Management of an Imminent Pathological Fracture of a Fibular Neomandible Via a Minimally Invasive Approach-A Case Report.

Fibular free flap reconstruction remains the workhorse of postmandibulectomy reconstruction. Dental implantation to support a dental prosthesis is a sought-after outcome when the area of resection involves tooth-bearing zones. Chronic perisoft tissue pedicle hyperplasia with secondary infection leading to gradual bone loss is a simple complication to manage in the general population, but it becomes a serious issue in the fibula mandibular reconstruction patient in that it can lead to pathological fracture of the fibula. A case of a patient with a near fracture of his fibula mandibular reconstr

Immediately loaded mandibular fixed implant prostheses using the all-on-four protocol: a report of 183 consecutively treated patients with 1 year of function in definitive prostheses.

PURPOSE: The purpose of this study was to evaluate a specific protocol using four implants to support immediately loaded fixed prostheses to restore edentulous and partially edentulous mandibles and report on the outcome after 1 year of function with the definitive prostheses. MATERIALS AND METHODS: A retrospective study was conducted of all patients who were treated between June 2008 and December 2010 with fixed prostheses that were loaded immediately after placement of implants. The provisional prostheses were later replaced with computer-aided design/computer-assisted manufacture titanium

Dental and facial injuries following sports accidents: a study of 130 patients.

Details of injuries to the face and teeth have been collected over a five-year period. One hundred and thirty patients were seen with injuries resulting from 21 different sports. Estimates of the numbers of people playing various team sports in the Bradford area suggest that the incidence of facial injuries is most common in rugby, followed by soccer and cricket. Miniature motor cycling and horse-riding are the most dangerous individual sports. The ages of injured patients varied widely in different sports, but the severity of injuries sustained is less than those due to other causes.

Single- versus multi-visit approach for fragment reattachment in complicated crown-root fractures: a cohort study.

BACKGROUND: Complicated crown-root fractures are a type of tooth fracture that involves the enamel, dentin, and cementum and accompanied by pulp exposure. The treatment of a complicated crown-root fracture is always challenging due to the difficulties in achieving a hermetic seal and a stable restoration with a fracture level close to the crestale bone level. This study aimed to evaluate and compare the efficacy of single-visit and multi-visit approaches for fragment reattachment in complicated crown-root fractures of anterior teeth. METHODS: Two cohort consist of 10 adolescent patients in ea

Role of sclerostin deletion in bisphosphonate-induced osteonecrosis of the jaw.

PURPOSE: Bone resorption inhibitors, such as bisphosphonates (BP) and denosumab, are frequently used for the management of osteoporosis. Although both drugs reduce the risk of osteoporotic fractures, they are associated with a serious side effect known as medication-related osteonecrosis of the jaw (MRONJ). Sclerostin antibodies (romosozumab) increase bone formation and decrease the risk of osteoporotic fractures: however, their anti-resorptive effect increases ONJ. Thus, this study aimed to elucidate the role of sclerostin deletion in the development of MRONJ. METHODS: Sclerostin knockout (S

Infection after rigid internal fixation of mandibular fractures: a clinical and radiologic study.

Of 214 patients with mandibular fractures treated following the AO/ASIF principle of rigid fixation, 13 patients (6.1%) developed postoperative infection. The yearly percentage decreased remarkably during the study period. When rigid fixation became a routine method, the infection rate was as low as the corresponding figure for cases treated with nonstable techniques and maxillomandibular fixation (MMF). All but one of the infected fractures were in the angular region of the mandible. Teeth had been extracted from the fracture line in 9 of the 10 dentulous patients. It was concluded that erron

Fracture resistance of teeth obturated with different root canal sealers: a systematic review.

BACKGROUND: Endodontically treated teeth are more prone to fracture due to structural loss during canal preparation and alterations in dentin biomechanics. Root canal sealers and obturation materials contribute not only to sealing ability but also to potential reinforcement of the remaining tooth structure. However, their influence on fracture resistance remains unclear due to variability in study designs and outcomes. OBJECTIVE: To systematically evaluate and synthesize the available evidence on the fracture resistance of endodontically treated teeth obturated with different root canal seale

Comparative Evaluation of Survival Against Fracture in Premolars Treated Endodontically for Cervical Lesions and Restored with Resin and Crowns.

BACKGROUND: For endodontic treatment, coronal restoration is crucial in preventing coronal leakage and protecting against fractures in the remaining tooth structure. AIM: To evaluate the effect of the restorations in endodontically treated teeth on survival against fracture in premolars having cervical lesions to identify the prognostic factors depicting the fracture. METHODS: Endodontically treated premolars had exposed cervical lesions, and crowns or resin composites were assessed. In the included premolars, the fracture presence was noted along with the prognostic factors associated. The

Dental anomalies in mental patients.

Fifty-three patients were examined for dental abnormalities at an institution specializing in care of the mentally deficient in Bauru, São Paulo State. The incidence of teeth with abnormal morphology, mainly second molars with an abnormal number of cusps, was extremely high. Enamel hypoplasia was frequently found in anterior teeth, and the percentage of fractured maxillary incisors was significantly higher than that observed in normal individuals.

Treatment of a crown-root fracture with concomitant root fracture.

The aim of this case report was to present a treatment strategy for severely injured teeth in children and adolescents by using periodontal surgery and adhesive techniques. A 16-year-old boy presented after a severe bicycle accident. A radiograph revealed a root fracture with connective tissue healing and displacement. Clinically, a crown-root fracture as well as an uncomplicated crown fracture of the same fragment was obvious. The root canal was prepared and filled with gutta-percha to the fracture line. A full periodontal flap was performed, and the crown-root fragment was adhesively fixed.

Orthodontic treatment for oral rehabilitation after multiple maxillofacial bone fractures.

We present the orthodontic treatment of a patient with occlusal dysfunction after plastic surgery for multiple maxillofacial bone fractures caused by a traffic accident. The patient had mandibular deviation to the right because of inappropriate repositioning and fixation of the fractured bone and complete avulsion of both mandibular central incisors. The bilateral mandibular incisors, canines, and premolars were also suspected of partial avulsion or alveolar bone fracture. Several tests, including percussion and dental computed tomography, were performed on these teeth to rule out ankylosis an

Cemental tear: An overlooked finding associated with rapid periodontal destruction. A case series.

Cemental tear is defined as cementum fragment completely or partially detached from the root surface, and it has been associated with localized rapid periodontal breakdown. Although history of trauma and/or attrition may be risk factors, the etiopathology of cemental tear remains unknown. This case series aims to discuss the clinical, radiographic and histopathologic features of cemental tears to aid clinicians in making differential diagnosis. Three teeth from three patients presenting a periradicular lesion underwent an exploratory surgery to determine the cause and provide treatment. Soft a

Effect of splinting times on the healing of intra-alveolar root fractures in 512 permanent teeth in humans: A Scandinavian multicenter study.

BACKGROUND/AIM: Long-term splinting of teeth with intra-alveolar root fractures has been considered important for the deposition of hard tissue between the root fragments. The aim of this study was to compare the healing outcomes in teeth with intra-alveolar root fractures relative to splinting times in three dental centers in Scandinavia, using historical data. MATERIALS AND METHODS: A total of 512 maxillary and mandibular incisors from three dental trauma centers were included in the study. Two of the centers used long-term splinting protocols of two to three months while the other center u

Cemental tear: Literature review, proposed classification and recommendations for treatment.

Cemental tears are an important condition of relevance to Endodontics but are often overlooked. A cemental tear is the partial or complete detachment of the cementum from the cemento-dentinal junction or along the incremental line within the body of cementum. The limited attention received is most likely due to the limited awareness amongst dental professionals and challenges in accurately diagnosing them, resulting in misdiagnosis and erroneous treatment. The aim of this review is to describe the: (i) epidemiology and predisposing factors; (ii) clinical, radiographic and histological features

Discrete element models of tooth enamel, a complex three-dimensional biological composite.

Enamel, the hard surface layer of teeth, is a three-dimensional biological composite made of crisscrossing mineral rods bonded by softer proteins. Structure-property relationships in this complex material have been difficult to capture and usually require computationally expensive models. Here we present more efficient discrete element models (DEM) of tooth enamel that can capture the effects of rod decussation and rod-to-interface stiffness contrast on modulus, hardness, and fracture resistance. Enamel-like microstructures were generated using an idealized biological growth model that capture
