[Brugada syndrome--a rare cause of syncope and sudden death].

Brugada syndrome is a rare hereditary condition comprising electrocardiographic findings and an increased risk of sudden death due to ventricular fibrillation. The transmission is autosomal dominant with incomplete penetrance, mainly affecting males. The clinical manifestations include syncope, sudden cardiac death, nocturnal agonal breathing, documented ventricular tachycardia/fibrillation, and inducibility of arrhythmias during electrophysiologial study. The ECG should typically have an appearance of a right bundle branch block with a coved ST-segment elevation ≥ 2 mm, followed by a negative…

Tilt-table testing of patients with pacemaker and recurrent syncope.

The diagnosis of recurrent syncope in patients with pacemakers (PM) is quite challenging and the etiology of syncope is often multifactorial. To portray the mechanism of syncope in PM patients, we report the results of head-up tilt table testing (HUT) in a series of patients with PM, originally implanted for reasons other than neurally mediated syncope, referred due to syncope or pre-syncope (aborted syncope, vertigo, suspected orthostatic hypotension). Forty-one patients with PM undergoing a HUT in our syncope unit between January 1st, 2007 and December 31st 2011 were included. A standard HUT…

Cardioinhibition can be the unique manifestation of epilepsy-like syncope.

Sudden death and syncope remain frequently unexplained despite numerous investigations. Here, we report the case of a pacemaker-implanted patient who presented during video-polysomnography recording a complete atrioventricular block simultaneously with an electrical seizure. Remarkably, the patient was completely asymptomatic. He had a history of recurrent syncope previously diagnosed as convulsive vasovagal syncope with cardioinhibition. This observation challenges the current belief that epilepsy-like syncope is a partial complex seizure systematically characterized by a stereotypical clinic…

Syncope as initial symptom for nephrotic syndrome: a case report.

Although syncope and nephrotic syndrome are frequently encountered independently in pediatric practice, syncope as the initial symptom for nephrotic syndrome is rarely observed in the pediatric age group. In this report, we present the case of 3-year-old boy with nephrotic syndrome who presented with a history of three episodes of syncope before admission. The syncope occurred after excessive fluid loss or inadequate intake of fluids and was relieved spontaneously. History taking revealed that the early morning palpebral edema, and laboratory tests showed decreased plasma protein levels and el…

Intermittent brugada syndrome presenting with syncope in an adult female.

Background. Brugada syndrome accounts for 4-12% of all sudden deaths worldwide and at least 20% of sudden deaths in patients with structural … y encountered clinical entity which may have a fatal outcome. This syndrome primarily presents with syncope. It should be considered as a component of differential diagnosis in patients with family history of syncope and sudden cardiac death.

An ependymal cyst in cerebello-pontine angle presenting with syncope.

Intracranial ependymal cysts are rare, congenital, benign lesions. These commonly occur in the supratentorial regions and usually generate no symptoms. The cerebellopontine angle (CPA) is an extremely rare site for ependymal cysts. Furthermore, there are no previous reports of CPA ependymal cysts related to syncope. We report a case of ependymal cyst in the left CPA with syncope. The patient underwent a cardiologic evaluation for syncope after admission, but there were no definite cardiologic abnormal findings. He underwent fenestration into the subarachnoid space, and the pathologic diagnosis…

Implantable diagnostic monitors in the early assessment of syncope and collapse.

The implantable diagnostic monitor, or loop recorder (ILR) is a subcutaneous monitor capable of continuous ECG monitoring up to 3 years. It is an elegant investigative tool for the assessment of patients with recurrent, unexplained syncope in whom an arrhythmic cause needs to be excluded, and is now increasingly utilised very early in the diagnostic work-up of these patients, in line with current guidelines. This review examines the evidence underpinning these recommendations as well as the relevant clinical trials evaluating the use of the ILR in syncope. Continued research will be needed to …

A young healthy male with syncope and complete heart block.

INTRODUCTION: It is rare to see heart block as the first-and-only presentation of Lyme disease, the reported incidence being just 1% in untreated patients. CASE PRESENTATION: We report a patient who presented with syncope secondary to a fluctuating heart block. He had no previous manifestations of Lyme disease. Lyme carditis was suspected after the other usual etiologies of heart block were ruled out. Later, serologies came positive and patient's heart block resolved on ceftriaxone therapy. CONCLUSION: In at-risk individuals, clinicians should rule out this readily-treatable cause of heart b…

[Calcified amorpheus tumour of the heart as the cause of near syncope].

Calcified amorphoeus tumour of the heart (cardiac CAT) is a rare non-neoplastic tumour of the heart. To the best of our knowledge, this is the first case report of cardiac CAT in Scandinavia. The patient was a 55-year-old woman with obesity, hypertension, hyperlipidaemia, a history of smoking, and a family history of ischaemic heart disease. She presented with dyspnoea and near syncope. The tumour was resected, and cytopathology was preformed on its content. Based on the cytopathology, the pathologist concluded that the tumour most likely was cardiac CAT, though a calcified myxoma could not be…

Syncope in a patient being treated for hepatic and intestinal amoebiasis.

A 63-year-old man presented to our hospital with amoebic liver abscess and was treated successfully for the same. During the course of his treatment, he developed syncopal attacks and was found to have Torsades de Pointes on electrocardiogram. The patient was treated with intravenous magnesium and direct current cardioversion. Hypokalaemia, chloroquine and sepsis were suspected to have precipitated the arrhythmia. The patient remained arrhythmia-free following the correction of these factors.

Dreaming experience as a useful diagnostic clue for syncopal episodes.

BACKGROUND: The differential diagnosis between epileptic seizures and syncopes is a common occurrence in clinical practice. The manifestations of seizure and syncope sometimes overlap, and available diagnostic testing often not provides a conclusive answer. Syncope is often preceded by a symptom complex characterized by lightheadedness, generalized muscle weakness, giddiness, visual blurring, tinnitus, and gastrointestinal symptoms. These subjective symptoms are very important in guiding the diagnosis. In our experience, the impression of coming out of a dream after the syncopal episode is a s…

Hyperoxic chemoreflex sensitivity is impaired in patients with neurocardiogenic syncope.

BACKGROUND: During the development of neurocardiogenic syncope (NCS) postural dependant venous blood pooling sets off a cascade of autonomic reflexes. This causes an initial rise in sympathetic tone, which is followed by an overshoot parasympathetic activation resulting in systemic vasodilatation and/or sinus bradycardia. However, other factors like associated hyperventilation or changes in blood gas content may also contribute to syncope. Hyperoxic cardiac chemoreflex sensitivity (CHRS) is an autonomic functional test that describes the heart rate decrease in response to increases in blood ox…

Microscopic polyangiitis in an adolescent presenting as severe anemia and syncope.

Microscopic polyangiitis (MPA) is an autoimmune systemic vasculitis of small vessels. The condition has been best characterized in older adults and little is known of the natural history of this disease in children and adolescents. In this report, a case of an adolescent presenting with symptomatic anemia and syncopal episodes is described. An extensive evaluation ultimately led to the diagnosis of MPA. The unique findings in this case and review of the literature are presented, outlining the variable clinical presentations and challenge of diagnosing this condition in pediatric patients.

Pathophysiology, diagnosis, and treatment of orthostatic hypotension and vasovagal syncope.

Orthostatic hypotension (OH) occurs in 0.5% of individuals and as many as 7-17% of patients in acute care settings. Moreover, OH may be more prevalent in the elderly due to the increased use of vasoactive medications and the concomitant decrease in physiologic function, such as baroreceptor sensitivity. OH may result in the genesis of a presyncopal state or result in syncope. OH is defined as a reduction of systolic blood pressure (SBP) of at least 20 mm Hg or diastolic blood pressure (DBP) of at least 10 mm Hg within 3 minutes of standing. A review of symptoms, and measurement of supine and s…
