Using a posterior repair and rectal suspension procedure for those patients who need surgical treatment of rectal prolapse, we have treated 46 patients over a period of 17 years at Children's Mercy Hospital in Kansas City, MO. One patient with caudal dysgenesis died of multiple congenital anomalies following two unsuccessful attempts at posterior repair and suspension. We developed an in vitro suspension co-culture system to examine the interaction of 1st, 2nd and 3rd trimester purified cytotrophoblasts with human endometrium. Eighteen women with stress incontinence underwent endoscopic suspension of the bladder neck by the method of Stamey. The procedure was carried out in a department of general surgery. Pre- and postoperatively the patients were evaluated by pelvic examination with stress test, colpo-cysto-urethrography, urinary flow rate measurement, cystometry, and culture of the urine. Fifteen became fully continent at once, another two after having been treated further by posterior colpoperineorraphy, while one remained incontinent. Thirteen operations of endoscopic bladder neck suspension (Raz method) were performed for 12 patients with stress incontinence between October, 1986 and August, 1988. In all patients, except for one with small bladder capacity, the operations were successful. Aminozolamide (6-amino-2-benzothiazolesulfonamide) is a carbonic anhydrase inhibitor derived from ethoxzolamide that has been shown in gel formulation to lower IOP in rabbits, primates and humans. This open trial was conducted in 50 pregnant women, presenting during the 2nd and 3rd trimesters of their pregnancy typical symptoms of gastro-esophageal reflux (pyrosis, regurgitations, retro-sternal burning sensations, dyspepsia, epigastric burning). The treatment with Gaviscon, suspension to drink, lasted 1 month at a dose of 2 tablespoons, 4 times/day after meals and in the evening at bedtime. Remembering that the diagnosis of stress urinary incontinence is essentially clinical, the authors describe the fundamental stages of their technique, i.e. the musculo-aponeurotic suspension of the bladder neck (Goebell-Stoeckel technique). This operation has enabled them to have good results in 88% of their cases. The long-term study of these results shows slight deterioration of the success rate which becomes stable after 2 years.

A new technique for the correction of trochanteric bulges is described. A buried dermal-fat flap is used as a suspensory mechanism to hang the weight of the thigh skin and subcutaneous tissues on the deep fascia, rather than on the upper skin flap. This technique is particularly applicable to patients with asymmetry of the buttocks and thighs as well as those with ptosis of the buttocks. Another advantage is that it creates a new gluteal fold at a predetermined higher level.
