Prognostic Factors and Clinical Features of Neonatal Splenic Rupture/Hemorrhage: Two Cases Reports and Literature Review.

Background: Neonatal splenic rupture/hemorrhage (SRH), an extremely rare and potentially fatal presentation, can spontaneously resolve without surgical treatment; However, treatment approaches remain controversial. The present study aimed to describe and analyze the clinical features and therapies of neonatal SRH and therapeutic approaches. Methods: We present the cases of two patients and review another 37 cases reported in English-literature. The literature search included all articles published in PUBMED from inception between January 1968 and December 2019. Demographic data, precipitating …

Early hypovolemic shock and abdominal distention due to neonatal splenic rupture: urgency of diagnosis and management.

UNLABELLED: Splenic rupture in the neonatal period is a rare condition that can be complicated by hemorrhagic shock. The symptoms are not very specific, rendering the diagnosis difficult and often delayed; sometimes only discovered at autopsy. We report five cases diagnosed in the Rhône-Alpes region of France. From these observations and from a review of the literature, the circumstances of the occurrence, the clinical signs, and the therapeutic possibilities are discussed. In the presence of severe anemia with pallor and abdominal distension, particularly in the context of a difficult birth, …

Splenic rupture in a plasma cell leukemia, mobilized with G-CSF for autologous stem cell transplant.

Splenic rupture (SR) is a rare adverse event observed in patients treated with G-CSF as a peripheral hematopoietic stem cell (PHSC) mobilizing agent, mostly in myeloma multiple and amiloidosis; to date, to our knowledge, it has not been previously described in plasma-cell leukemia (PCL). We report a case of a woman with PCL, who presented a SR after PHSC mobilization with Cyclophosphamide+G-CSF. The spleen removed showed hematopoietic foci and amiloid material. In the course of a second mobilization, 2 months after, the patient died from sepsis. We considered it important to report this case, …

Spontaneous splenic rupture in infectious mononucleosis: conservative management with gradual percutaneous drainage of a subcapsular hematoma.

Spontaneous splenic rupture (SSR) is a rare but potentially lethal complication of infectious mononucleosis (IM). Because the inflamed spleen is usually enlarged, congested, and friable, emergency splenectomy is recommended. We describe the conservative management of a SSR in a 16-year-old boy with IM. A pigtail catheter was inserted under ultrasonographic guidance and left in place for 36 h. This allowed the successful evacuation of the hematoma without compromising the splenic parenchyma.

Splenic Rupture Presenting 48 Hours after Colonoscopy: A Case Report Emphasizing the Need for Prolonged Monitoring.

Splenic rupture constitutes a rare yet potentially life-threatening complication following colonoscopy. We report a case of a young male patient who presented with abdominal pain for 2 days, which began 6 hours after colonoscopy. Clinical examination revealed periumbilical tenderness, with laboratory findings significant for leukocytosis (WBC: 20.32 × 109/L), an initial hemoglobin of 113 g/L, and a significantly elevated D-dimer (15.25 mg/L). Abdominal computed tomography confirmed splenic rupture, perisplenic hematoma, and hemoperitoneum. Following multidisciplinary consultation involving hep…

Computed tomography imaging and observation of hemorrhage in traumatic splenic rupture pre and post partial splenectomy.

BACKGROUND: Radiological analysis of splenic images was conducted to evaluate postoperative hemorrhage following partial splenectomy in 50 patients. Computed tomography (CT) was used as the primary imaging method for postoperative assessment. The study included 41 males and nine females aged 16-79 years (mean age 52) treated between December 2019 and July 2023. METHODS: Clinical management focused on identifying preoperative bleeding risks, monitoring blood pressure, managing anticoagulant therapy, and early detection of postoperative hemorrhage. Patient care also included vital-sign monitori

Delayed splenic rupture following video-assisted thoracoscopic left upper lobectomy: a case report and literature review.

Video-assisted thoracoscopic surgery (VATS) is the preferred surgical approach for early-stage lung cancer, though it can rarely cause life-threatening delayed splenic rupture (DSR). With an insidious latent course, DSR frequently eludes early diagnosis and may progress to fatal hemorrhagic shock. We herein describe a 70-year-old male with a prior subtotal gastrectomy who developed DSR on postoperative day 6 after VATS left upper lobectomy. The patient presented with acute hypotension and tachycardia without overt abdominal signs, was confirmed via bedside ultrasonography (US) and diagnostic p…

Chronic HIV infection complicated by pancreatitis, pancreatic tail pseudocyst and atraumatic splenic rupture: a case report.

BACKGROUND: Acute pancreatitis is an uncommon but clinically important complication in people living with HIV (PLWH) and has been linked to HIV itself, older nucleoside reverse transcriptase inhibitors, protease inhibitors via hypertriglyceridemia, and multiple opportunistic or metabolic comorbidities. Atraumatic splenic rupture (ASR) is rare and has been described in association with acute or chronic pancreatitis and, more rarely, with HIV infection. However, the coexistence of chronic HIV infection, pancreatitis with pancreatic tail pseudocyst, and ASR has seldom been reported. We present a …

Splenic artery aneurysm with double-rupture phenomenon and circulatory collapse following anesthesia induction: A case report.

BACKGROUND: Splenic artery aneurysm (SAA) rupture is a rare, life-threatening condition characterized by acute intra-abdominal hemorrhage and hemodynamic instability. Ruptured SAAs may exhibit a biphasic and relatively slow clinical progression, commonly referred to as the "double-rupture phenomenon". The reported incidence of the double-rupture phenomenon ranges 12%-21% in patients with ruptured SAAs, potentially due to variations in intra-abdominal pressure. Following anesthesia induction, muscle relaxation can decrease intra-abdominal pressure, potentially triggering the double-rupture phen

Spontaneous splenic rupture in systemic lupus erythematosus: A rare case report and review of pathophysiological insights.

This case report details a rare instance of atraumatic splenic rupture presenting as minimal clotting hemoperitoneum in a patient with systemic lupus erythematosus (SLE) undergoing immunosuppressive therapy. This uncommon complication highlights the potential for systemic lupus to impair splenic hemostasis, immunity, and filtration, posing risks to functional and life prognosis, particularly in cases of sudden, significant hemoperitoneum. While acute presentations are typical, progressive forms may also manifest. Hemostatic splenectomy remains the definitive treatment. This case emphasizes the…

[Acute necrotizing pancreatitis with hemorrhagic shock in secondary splenic rupture: a case report and literature review].

Atraumatic splenic rupture is a rare complication of acute and chronic pancreatitis. It arises due to its anatomical proximity to the pancreas, for instance, due to erosion of large pseudocysts or walled-of-necrosis (WON).Following we describe the case of a 62-year-old woman who presented for further diagnostics and treatment of acute pancreatitis with the development of large walled-of necrosis (WON) in the pancreatic corpus and tail. During the course, the patient developed a hemorrhagic shock. An emergency computer tomography (CT) of the abdomen revealed a ruptured spleen with a large capsu…

Spontaneous Pathologic Splenic Rupture in a Patient with Plasmodium falciparum Infection, First Case Reported in Israel.

Travelers exposed to malaria may develop severe disease and complications. A less well-known complication is spontaneous pathologic splenic rupture, which is still under-reported and has never been reported in Israel. In this paper, we report a 23 years old healthy young man presenting in the emergency department, two weeks after coming back from Sierra Leone, with intermittent fever, mild tachycardia and mild left upper quadrant abdominal pain. The patient was diagnosed with Plasmodium falciparum infection and developed rapidly after hospital admission spleen rupture. He was managed conservat…

[Hematemesis due to rupture of splenic artery pseudoaneurysm in association with segmental arterial mediolysis:a case report].

A gastric ulcer was detected in a 54-year-old man who underwent upper gastrointestinal endoscopy for hematemesis. An abdominal contrasted computed tomography scan detected a splenic artery aneurysm adjacent to the gastric wall. Endoscopic hemostasis was thought to be risky owing to possible rupture of the aneurysm. Rupture of a splenic artery pseudoaneurysm due to segmental arterial mediolysis (SAM) was diagnosed by abdominal angiography, and subsequently transcatheter arterial embolization was performed. In cases of upper gastrointestinal hemorrhage, hemostasis is often performed during the e

Richter's transformation presenting as splenic rupture after 6 years of complete remission of chronic lymphocytic leukaemia.

Richter's transformation is a rare clinical condition occurring in about 5-10% of patients with chronic lymphocytic leukaemia (CLL). Patients usually present with lymphadenopathy, hepatosplenomegaly and elevated serum lactate dehydrogenase levels. These patients have a very poor prognosis with a median survival of about 10 months. We present a patient, with a history of CLL in complete remission, who presented with splenic rupture requiring splenectomy. She was eventually diagnosed with diffuse large B-cell lymphoma with Richter's transformation.

Successful conservative treatment of spontaneous splenic rupture secondary to Babesiosis: a case report and literature review.

Babesiosis is caused by a protozoan parasite of the genus Babesia. In the United States, the usual infective organism Babesia microti, is most commonly transmitted through the bite of an infected Ixodestick. While the majority of patients exhibit sub-clinical signs and symptoms, significant illness can result. Spontaneous splenic rupture is a life-threatening complication of some viral and protozoan infections. We present a case of Babesiosis with spontaneous splenic rupture in which conservative management with blood transfusions and hospital-based care were successful, and the patient was sp…

Blunt splenic rupture--experience in a preserving non-operatively orientated care team in a tropical hospital.

OBJECTIVES: With a view to the prevention of immediate and later complications of splenectomy, especially the risk of overwhelming post-splenectomy sepsis syndrome (OPSS), conservative treatments have been proposed when the haemodynamic condition of the patient permits this. In this study, we present our experience in a preserving non-operatively orientated care team in a tropical hospital. PATIENTS AND METHODS: Twenty patients admitted to Hoima Hospital, Hoima, Uganda with splenic injuries from blunt abdominal trauma between July and December 1995 were included in the study. For every patien
