Crown lengthening and restorative treatment in mutilated molars.

Crown lengthening has been advocated as a treatment modality to restore teeth with a clinical crown reduced subsequent to different kinds of trauma. Multirooted teeth, however, present certain anatomic features, such as the furcation area and corresponding interradicular bone, the retromolar area, and the external oblique ridge, that may limit the possibility for soft tissue and bone reduction and minimize the effectiveness of crown-lengthening procedures. This article describes surgical modifications to overcome the anatomic difficulties that multirooted teeth present when crown lengthening i

Continuous tooth generation in mouse is induced by activated epithelial Wnt/beta-catenin signaling.

The single replacement from milk teeth to permanent teeth makes mammalian teeth different from teeth of most nonmammalian vertebrates and other epithelial organs such as hair and feathers, whose continuous replacement has been linked to Wnt signaling. Here we show that mouse tooth buds expressing stabilized beta-catenin in epithelium give rise to dozens of teeth. The molar crowns, however, are typically simplified unicusped cones. We demonstrate that the supernumerary teeth develop by a renewal process where new signaling centers, the enamel knots, bud off from the existing dental epithelium.

3D digital analysis of tooth movement with magnets and elastics in vitro.

OBJECTIVES: Magnets have many advantages in orthodontics, and our previous studies confirmed their therapeutic potential through 3D-data analysis. The aim of this study was to compare tooth movements, including rotation, obtained via magnetic and elastic forces in crowded cases in vitro. METHODS: Typodont models mimicking a crowded case were prepared. In the magnetic force-driven orthodontics (MG) group, Nd-Fe-B magnets were attached to the labial surfaces from UR4 to UL4 for attracting force, and to UR6 and UL6 for repulsing force. For the elastic force-driven orthodontic (EL) group, bracket

Integrating Various Ceramic Materials and Adhesive Concepts to Achieve a Functional and Esthetic Result.

Various adhesive techniques and ceramic materials can be incorporated to create a biological, functional, and esthetic smile and occlusion. In this case report, a 37-year-old woman presented to a dental school clinic seeking treatment for missing teeth. She also was dissatisfied with the color of her existing teeth and desired a broader smile (buccal corridor). In addition, tooth No. 9 had external resorption and needed to be extracted and replaced. With a previous history of unsatisfactory orthodontic treatment, the patient declined orthodontics despite the benefits it could have provided. A

Witkop's tooth and nail syndrome: a multifaceted approach to dental management.

Witkop's tooth and nail syndrome is a rare autosomal dominant disorder of ectodermal dysplasia characterized by hypodontia and nail dysplasia. Mutations in MSX-1 have been shown to be associated with this syndrome. There is failure of development and eruption of the dentition. Tooth shape may vary; the most common forms are conical and narrow crowns. The nails may be spoon shaped and slow growing and affect both finger and toe nails. The nail involvement is more severe in childhood. The present case describes a 14-year-old boy who showed the characteristic features of Witkop's syndrome. A mult

A 2-year retrospective cohort study of pulp-related outcomes of permanent anterior teeth with uncomplicated crown fractures.

BACKGROUND: Uncomplicated crown fractures (UCFs) are the most common form of traumatic dental injury in permanent dentition. However, studies on long-term pulp outcome in South Asian populations are limited. This study aimed to analyse pulp survival of permanent anterior teeth with uncomplicated crown fractures in a Sri Lankan population. METHODS: A retrospective cohort study was conducted following STROBE (Strengthening the Reporting of Observational Studies in Epidemiology) guidelines. Records of 257 teeth with uncomplicated crown fractures in patients attending a tertiary dental care centr

Clear Aligner Treatment of a Class II Division 2 Patient With Multiple Anterior Crowns and Molar Mesial Tipping Through Cantilever Arms: A Case Report.

Patients with Class II Division 2 malocclusion, often presenting with severe deep overbite and poor facial esthetics, are one of the most challenging cases in orthodontic practice. This case report shows the successful treatment of a Class II Division 2 deep bite patient with multiple anterior crowns using clear aligners. After the extraction of Tooth 45, we moved the right mandibular molars mesially to achieve a Class I relationship. Cantilever arms and elastics were used to correct the molar mesial tipping. We precisely controlled the space between prostheses by digital design. At the end of

Occlusal considerations in implantology: good occlusal practice in implantology.

UNLABELLED: This article is concerned with implants that are being used for fixed crown and bridgework rather than removable prostheses. The huge increase in the provision of implants over the past two decades is set to continue. Most of the research is related to avoiding failures in implants. This research, in the main, has concentrated on the essential interface between the artificial implant and living bone: osseointegration. The other interface, which is worthy of our full attention, is the one between the implant-supported crown and the antagonist tooth: the occlusion. CLINICAL RELEVANC

A novel management of vertical root fracture in an endodontically treated mandibular premolar with titanium screw: A case report.

INTRODUCTION: Vertical root fracture is one of the conditions prevalent in endodontically treated teeth. The treatment options include extraction of the fracture fragment or the affected tooth as a whole. This case report highlights the successful management of vertical root fracture in an endodontically treated mandibular first premolar. METHOD: Surgical intervention was initiated, and a full-thickness flap was elevated. The separated fracture segment was safely retrieved, and the existing caries were removed and then reattached to the main fragment with the help of a titanium screw. The too

Palatal Supernumerary Tooth in a Pediatric Patient.

This report describes the case of a child with a congenital palatal lesion that grew rapidly in the first year of life and was found to be a supernumerary tooth. A 14-month-old male presented with a congenital midline palatal lesion visible behind his newly erupted maxillary central incisors. The lesion had been present since birth and was round, raised, firm, and covered with normal-appearing mucosa. The results from CT imaging indicated the lesion was a rudimentary tooth crown. It was excised and confirmed to be a supernumerary tooth. The patient healed without complications. Congenital pala…

Deposition of calcified tissue around an overextended gutta-percha cone: case report.

Root canal treatment was performed in a mandibular right second premolar with a periapical lesion and apical resorption. The root canal was prepared with K-files using the step-back technique and 3% NaOCl as an irrigant; during obturation gross overfilling of gutta-percha occurred. The tooth was permanently restored with a post and core along with a crown. Although healing of the periapical lesion occurred and the patient reported that he was symptom-free, the tooth was extracted after 4 years because of a subgingival root fracture. Following extraction the tooth was examined with SEM. The exa

Esthetic maxillary arch vertical location for the osseointegrated cylinder implant.

The single tooth edentulous space can be treated with an osseointegrated implant if presurgical prosthetic planning is specific and accurate. To establish accurate placement of the implant, the position of the mandibular incisor must be studied in relation to the exit point of the implant in the maxillary arch. A sufficient space for placing the implant abutment components, as well as for the crown, must be provided.

Rare presentation of adenomatoid odontogenic tumor in a pediatric patient: a case report.

The adenomatoid odontogenic tumor (AOT) is a painless benign tumor with slow growth, usually asymptomatic. It has three variants: follicular, extrafollicular, and peripheral. In the follicular type, the tumor is associated with an impacted tooth, and maxillary canines are the most frequently affected. Association with primary teeth is very rare. Treatment consists essentially in a total lesion enucleation. The objective of this paper is to present the clinical case of a 7-year-old female patient with an adenomatoid odontogenic tumor associated with the crown of the left lower deciduous canine

Conservative treatment of dens evaginatus of anterior teeth.

Dens evaginatus is a developmental malformation characterized by the presence of an extra cusp. In the anterior dentition, dens evaginatus is more commonly found in the maxilla and on the palatal surface of the tooth. Treatment may include root canal therapy followed by either an aesthetic restoration or a full crown coverage. In cases of mesiodens, supernumerary teeth or crowded dentition, extraction is often indicated. Presented is a case of a conservative treatment modality of dens evaginatus in a mandibular central incisor.

Case of hidden tooth: mesiodens fortuitously discovered on a cone-beam CT examination.

Mesiodens is the most common type of supernumerary tooth, located between the maxillary central incisors. A young man was referred by his orthodontist for management of a supernumerary tooth located in quadrant I, superposed to the bottom of the right maxillary sinus, distally orientated with the crown in contact with the apex of the palatal root of the maxillary first molar. The tooth was found on a panoramic radiography before starting his orthodontic treatment. To remove it and in order to study, its relationship to the anatomical structures a cone-beam CT examination was performed. This re

Central odontogenic fibroma of the maxilla.

Central odontogenic fibroma (COF) is an uncommon tumor that accounts for 0.1% of all odontogenic tumors; it has been defined as a benign neoplasm of the jaw. Clinically, the lesion grows slowly and leads to cortical expansion. Radiologically, the most common finding is multilocular radiolucency. The lesions are associated with the crown of an unerupted molar, premolar, or incisor tooth and in some cases, with root resorption or displacement. Histologically, the lesion is characterized by mature collagen fibers and numerous fibroblasts. COF responds well to surgical enucleation with no tendency
