This article describes how dentists can recognize osteoporosis before fractures develop, and discusses whether osteoporosis affects tooth loss or inhibits implant osseointegration. Some success in diagnosing osteoporosis has been obtained using clinical questionnaires that attempt to identify those who have strong risk factors for the disease, and analysis of the sparse trabeculation and thinning of the mandibular cortex often seen in dental panoramic radiographs. The role of osteoporosis in periodontal disease is unclear as there are many conflicting reports, but the evidence suggests that to…

The dental remains of ten adult chimpanzees from Gombe National Park, Tanzania, were examined for enamel attrition, caries, abscesses, periodontal disease, and tooth loss. Age was the underlying factor in the development of dental pathology, in that enamel wear was present to some extent in all ten but was uniformly severe only in the three for whom estimated age at death was 39-43 years. In turn, enamel wear appears to have been the direct cause of abscess development, periodontal disease, and tooth loss. Periodontal disease was commonly expressed as alveolar resorption, particularly around t…

The aim of the present study was to use a decision making model in order to assess the periodontal treatment needs of a random sample of employees in a large Swedish industrial corporation. The model used provided data on critical bone loss limits for different tooth types and ages, beyond which treatment must be initiated, in order to fulfill the goal of maintaining all teeth in a functional state throughout life. A sample comprising 192 subjects belonging to four age strata (31-35 yr, 41-45 yr, 51-55 yr, and 61-65 yr) was involved. From each subject, a full mouth series of intra-oral radiogr

Dental trauma continues to cause loss of teeth, particularly in the anterior region of the maxilla. Reimplanting the tooth with the fragment repositioned by turning it through 180 degrees presents a conservative treatment option that makes it possible to keep a tooth, at first indicated for extraction, functioning and with a satisfactory esthetic appearance.

Vertical root fracture (VRF) has been a great challenge in dentistry; most fractures often result in tooth extraction. Inflammation of tissues around the fractured root is the main reason for tooth extraction. Based on the strategic importance of some fractured teeth, treatment may be necessary and often complicated. However, performing a proper repair or even splinting the fractured segments may result in tooth preservation. Accordingly, in this case we report a new method for fractured tooth preservation. The surgical exposition of the fracture tooth was carried out through the radicular por

Cemental tears have been described as a fragment of cementum becoming detached from the root surface, which can lead to severe periodontal breakdown. A 50-year-old woman with no history of periodontal attachment loss had an asymptomatic, isolated deep pocket on the mesial aspect of tooth No. 14. The radiographic examination revealed a thin, chip-like radiopacity along the lateral aspect of tooth No. 14. Periodontal flap surgery revealed a cemental tear on the apical part of the tooth. The root fragment was removed and the intrabony lesion was treated with enamel matrix derivative. A clinical e

The loss and replacement of anterior maxillary teeth poses several challenges. In patients refusing implant surgery, when minimal tooth reduction is desired, a fiber-reinforced composite fixed-partial denture may be used as a conservative alternative to a conventional fixed-partial denture for replacement of a single missing tooth. This article describes a clinical technique and six-year follow-up. The patient presented with a missing maxillary central incisor due to localized juvenile periodontitis. The abutment teeth were clinically stable. The advantage of supragingival margins and minimal

OBJECTIVE: The aim of this study was to investigate whether labial tooth inclination and alveolar bone loss affect the moment per unit of force (Mt/F) in controlled tipping and consequent stresses on the periodontal ligament (PDL).
METHODS: Three-dimensional models (n = 20) of maxillary central incisors were created with different labial inclinations (5°, 10°, 15°, and 20°) and different amounts of alveolar bone loss (0, 2, 4, and 6 mm). The Mt/F necessary for controlled tipping (Mt/Fcont) and the principal stresses on the PDL were calculated for each model separately in a finite element anal

The wide range of interrelations between orthodontics and periodontology shows that besides the preventive significance in the mixed dentition, another focal point is the comprehensive rehabilitation of periodontally affected patients. With increasing experience in the field of adult orthodontics, setting therapeutic limits becomes increasingly difficult. It is impossible in principle to define in exact metric terms the probing depth or attachment loss up to which orthodontic tooth movement is feasible. The fact is rather that the orthodontic-periodontal treatment goal can be defined only afte

The present longitudinal radiographic investigation was designed to study the change in the alveolar bone height over 6 years in relation to tooth type, age and the presence of previous bone loss in a group of 142 subjects (age 18-67 years) who were not under systematic periodontal treatment. The subjects were examined radiographically at baseline, and after 2 and 6 years, and sites showing significant (greater than or equal to 1 mm) change in the alveolar bone height during 2 consecutive examinations were identified. 90.2% of all examined sites showed no bone loss during the 6 years, while 8.

The organization and renewal capacity of teeth vary greatly among vertebrates. Mammals have only one row of teeth that are renewed at most once, whereas many nonmammalian species have multirowed dentitions and show remarkable capacity to replace their teeth throughout life. Although knowledge on the genetic basis of tooth morphogenesis has increased exponentially over the past 20 years, little is known about the molecular mechanisms controlling sequential initiation of multiple tooth rows or restricting tooth development to one row in mammals. Mouse genetics has revealed a pivotal role for the

Although the traditional treatment planning options still exist whether to open or close space associated with a congenitally missing tooth, with the advent of implants, treatment planning for space opening has changed. Often there is a delay between the completion of orthodontic treatment and the placement of implants with a risk of bone loss occurring in the missing tooth space over the intervening years. A greater emphasis during treatment planning is now placed on the preservation of bone in the missing tooth space where space opening is indicated.

Implant treatment in individuals with periodontitis-associated tooth loss is frequently debated. The objective of the present systematic review was to assess the principles and outcome of implant treatment in periodontitis-susceptible patients. Studies considered for inclusion were searched in MEDLINE (PubMed) and relevant journals were hand-searched. The search was restricted to studies published in English from 1980 to 2006. Prospective and retrospective cohort studies assessing implant treatment in partially and totally edentulous individuals with a history of periodontitis-associated tooth…

Cases in which teeth have only the cervical third remaining from orthodontically induced external root resorption, cast the following doubts: 1) What care should be taken to keep these teeth in mouth with the least risk possible? 2) What care should be taken with regards to reading of imaging exams, particularly in terms of accurately determining cervical root and bone loss? 3) Why is not endodontic treatment recommended in these cases? The present study aims at shedding light on the aforementioned topics so as to induce new insights into the theme.

Retrobulbar hemorrhage (RBH) refers to hemorrhage within the bony orbital cavity and most commonly results from periorbital surgery or trauma. RBH following tooth extraction is a rare occurrence. Patients with RBH will endorse symptoms of periorbital pain, double vision, or vision loss, and present with evidence of proptosis, chemosis, or subconjunctival hemorrhage. Irreversible vision loss may occur if orbital compartment syndrome (OCS) results in the setting of RBH and is not expediently treated. Herein we present a case of a 72-year-old female who developed a RBH and OCS immediately after r

Occasionally, tooth fractures may demonstrate exaggerated pathologic root migration (PRM), often resulting in tooth loss. The early propagation of the root fracture may not be readily identifiable through clinical inspection and conventional radiographic imaging. Ultimately, increased root separation, isolated deep periodontal probing depth, and characteristic radiolucent changes may facilitate the diagnosis. This article describes 3 patients with unusual presentations of PRM in endodontically treated teeth restored with full-coverage crowns. One case illustrates the use of cone beam computed …

The clinical condition of a patient with chronic adult-type periodontal disease, as well as gingival recession reaching the tooth apex on a mandibular lateral incisor, is discussed. Because the use of conventional techniques would have resulted in tooth loss, the guided tissue regeneration procedure was applied, with successful results.

Primary hyperoxaluria is a rare, inherited autosomal recessive disease caused by defects in the metabolism of glyoxylate. Oral manifestations of hyperoxaluria are rare. However, bone and tooth resorption may be the result of chronic inflammation and the presence of osteoclastic cells surrounding the oxalate crystal deposit. A deposit of calcium oxalate in the periodontium was identified in a patient with end-stage renal disease. Dental radiographs indicated bone loss and external tooth resorption. Radiolucent image in the inferior incisor region was observed and removed. The tissue showed gran

INTRODUCTION: Investigating the stress distribution generated by orthodontic forces within the periodontal ligament (PDL) is critical for quantifying mechanical stimuli, correlating them with subsequent histological responses, and ultimately identifying evidence-based optimal force levels. This study aimed to evaluate the combined biomechanical effects of alveolar bone height loss, tooth movement type, and alveolar bone density on the initial retraction of the maxillary central incisor, and to quantify the weighting of each factor on PDL stress.
METHODS: A three-dimensional (3D) finite elemen
