Long-term effects of auditory training in severely or profoundly deaf children.

Despite new technological advances in the rehabilitation of audition in profoundly deaf children, auditory training remains a fundamental part of their education. Consequently, it is necessary to learn what kind of stimuli, what kind of auditory tasks, and what kind of learning procedures generate faster and more long-lasting benefits. The present study evaluates a training program that includes numerous nonlinguistic stimuli that tap into several means of cognitive processing. The program is based on an implicit learning procedure and was tested with six severely or profoundly deaf children.

Otoacoustic emissions recorded at high rates in patients with confirmed acoustic neuromas.

Otoacoustic emission testing was carried out on 39 patients with confirmed acoustic neuromas, and satisfactory emissions were recorded from the neuroma ear of 59% of them. A comparison of the patients with and without emissions showed no significant differences in low-frequency or high-frequency hearing loss, optimum speech discrimination score, or canal paresis in the affected ear between the two groups. Emissions were recorded at stimulus rates up to 5,000 clicks/s by using the maximum length sequence (MLS) technique. The decrease in the emission amplitude with increase in click rate (rate s

Sensitivity of distortion product otoacoustic emissions in noise-exposed chinchillas.

The present study investigates the effect of small amounts of outer hair cell (OHC) loss on distortion product otoacoustic emission (DPOAE) levels and evoked potential permanent threshold shifts (PTS) in a population of 12 noise-exposed chinchillas. The group mean DPOAE level, which decreased by up to approximately 15 dB in the presence of less than 8 dB PTS and 15% OHC loss, indicates that DPOAEs can detect an underlying cochlear pathology (i,e., OHC damage/loss) despite the presence of normal to near normal thresholds. The sensitivity of DPOAEs in detecting OHC loss makes this test measure s

Auditory nerve disease of both ears revealed by auditory brainstem responses, electrocochleography and otoacoustic emissions.

We report on two patients who showed absence of auditory brainstem response (ABR) but broad compound action potentials on electrocochleograms and almost normal otoacoustic emissions, together with absence of caloric response and preservation of per rotatory nystagmus for both ears. Patient 1, a 53-year-old woman, had noted auditory and vestibular problems since the age of 15 years, and Patient 2, a 68-year-old woman, had noted problems of the same age of 30 years. They could hear words and understand sentences if spoken slowly, but they could not discriminate monosyllables very well. Their aud

[Long-term results of gentamycin therapy of Menière's disease].

During the years 1976 to 1987, 82 patients with Ménière's disease received a tympanoclysis with gentamicin on a daily basis. Dosage was titrated up to the initial symptoms of vestibular or cochlear intoxication. Sixty-four patients filed for a complete follow-up examination; it was possible to interview 15, while three patients were lost. Vertigo was eliminated in 70 (89%) cases. Nine patients (11%) remained disabled, while demonstrating typical paroxysmal positional vertigo in three cases. Speech discrimination improved in 13%, stabilized in 54%, and deteriorated in 33%. Hearing improved sign

Perception of illusory clusters: the role of native timing.

We explore the influence of native timing patterns on nonnative speech perception, by asking whether a nonnative CVCV sequence can be perceived as CCV when the temporal organization of nonnative CVCV is similar to native CCV. To explore this question, Georgian listeners are tested on a CCa-CVCá discrimination in French. Georgian has a rich word-onset cluster inventory, with component consonants loosely timed. The loose timing often, though not always, results in a schwa-like CC transition. French, the stimulus language, exhibits tighter timing in biconsonantal clusters, no vocalic transitions,

Development of the Children's Communication Checklist (CCC): a method for assessing qualitative aspects of communicative impairment in children.

The Children's Communication Checklist (CCC) was developed to assess aspects of communicative impairment that are not adequately evaluated by contemporary standardised language tests. These are predominantly pragmatic abnormalities seen in social communication, although other qualitative aspects of speech and language were also included. Some items covering social relationships and restricted interests were incorporated, so that the relationship between pragmatic difficulties and other characteristics of pervasive developmental disorders could be explored. Checklist ratings were obtained for 7

Vestibular schwannomas and auditory function: options in large T3 and T4 tumors?

OBJECTIVE: While hearing preservation has become an accepted treatment goal, there are still major doubts whether and to what extent this aim is feasible in large tumors that cover the auditory nerve and involve the brainstem. METHODS: One thousand eight hundred (1,800) vestibular schwannomas (VS) were operated on at Nordstadt Neurosurgical Department from 1978 to 1999 by the senior surgeon (MS). There were 1,800 tumors in 1,750 patients, 1,605 patients had unilateral tumors, 145 patients had neurofibromatosis type 2 with bilateral tumors, 195 of which were treated surgically in our unit. Microsurgery versus radiosurgery in the treatment of small acoustic neurinomas.

Microsurgical preservation of the facial nerve during removal of acoustic neurinomas can hardly be compared with microsurgery of the eighth cranial nerve. Many more anatomical and pathogenetic factors are involved that need careful consideration. In small neurinomas, of grades I and II, total extirpation of the tumour with preservation of both the facial nerve and segments of the vestibulocochlear nerve not directly involved by the tumour has become a safe and practical technique. In small acoustic neurinomas immediate facial nerve function could be preserved in 88% and "useful hearing" could

[Long-term results of multi-channel stimulation of the cochlear nucleus with auditory brain stem prostheses].

Since 1992 11 subjects with bilateral neural deafness due to cerebellopontine angle tumors have been provided with a multichannel auditory brainstem implant. The long term follow-up varies between 12 and 48 months. 10 subjects experienced auditory sensations with various pitch during stimulation of several surface electrodes, 7 of them are still full time users. 1 patient died 18 months after implantation after having been using his device continuously and 1 died due to pneumonic emboly without having been stimulated. 2 subjects revealed uncomfortable side-effects such as motoric or somatosens

In-the-canal hearing aids. Their use by and benefit for the younger and elderly hearing-impaired.

The present investigation was performed in order to evaluate the use and benefit of in-the-canal hearing aids (ITC-HAs; Dan 131) in a group of elderly and younger subjects. An extensive questionnaire was mailed to 256 hearing aid users. The responding group comprised 220 subjects (86%) and consisted of 108 males and 112 females with a median age of 69 years, range 11-89. In order to compare between the elderly and the younger subjects, the material was subdivided into two groups: one less than 70 years (n = 113), and the other greater than or equal to 70 years (n = 107). The median values of t

Sudden hearing loss and cerebellopontine angle tumors.

Lesions of the cerebellopontine angle may cause a sudden hearing loss. To illustrate and reaffirm the importance of a thorough neurotologic evaluation we performed a retrospective review of the audiologic and clinical data available on 506 patients with proven lesions of the cerebellopontine angle. Seventy-seven patients (15.2%) were found to have a history commensurate with sudden hearing loss. We reviewed the relevant audiologic data as well as the clinical circumstances of this select patient population.

[Direct recording from cochlear nerve via a ball-electrode in transtemporal acoustic neuroma surgery].

BACKGROUND: Intraoperative monitoring (IOM) of the cochlear nerve function during acoustic neuroma surgery is employed to assist in preserving hearing. So far, Auditory Brain-stem Response (ABR) is considered to be an optimal method for intraoperative monitoring in transtemporal approach. The aim of this study was to perform direct recording of the cochlear nerve action potential after resection of the tumor by using a ball-electrode and to evaluate the use of this method in predicting the postoperative hearing. The obtained data were compared to the simultaneous ABR results and to the postope

Auditory performance and cortical plasticity in blind individuals with sensorineural hearing loss: insights into dual sensory impairment.

BACKGROUND: Blind individuals with normal hearing frequently exhibit enhanced auditory abilities driven by compensatory cross-modal neuroplasticity. However, it remains unclear whether these compensatory advantages persist in individuals with dual sensory impairment. This study investigated auditory processing and cortical plasticity in early-blind, late-blind, and sighted individuals, all presenting with concurrent sensorineural hearing loss. METHOD: Participants completed auditory behavioral tests, including working memory, spectral ripple discrimination, temporal modulation detection, and

Metabolic investigations in Menière's disease. Preliminary findings.

The aetiology of Menière's disease is unknown but in recent years many theories have been advanced to explain the observed pathological changes. These include abnormal metabolic states, allergy and personality type. In an attempt to throw more light on this and to confirm or refute the earlier work we studied twenty-seven (fifteen male:twelve female) patients with Menière's disease who conformed to the criteria set out by the Committee of Hearing and Equilibration of the American Academy of Ophthalmology and Otolaryngology (Alford, 1972). In this series the incidence of abnormal metabolic stat

Discrimination of multichannel-compressed speech in noise: long-term learning in hearing-impaired subjects.

OBJECTIVE: It takes time for an individual to obtain optimal benefit from a new hearing aid. This research examines the possibility that similar long-term learning can be seen in consecutive laboratory studies of multichannel compression (MCC). DESIGN: Three studies of different parameters of MCC processing, carried out over the period of 1 yr, included the same 15 hearing-impaired subjects and one identical MCC-processing condition. The full-range MCC had 8, 12, or 16 independent frequency channels, using a Robinson-Huntington compression algorithm. The City University of New York nonsense s

A Comparative Study of Audiologic Outcomes for Two Transcutaneous Bone-Anchored Hearing Devices.

OBJECTIVE: In 1977, Tjellstrom et al. used the concept of osseointegration and implanted patients with a titanium screw as part of the first bone-anchored hearing solution. Despite good hearing outcomes with the percutaneous device, there has been a drive for the development of transcutaneous bone conduction systems because of soft tissue complications, aesthetics, and loss or failure of the abutment. This study compares audiologic and quality of life questionnaire outcomes (author-designed questionnaire) for two transcutaneous bone conduction implants. STUDY DESIGN: Cross-sectional cohort st
