Magnetic resonance imaging continues to dominate the literature on spinal imaging and further advances have occurred during the last year. It is now the investigation of choice for most problems. Intraoperative sonography also may have a place, but other modalities are being superseded. Plain films retain a value in trauma.

Infertility is a significant and frustrating problem for many men after spinal cord injury. The two major causes are poor semen quality and ejaculatory dysfunction. Factors attributed to poor semen quality include stasis of prostatic fluid, testicular hyperthermia, recurrent urinary tract infections, abnormal testicular histology, possible changes in the hypothalamic-pituitary-testicular axis, possible sperm antibodies, chronic long-term use of various medications, and type of bladder management. Sparganosis of the spinal cord is a rare disease: only two cases have previously been described. A third case of spinal sparganosis is reported in which a sparganum was removed from the central canal of the patient's spinal cord. The clinical presentation and pathology are described. A brief review of the life cycle of the Spirometra is given and the possible mode of infestation is discussed.

Family physicians are often unfamiliar with the care of patients with spinal cord injuries because they may have only one such patient in their practice. Urinary tract infections, constipation, and decubitus ulcers are the most common problems, and autonomic dysreflexia the most serious emergency that family physicians treat in this population. This article addresses these areas, as well as spasticity, sexuality, depression, and the acute abdomen.

An analysis of 221 clinical observations showed that in 23.1%, spinal cord tumours were characterized by a remitting course. The pathogenesis of remissions was rather complicated and was partly determined by ischemic impairments of the spinal blood circulation and by repeated exacerbations of the spinal pathological process in the medulla. A prolonged remitting course of spinal cord tumours was to a certain degree associated with a marked protective reaction of the body when the growth of tumour cells might be inhibited due to "biological regression".

A 71-year-old man experienced gradually progressive leg weakness, urinary retention, and mild loss of sensation in dermatomes T8 through T12 bilaterally. After 5 to 6 weeks of illness, he developed flaccid paraplegia and sensory loss below T8. He died 16 weeks after onset of neurological symptoms. The lumbosacral spinal cord including the cauda equina was examined in 100 unselected autopsied cases of infants dying at or near term. A striking finding in 10% of these cases was the occurrence of bilateral collections of ependymal cells lying in rows on the surface of the lumbosacral cord. Spinal cords from young (125 g) and adult (400 g) albino rats have been removed from the vertebral column by pressurized hydraulic ejection. Histologic examination of the spinal cords has revealed no structural damage to the tissue. In view of the rapidity and ease of performance, this technique is highly preferable to that of laminectomy in obtaining specimens for routine light microscopy.

A chordoma arose in the lumbar region of the vertebral column from the last two vertebrae of a 112 week old female control F344 rat. Macroscopically it was a mass measuring 32 by 22 mm reaching the pelvic region and pressing against the colon and urinary bladder. Histologically the tumor cells were arranged into lobules and the cells grew in cords. Vacuolated large cells infiltrated the bone and surrounding muscles. This is the first reported case of chordoma in a rodent.

A total of 109 patients were studied after receiving radiation therapy that included a dose to the spinal cord. In addition to irradiation of the primary site, 59 patients received radiation to the lower neck. Transverse myelopathy developed in three patients; all three had been treated with fields to the lower neck. Twelve patients with transient disturbances of the blood circulation in the spinal cord are described. Experiments were carried out on parathyroprival cats with varying degree of motor disorders. Disturbed reflex activity of the spinal cord was characterized by activation of motoneurons and the system of intercalary neurons, abatement of the phenomenon of posttetanic potentiation as well as by conductivity facilitation in the reflex arch. Selective cases have been presented to demonstrate the usefulness of computed tomography in disorders of the spine and spinal cord. The osseous structures are more readily demonstrated than the soft tissues. The cord is difficult and inconsistently identified. However, cystic lesions (syringomyelia) may, at times, be demonstrated. The transverse projection of the CT scan is an added benefit in demonstrating bony spinal lesions. The use of metrizamide with faster scanners should improve the usefulness of this technique in evaluating intraspinal lesions.

Cats were made physically dependent on sodium pentobarbital using the "maximally tolerable" dosing technique. All animals treated this way receive equieffective doses chronically and all become severly dependent. A study was made of the pattern of spinal cord injury in the elderly who are forming an ever-increasing proportion of our population. It was noted that the elderly patients had a higher incidence of cervical injuries than other age-groups with a much lower incidence of dorsal and lumbar injuries.
