Potential new drugs for human African trypanosomiasis: some progress at last.

PURPOSE OF REVIEW: This review covers recent developments towards novel treatments for human African trypanosomiasis (HAT). RECENT FINDINGS: Within the past decade, some important advances in the treatment of HAT have been made. One old drug, melarsoprol, previously administered over a period of a month or more, is now given in a 10-day regimen greatly reducing hospital costs. A combination chemotherapy, eflornithine alongside nifurtimox, has been introduced to decrease the time frame and overall dosing of eflornithine and reducing the risk of drug resistance emerging. One new, orally availab…

Discovery of halo-nitrobenzamides with potential application against human African trypanosomiasis.

A series of halo-nitrobenzamide were synthesized and evaluated for their ability to block proliferation of Trypanosoma brucei brucei. A number of these compounds had significant activity against the parasite, particularly 2-chloro-N-(4-chlorophenyl)-5-nitrobenzamide 17 which exhibited low micromolar inhibitory potency against T. brucei and selectivity towards both malaria and mammalian cells.

Clinical aspects of 2541 patients with second stage human African trypanosomiasis.

The clinical symptoms and signs of patients with second stage HAT are described for a large cohort of patients treated in a prospective multicentre, multinational study. Special emphasis is given to the influence of disease stage (duration, number of WBC in CSF) and patient age to the clinical picture. Even though the frequencies of symptoms and signs are highly variable between centres, the clinical picture of the disease is similar for all countries. Headache (78.7%), sleeping disorder (74.4%) and lymphadenopathy (56.1%) are the most frequent symptoms and signs and they are similar for all s

Fine mapping of trypanosomiasis resistance loci in murine advanced intercross lines.

We have previously reported the results of genome-wide searches in two murine F(2) populations for QTLs that influence survival following Tr … sly in the equivalent F(2) generations. Mapping in the AILs also resolved the Tir3 locus into three trypanosomiasis resistance QTLs, revealing a degree of complexity not evident in extensive studies at the F(2) level.

Short-course eflornithine in Gambian trypanosomiasis: a multicentre randomized controlled trial.

OBJECTIVE: A randomized controlled trial was conducted to determine whether 7 days of intravenous eflornithine (100 mg/kg every 6 h) was as effective as the standard 14-day regimen in the treatment of late-stage Trypanosoma brucei gambiense trypanosomiasis. METHODS: A total of 321 patients (274 new cases, 47 relapsing cases) were randomized at four participating centres in Congo, Côte d'Ivoire, the Democratic Republic of the Congo, and Uganda to one of these treatment regimens and followed up for 2 years. RESULTS: Six patients died during treatment, one of whom was on the 7-day regimen, wher…

Peripheral nervous system involvement in human and experimental chronic American trypanosomiasis.

An electrophysiological and histological study of the muscle and the peripheral nervous system (PNS) was carried out in chronic human American trypanosomiasis (Chagas' disease) and in an experimental Chagas' disease (Chd) mouse model. Altogether 995 patients with chronic Chd and 261 mice, experimentally infected with RA and CA-I parasite strains, were investigated. Results were compared with matched controls. Techniques employed in humans were: clinical assessment, conventional electromyography (EMG), estimated number of motor units, motor and sensory nerve conduction velocities, repetitive ne…

Subcurative chemotherapy and fatal post-treatment reactive encephalopathies in African trypanosomiasis.

The treatment of late-stage African sleeping sickness in man is often complicated by a post-treatment reactive encephalopathy. The bases of this pathological reaction was investigated in a mouse model of African trypanosomiasis. Subcurative treatment with diminazene aceturate, which did not clear parasites from the central nervous system, resulted in a post-treatment meningoencephalitis similar to that seen in man. By contrast, a curative regimen of melaminylthioarsenite and 5-nitroimidazole, which cleared parasites from the central nervous system, did not cause any pathological reaction in th…

Trypanosome-induced suppression of anti-parasite responses during experimental African trypanosomiasis.

The effect of Trypanosoma brucei infections on parasite-specific antibody responses was examined by immunization of infected mice with radio-attenuated trypanosomes of a noncross-reacting clone. Antibody titers (total Ig) against variant surface antigen were determined by indirect immunofluorescence. The suppression resulting from acute infection was virtually complete, and is associated with the failure to control the first relapse variant of the acute clone. The suppression resulting from chronic infection was much less severe, and subsequently variant populations of the chronic clone are su

[Human trypanosomiasis foci in OCCGE member countries: present situation (author's transl)].

Using official epidemiological informations, authors study evolution of sleeping sickness epidemiological situation in seven OCCGE countries: Benin, Ivory Coast, Upper Volta, Mali, Niger, Senegal and Togo. 745 new cases were diagnosed in 1977 and more than 600 in 1978. We observe the extinction of historic foci in Niger and Senegal, the persistance of foci in Upper Volta and Mali and the reviviscence of foci in Benin, Ivory Coast and Togo. One of the prominent points of the present endemia is the importance of people migrations, especially between Ivory Coast and Upper Volta. It would be of ma

One health research to mitigate health burden of American trypanosomiasis in Mexico.

American trypanosomiasis (AT), or Chagas disease, is an ancient disease with an interesting history. Despite efforts to control AT, the global burden of this neglected vector-borne disease persists. More than six million people around the world are infected with Trypanosoma cruzi (T. cruzi), and about 75 million are at risk of infection in the Americas. Mexico is one of the countries most affected by AT with 4 million people at risk of infection with T. cruzi. Approximately 60 % of the accumulated confirmed cases occurred in the southeast region; however, an increase in the number of cases was…

The Transmission of Animal African Trypanosomiasis in Two Districts in the Forest Zone of Ghana.

Animal African trypanosomiasis, also known as nagana, is caused by Trypanosoma species, which cause significant clinical diseases and lead to losses in animal production. We carried out a cross-sectional survey to investigate the composition of vectors and parasite diversity in two districts in the eastern region of Ghana where pigs and cattle were exposed to tsetse bites. We performed cytochrome c oxidase subunit 1 polymerase chain reaction (PCR) to identify tsetse species and internal transcribed spacer 1 PCR to identify Trypanosoma species. Also, we investigated the source of tsetse blood m…

Domestic dogs as reservoirs for African trypanosomiasis in Mambwe district, eastern Zambia.

The control of African trypanosomiasis (AT) in Eastern and Southern Africa, including Zambia, faces huge challenges due to the involvement of wild and domestic animal reservoirs. Free-roaming dogs in wildlife-populated and tsetse-infested villages of Zambia's Mambwe district are exposed to infectious tsetse bites. Consuming fresh raw game meat and bones further exacerbates their risk of contracting AT. We focus on the reservoir role of such dogs in maintaining and transmitting diverse species of trypanosomes that are infective to humans and livestock in Zambia's Mambwe district. A cohort of 16…

Biochemical and genotyping analyses of camels (Camelus dromedaries) trypanosomiasis in North Africa.

Camels are considered an important food source in North Africa. Trypanosomiasis in camels is a life-threatening disease that causes severe economic losses in milk and meat production. Therefore, the objective of this study was to determine the trypanosome genotypes in the North African region. Trypanosome infection rates were determined by microscopic examination of blood smears and polymerase chain reaction (PCR). In addition, total antioxidant capacity (TAC), lipid peroxides (MDA), reduced glutathione (GSH), superoxide dismutase (SOD) and catalase (CAT) were determined in erythrocyte lysate.…

Raman spectroscopic analysis of skin as a diagnostic tool for Human African Trypanosomiasis.

Human African Trypanosomiasis (HAT) has been responsible for several deadly epidemics throughout the 20th century, but a renewed commitment to disease control has significantly reduced new cases and motivated a target for the elimination of Trypanosoma brucei gambiense-HAT by 2030. However, the recent identification of latent human infections, and the detection of trypanosomes in extravascular tissues hidden from current diagnostic tools, such as the skin, has added new complexity to identifying infected individuals. New and improved diagnostic tests to detect Trypanosoma brucei infection by i…
