Experimental tractional retinal detachment in rabbits. Clinical picture and histopathologic features.

A simple reproducible model for the creation of tractional retinal detachments in rabbits is presented. This model comprises retinal membranes composed of different cell types derived from ocular tissues, and eliminates the need for lengthy and expensive tissue culturing procedures. The injected cell bolus rapidly migrates to the optic disc and forms surface retinal membranes. These lead to tractional retinal detachment confined to the area of the vascular medullary rays. The rapidity and severity of the clinical process seems to be directly dependent upon the number of cells injected. Good vi…

Exercise only, exercise with mechanical traction, or exercise with over-door traction for patients with cervical radiculopathy, with or without consideration of status on a previously described subgrouping rule: a randomized clinical trial.

STUDY DESIGN: Randomized clinical trial. OBJECTIVES: To examine the effectiveness of cervical traction in addition to exercise for specific subgroups of patients with neck pain. BACKGROUND: Cervical traction is frequently used, but its effectiveness has not been adequately examined. Existing studies have failed to target patients most likely to respond. Traction is typically recommended for patients with cervical radiculopathy. A prediction rule has been described to identify a narrower subgroup of patients likely to respond to cervical traction. METHODS: Patients with neck pain and signs o…

Clinical application research of internal traction-assisted suspended closure in endoscopic transcecal appendectomy.

BACKGROUND: Endoscopic transcecal appendectomy (ETA) has gained increasing clinical adoption as a minimally invasive surgical approach. However, conventional appendiceal stump closure techniques present notable limitations due to their operational complexity. This technological constraint underscores the clinical need to develop optimized closure methodologies. In this study, we proposed an innovative "internal traction-assisted suspended closure (ITASC)" technique and conducted a multicenter retrospective comparative study to evaluate its clinical efficacy. METHODS: Data from patients who un…

Clinical Efficacy of an Innovative Multidimensional Traction Therapy in Moderate Adolescent Idiopathic Scoliosis.

This protocol aims to determine whether a patient-specific, multidimensional traction (MDT regimen), delivered alongside standard bracing and mirror-corrective exercises, can improve structural correction in moderate adolescent idiopathic scoliosis (AIS). The study is a pilot randomized clinical trial (RCT) enrolling skeletally immature adolescents (Cobb 20 - 45°) and allocating them 1:1 to (i) brace plus mirror-corrective exercises or (ii) the same program augmented by MDT. Traction vectors are individualized to the curve pattern and flexibility. Interventions are provided over a 3-month peri…

Self-Made Internal Traction Method For Endoscopic Submucosal Dissection of Early Gastric Cancer.

This protocol describes a self-made multiring thread method to provide controlled internal traction during endoscopic submucosal dissection (ESD) for early gastric cancer. ESD is a minimally invasive treatment that enables en bloc resection; however, the procedure can be technically challenging when adequate visualization and access to the submucosal layer are limited. Conventional traction methods, such as the dental floss clip technique, may be hindered by interference with the endoscope and instability of clip attachment. To address these limitations, a multiring thread device was developed…

Bilateral Papillary Traction due to Persistent Hyaloid Vessel Remnants in Aymé-Gripp Syndrome.

INTRODUCTION: Although papillary traction often arises from anomalous vitreous detachment, it can be related to traction from persistent remnants of the fetal hyaloid vasculature. CASE PRESENTATION: A 17-year-old girl with Aymé-Gripp syndrome was incidentally found to have bilateral optic nerve elevation in the setting of bilateral persistent hyaloid vessel remnants causing papillary traction. Brain imaging was unremarkable. CONCLUSION: This case highlights the rare occurrence of bilateral papillary traction in a young patient with a genetic syndrome and underscores the importance of conside…

Preoperative halo gravity traction (HGT) in severe rigid scoliosis - A single centre study.

BACKGROUND: Managing severe and rigid spinal deformities with limited flexibility and high Cobb angles (≥90°) remains a significant challenge in spine surgery. Halo Gravity Traction (HGT) has been adopted preoperatively to optimize surgical tolerance, facilitate spinal correction, and improve surgical outcomes. OBJECTIVE: This study aimed to determine the outcomes of severe scoliosis managed by preoperative HGT followed by definitive spinal fusion. METHODS: A retrospective analysis of 27 patients was conducted at a single center between 2010 and 2023. The inclusion criteria were patients wit…

Endoscopic submucosal dissection of duodenal Brunner's gland mass using transpyloric traction wire fixation.

BACKGROUND AND AIMS: Brunner's gland proliferative lesions, including adenomas and hamartomas, are rare benign duodenal tumors that can cause upper gastrointestinal symptoms when large. Endoscopic submucosal dissection (ESD) in the duodenum is technically challenging because of limited maneuverability and thin wall anatomy. Careful planning of the incision line is crucial for safe resection and effective closure. This case illustrates a traction wire-assisted transpyloric fixation technique designed to improve visualization and stability during ESD. METHODS: A patient with a symptomatic, pedu…

Ciliary muscle traction during accommodation is able to induce optic nerve head deformation.

OBJECTIVES: To use finite element (FE) modeling and in vivo optical coherence tomography (OCT) imaging to explore the effect of ciliary muscle traction on optic nerve head (ONH) deformation during accommodation. METHODS: We developed a FE model to mimic the ciliary muscle traction during accommodation, and varied the stiffness of the sclera, choroid, Bruch's membrane (BM), prelaminar neural tissue and lamina cribrosa (LC) to assess their effects on accommodation-induced ONH strains. To validate the FE model, OCT images of the right eyes' ONHs from 20 subjects (25 ± 1.5 years, range: 24-28 yea…
