The use of wearable sensors for calculating gait parameters has become increasingly popular as an alternative to optoelectronic systems, currently recognized as the gold standard. The objective of the study was to evaluate the agreement between the wearable Opal system and the optoelectronic BTS SMART DX system for assessing spatiotemporal gait parameters. Fifteen subjects with progressive supranuclear palsy walked at their self-selected speed on a straight path, and six spatiotemporal parameters were compared between the two measurement systems. The agreement was carried out through paired da

BACKGROUND: 18F-THK5351 PET is used to image ongoing astrogliosis by estimating monoamine oxidase B levels. 18F-THK5351 preferentially accumulates around the substantia nigra (SN) and periaqueductal gray (PG) in the midbrain under healthy conditions and exhibits a "trimodal pattern." In progressive supranuclear palsy (PSP) and corticobasal syndrome (CBS), the midbrain 18F-THK5351 uptake can be increased by astrogliosis, collapsing the "trimodal pattern." We aimed to elucidate cases in which the "trimodal pattern" collapses in PSP and CBS.
PATIENTS AND METHODS: Participants in the PSP (n = 11)

The differential diagnosis of idiopathic normal pressure hydrocephalus (iNPH) and progressive supranuclear palsy (PSP) is difficult. The importance of proper diagnosis is particularly important for iNPH, which can be effectively treated with a ventriculoperitoneal (VP) shunt. In our case report, we present a unique case of a patient with overlapping symptoms and radiological findings of iNPH and PSP. Our patient underwent the VP shunt after a differential diagnostic evaluation which resulted in significant improvement in their clinical condition and quality of life, albeit for a short time.

BACKGROUND: Progressive supranuclear palsy (PSP) is a neurodegenerative disorder clinically characterized by progressive postural instability, supranuclear gaze palsy, parkinsonism, and cognitive decline caused by degeneration in specific areas of the brain including globus pallidus (GP), substantia nigra, and subthalamic nucleus. However, the pathogenetic mechanism of PSP remains unclear to date.Unbiased global proteome analysis of patients' brain samples is an important step toward understanding PSP pathogenesis, as proteins serve as workhorses and building blocks of the cell.

OBJECTIVES: This study aimed to explore the feasibility of radiomics features extracted from T1-weighted MRI images to differentiate Parkinson's disease (PD) from atypical parkinsonian syndromes (APS).
METHODS: Radiomics features were computed from T1 images of 65 patients with PD, 61 patients with APS (31: progressive supranuclear palsy and 30: multiple system atrophy), and 75 healthy controls (HC). These features were extracted from 19 regions of interest primarily from subcortical structures, cerebellum, and brainstem. Separate random forest classifiers were applied to classify different g

BACKGROUND AND PURPOSE: The main radiological finding in progressive supranuclear palsy (PSP) is reduced midbrain volume. Both qualitative (e.g., hummingbird sign) and quantitative (e.g., area measurements) markers have been noted. Recent studies have shown a similar reduction also in idiopathic normal pressure hydrocephalus (iNPH). The purpose was to investigate the reliability and accuracy of these markers in discriminating PSP from iNPH and controls.
METHODS: Eight neuroradiologists viewed sagittal MR images of the midbrain from 104 subjects: 26 PSP patients, 40 iNPH patients, and 38 healt

Clear speech refers to intentionally modifying conversational speech to maximise intelligibility. This study aimed to compare the speech behaviour of patients with progressive supranuclear palsy (PSP), multiple system atrophy (MSA) and Parkinson's disease (PD) under conversational and clear speech conditions to gain greater pathophysiological insight. A total of 68 participants including 17 PD, 17 MSA, 17 PSP and 17 healthy controls (HC) performed two readings of the same standardized passage. During the first reading, participants were instructed to read the text in an ordinary way, while dur

We investigated 998 serial Japanese forensic autopsy cases (0-101 years old, mean age 61.7 ± 21.9), with no case selection, using immunohistochemistry to detect cases with progressive supranuclear palsy (PSP). Twenty-nine cases (mean age 82.3 ± 7.2 years, 11 males, 18 females) fulfilled the National Institute of Neuronal Disorders and Stroke (NINDS)-PSP pathological criteria (2.9% of all cases, 4.6% of cases over 60). All had neuronal and glial inclusions in the basal ganglia and brainstem. However, 13 cases had low tau pathology and were categorized as atypical PSP. In addition to PSP patholo

PURPOSE: Progressive supranuclear palsy (PSP) is an akinetic-rigid syndrome that can be difficult to differentiate from Parkinson's disease (PD), particularly at an early stage. [99mTc]ethyl cysteinate dimer (ECD) SPECT could represent a widely available tool to assist in the differential diagnosis. In this study we used voxel-based analysis and Computerised Brain Atlas (CBA)-based principal component analysis (PCA) of [99mTc]ECD SPECT data to test whether: (1) specific patterns of rCBF abnormalities can differentiate PSP from controls and PD; (2) networks of dysfunctional brain regions can be

Progressive supranuclear palsy (PSP) is one of the most important causes of parkinsonism non responsive to therapy. Vascular parkinsonism is not uncommon. However, the cause-effect relationship between them is uncertain. We report on a 65 year old man with probable PSP who developed the clinical features of the disease after a ischaemic stroke. Magnetic resonance imaging disclosed a corticospinal tract Wallerian degeneration. There is not such an observation in the literature about this possible correlation.

The local cerebral blood flow (LCBF) at steady state and after the intravenous administration of levodopa (1 mg/kg) was measured by the xenon-enhanced computed tomographic method in six patients with progressive supranuclear palsy (PSP) and in nine patients with idiopathic Parkinson's disease. The baseline LCBF values in most brain regions in patients with PSP were lower than those in patients with Parkinson's disease, and hyperfrontality of the LCBF was lost. In patients with Parkinson's disease, the injection of levodopa markedly increased LCBF, especially in the striatum, thalamus, and inte

BACKGROUND: Progressive supranuclear palsy (PSP) causes falls, dysphagia, pneumonia, fractures, and death, but routine-care timelines and diagnostic-delay survival analyses remain limited.
METHODS: We retrospectively studied 68 patients with probable PSP at a Japanese single center. Outcomes were intervals from symptom onset to first visit, probable PSP diagnosis, aspiration pneumonia admission, fracture hospitalization, and death. Patients were grouped by the median onset-to-diagnosis interval (1005.5 days). Survival was analyzed from onset and diagnosis. Sensitivity analyses modeled delay c

BACKGROUND: Early differentiation of progressive supranuclear palsy from Parkinson's disease is challenging due to overlapping motor features, particularly in early disease stages where clinical misclassification is common. Wearable inertial sensors provide high-resolution gait data that may reveal disease-specific and clinically relevant signatures.
OBJECTIVE: To evaluate whether sensor-derived gait measures can distinguish progressive supranuclear palsy from Parkinson's disease using machine learning, and to compare performance between raw time-series signals and aggregate spatiotemporal va

Progressive supranuclear palsy (PSP) is a tauopathy, considered a movement disorder in which the clinical features can vary through the course of the disease. Some patients could manifest motor dysfunction since the onset of the disease, while others could present with psychiatric symptoms; therefore, PSP may be misdiagnosed as a different type of atypical parkinsonism, as a psychiatric disorder, or as frontotemporal dementia due to their similar clinical manifestations and basic neuroimaging characteristics. Here we present the case of a patient with PSP whose diagnosis was delayed due to an

INTRODUCTION: The atypical Parkinsonian disorder progressive supranuclear palsy (PSP) forms a diagnostic challenge, resulting in frequent misdiagnosis and delay in treatment. Although structural MRI can detect PSP signs at more advanced stages, emerging diagnostic tools such as tau-PET and quantitative susceptibility mapping (QSM) may allow for earlier detection. This exploratory study aimed to investigate differences in QSM data of patients with PSP and healthy controls (HCs) and for the first time assess possible correlations between QSM and tau-PET data in patients with PSP to explore the r

Clinical overlap with multiple other neurological diseases makes the diagnosis of autoimmune encephalitis challenging; consequently, a broad range of neurological diseases are misdiagnosed as autoimmune encephalitis. A 58-year-old man presented with abnormal behavior, irritability for 3 years, oculomotor disturbance, unsteady walking, and dysphagia and was suspected as having anti-dipeptidyl-peptidase-like protein 6 (DPPX) encephalitis as the anti-DPPX antibody was positive in the serum. However, the therapeutic effect of immunotherapy was unsatisfactory. Subsequently, colocalization of increa

The molecular pathogenesis of degenerative parkinsonisms, including Parkinson's disease (PD), progressive supranuclear palsy (PSP), and Multiple system atrophy (MSA), remains largely unknown. To gain novel insight into molecular processes associated with these diseases, we conducted a proteome-wide expression study in prefrontal cortex tissue from a cohort of 181 individuals, comprising PD (N = 73), PSP (N = 18), MSA (N = 17) and healthy control (N = 73). Using marker gene profiles, we first assess the cellular composition of the samples and, subsequently, identify distinct protein signatures
