterized by recurrent innate immune dysregulation and systemic inflammation. Distinguishing infection from hyperinflammatory disease activity in these patients is particularly challenging during cytokine-directed immunosuppressive therapy. We report a 20-year-old male with NLRP12/FCAS2-associated disease, systemic juvenile idiopathic arthritis (sJIA), and prior hemophagocytic lymphohistiocytosis/macrophage activation syndrome (HLH/MAS) who presented with bilateral gluteal cellulitis, recurrent fevers,

Mucormycosis Necrotizing Soft Tissue Infection: A Case Report of Fungal Infection Following a High-Speed Motorcycle Accident.

Mucormycosis is a rare fungal infection characterized by rapidly progressing infarction and necrosis of host tissue, frequently resulting in death. It is most well-known for causing a devastating rhinocerebral infection, however, cutaneous mucormycosis has been documented. While this opportunistic infection generally affects immunocompromised individuals or patients with uncontrolled diabetes, mucormycosis can also present following traumatic injuries. Infection following motor vehicle accidents accounts for as little as 3% of traumatic cutaneous mucormycosis cases, however, it can have devast

The reemergence of Streptococcus pyogenes in skin and soft tissue infections: a review of epidemiology, pathogenesis, and management strategies.

PURPOSE OF REVIEW: To discuss skin and soft tissue infections (SSTIs) caused by group A Streptococcus (GAS) by focusing on their pathogenesis, clinical manifestations, and management strategies. RECENT FINDINGS: GAS is responsible for a wide range of infections from mild disease to severe fatal invasive infections with high mortality rates. Invasive GAS (iGAS) infections affect both young and old individuals and account for 1.8 million cases worldwide, with a mortality rate of up to 20%. In addition, conditions resulting by immune responses triggered by GAS also contribute to GAS-associated m…

Children Are Not Small Adults: An Epidemiologic Review of Necrotizing Soft Tissue Infections in Pediatric Patients Compared With Adults.

BACKGROUND: Necrotizing soft tissue infections (NSTI) are rare but potentially life-threatening infections that can occur in both pediatric and adult patients. To date, few studies have examined NSTI in pediatric populations. Thus, the majority of guidelines for diagnosis and managing NSTI are based on adult data. The goal of this study was to utilize a large retrospective database to compare and contrast adult and pediatric cases of NSTI from a single center. METHODS: A retrospective review from a single tertiary center identified 446 patients with confirmed NSTI, 27 children (below 18 years…

Wound healing outcomes following treatment with synthetic hybrid-scale fiber matrix after resection of soft tissue tumors or infections.

BACKGROUND: Wide excision of soft tissue tumors or infections often results in large defects that can be challenging to manage. Advanced treatment modalities-including NPWT, skin grafts, and xenografts-can all be considered for post-resection wound management, but each has its limitations. An SHSFM, engineered to resemble human extracellular matrix, has demonstrated positive wound healing outcomes in prior studies. MATERIALS AND METHODS: Adult patients at a single institution who underwent resection of soft tissue tumor or infected tissue followed by treatment with SHSFM from 2020-2023 were r

Defining the Optimal Antibiotic Duration in Necrotizing Skin and Soft Tissue Infections: Clinical Experience from a Quaternary Referral Center.

Background: Early initiation of broad-spectrum antibiotic agents is a cornerstone of the care of necrotizing skin and soft tissue infections (NSTI). However, the optimal duration of antibiotic agents is unclear. We sought to characterize antibiotic prescribing patterns for patients with NSTI, as well as associated complications. Patients and Methods: Using an NSTI registry, we characterized antibiotic use at a quaternary referral center. Kaplan-Meier analyses were used to describe overall antibiotic duration and relative to operative source control, stratified by presence of other infections t…

Emergence of a mupirocin-resistant, methicillin-susceptible Staphylococcus aureus clone associated with skin and soft tissue infections in Greece.

BACKGROUND: Staphylococcus aureus causes various infections, including skin and soft tissue infections (SSTIs). In this study, methicillin-susceptible S. aureus (MSSA) from SSTIs among patients in three tertiary-care hospitals in Greece were studied in terms of antimicrobial resistance, clonal distribution, toxin and adhesin genes carriage. RESULTS: During a five-year period (2014-2018), 6145 S. aureus were recovered from 13,244 patients with SSTIs and tested for antimicrobial susceptibility. MSSA were 4806 (78.21 %) including 1484 isolates with mupirocin minimum inhibitory concentration (MIC…

The experiences of people who inject drugs of skin and soft tissue infections and harm reduction: A qualitative study.

BACKGROUND: Bacterial skin and soft tissue infections (SSTI) among people who inject drugs (PWID) are considered a public health concern. There is a lack of qualitative research examining the lived experience of PWID who have had SSTI. This paper explores PWID views and experiences of their SSTI, their perceptions on the causes of their SSTI and their harm reduction (HR) behaviours. The implications for HR service delivery and practice will be discussed. METHODS: Between October 2015-January 2016, 22 in-depth interviews were conducted with PWID who had experienced a SSTI within the past year.…

Role of outpatient parenteral antibiotic therapy in the treatment of community acquired skin and soft tissue infections in Singapore.

BACKGROUND: Treatment of community acquired skin and soft tissue infections (SSTIs) is a common indication for outpatient parenteral antibiotic therapy (OPAT) in USA, UK and Australasia, however data from Asia are lacking. OPAT is well established within the Singapore healthcare since 2002, however, systematic use of OPAT for the treatment of SSTIs remains infrequent. In this report, we describe the treatment and outcome of patients with SSTIs referred directly from Emergency Department (ED) to OPAT for continuation of intravenous (IV) antibiotics in Singapore, thus avoiding potential hospital…
