Dynamic compliance of the CSF system in iNPH (Part I) - in vitro investigation of the impact of spinal canal stenoses.

Spinal canal stenosis is an age-related degenerative condition observed mainly in the cervical and lumbar regions. Recent research has suggested that such stenoses influence cerebrospinal fluid (CSF) dynamics in idiopathic normal pressure hydrocephalus (iNPH) patients by increasing spinal canal flow resistance thereby directly impacting craniospinal dynamic compliance and intracranial pressure (ICP). This study experimentally investigates the effects of cervical and lumbar spinal stenoses on CSF dynamics using a validated in vitro model. To simulate varying degrees of stenosis, controlled redu…

Transforaminal Migration of K-Wire From Clavicle to Cervical Spinal Canal: A Case Report and Review of Literature.

INTRODUCTION: Kirshner wires are commonly used for orthopedic surgical fixation for shoulder injuries, particularly around the acromioclavicular joint. They are versatile, cheap, and minimally invasive in stabilizing bone fragments. However, there have been cases of secondary migration of K-wires from the clavicle to surrounding tissues, particularly around the cervical spine. CASE REPORT: We present a case of a 60-year-old female with left-sided neck pain and radiation to the left upper limb, a K-wire was found to have migrated through the C7-T1 foramina left side into the spinal canal poste…

Retained bullet in the cervical spinal canal and the associated surgical management conundrum: case report and review of the literature.

INTRODUCTION: Gunshot wounds (GSW) to the cervical spine remain uncommon. Surgery often does not yield significant neurological improvement and the decision to utilize surgery depends on a number of factors. We describe the case of a 28 year-old male suffering a complete spinal cord injury (SCI) secondary to a bullet lodged in the cervical spinal canal. We present the unique radiological findings and review the indications for and utility of spine surgery for cervical GSW. CASE PRESENTATION: The patient was a 28 year-old male involved in a motor vehicle accident immediately after sustaining a…

Redundant nerve roots of the cauda equina in lumbar spinal canal stenosis, an MR study on 500 cases.

PURPOSE: The purpose of this study was to evaluate magnetic resonance (MR)-detected redundant nerve roots (RNRs) of the cauda equina in patients with lumbar spinal canal stenosis. METHODS: A total of 500 lumbar MR studies in patients with lumbar spinal canal stenosis were reviewed for the presence and characteristics of RNRs of the cauda equina. The length of the RNRs relative to the height of the upper vertebral body of the level of the stenosis was used as a prognostic indicator. RESULTS: RNRs were detected in 15% of the patients, the majority above the level of the stenosis (85%) and loop…

A case of Proteus syndrome with severe spinal canal stenosis, scoliosis, and thoracic deformity associated with tethered cord.

Proteus syndrome is a rare, sporadic, hamartomatous disorder manifesting with multifocal overgrowth of tissue. The features seem to develop most often during childhood. Vertebral overgrowth with severe spinal canal stenosis is unusual, although scoliosis with abnormal vertebral bodies is one of the typical features of Proteus syndrome. We report a case of Proteus syndrome with severe spinal canal stenosis, scoliosis, cervical kyphosis, and thoracic deformity with airway obstruction because of asymmetrical overgrowth of vertebrae and ribs associated with a tethered cord, lipomas, strawberry hem…

Lumbar vertebral growth plate displaced into the spinal canal. A case report of a 15-year-old boy.

Fracture of the lumbar cartilage plate is very uncommon. When the fragment protrudes into the spinal canal, the symptoms may mimic a protruded lumbar intervertebral disc. This may delay proper surgical treatment and may reduce the chance for neural tissue recovery.

[Radiographic and myeloscintigraphic evaluation of normal aging changes in the lumbar spine and the relationship with lumbar spinal canal stenosis].

In 250 subjects with normal lumbar spine and 39 patients with lumbar spinal canal stenosis (LSCS), radiographic measurement such as canal-to-body ratio (CBR), intervertebral space, inclination and thickness of the facet were performed to statistically evaluate normal degenerative changes in lumbar spine and the difference between normal degenerative changes and those of LSCS. Results were as follows: in normal subjects, 1) intervertebral space became narrower with aging (below L3-4), 2) the inclination of the facet increased in all lumbar spines, 3) the thickness of facet showed no relationshi…

Laminoplasty for patients with compressive myelopathy due to so-called spinal canal stenosis in cervical and thoracic regions.

The author reports detail of a simplified technique named laminoplasty, as an alternative to laminectomy that allows osteoplastic enlargement of the spinal canal in the cervical and thoracic spines. The postoperative spinal deformities from laminectomy can be prevented by employing this method since it promotes stable clinical results. The indications, technique, and results of the procedure are discussed in ten cases. The laminoplasty is recommended for compressive myelopathy due to narrowed spinal canal, especially in the cervical region. Clinical neurologic improvement tends to parallel the…

A comparison of radiographic methods of diagnosing constrictive lesions of the spinal canal. Toshiba unit vs. CT scanner.

With the advent of computerized tomography (CT), a new method of visualizing the spinal canal in cross-section has been created. Before the introduction of CT scanning, evaluation of the cross-sectional anatomy of the spinal canal was accomplished chiefly by the Toshiba unit. This study compares these two forms of tomography and discusses their relative effectiveness in diagnosing constrictive lesions of the spinal canal.

[Possibilities of cervical fusion surgery in the spinal canal with intervertebral disk degeneration, Therapeutic and case-report contribution].

This is a report of a patient with pains in the shoulders and arms, atrophies of the small muscles of the hands, and paraspasms of the legs. After having suffered from the disease for eight to nine years, the patient was diagnosed as having cervical myelopathy. Measurements of the bony cervical vertebral column, which were made on a lateral roentgenogram, showed extreme narrowness of the spinal canal, the measured values obtained being below the lower limit. In addition, the cervical canal was narrowed at three levels by the formation of projections on degenerate disks. This suggested the most…

Acquired Lumbosacral Spinal Dural Arteriovenous Fistula in Association with Degenerative Lumbosacral Disc Herniation and Spinal Canal Stenosis: Report of Two Cases and Review of the Literature.

The authors describe two cases harboring lumbosacral spinal dural arteriovenous fistulas (SDAVFs) manifested with nonspecific initial symptoms, leading to misdiagnosis and unnecessary procedures. A curvilinear flow void in the lumbar region and thoracic cord congestion with subtle perimedullary flow voids were detected on magnetic resonance imaging (MRI) in both patients. Contrast-enhanced magnetic resonance angiography and spinal angiography confirmed the SDAVFs in the lower lumbar and sacral region. Both fistulas were located at the same level of disc herniation and spinal canal stenosis and…

Risk factors for adjacent segment disease after lateral lumbar interbody fusion for lumbar spinal canal stenosis with instability.

PURPOSE: Lateral lumbar interbody fusion (LLIF) achieves indirect decompression by restoring disc height; however, excessive cage elevation may increase the risk of adjacent segment disease (ASD). This relationship remains unclear. Therefore, this study aimed to identify ASD risk factors following LLIF by focusing on intervertebral height gain and comparing outcomes of LLIF and decompression using propensity score matching (PSM). METHODS: Seventy-six patients who underwent single-level LLIF for lumbar stenosis with instability were analyzed to identify ASD-related risk factors. Univariate and
