Tuberculin reactivity in healthy school children in Pondicherry.

Five hundred healthy school children in the age group 5-9 years underwent tuberculin testing. Seventy nine per cent of the study population had a BCG scar. 9.2% children were found to be suffering from malnutrition, using the Quetlet's Index and 7.8% were found to be malnourished by weight for age method. 18.6% of the children were tuberculin positive (induration > or = 10 mm) and the prevalence of tuberculin positivity increased with age. The numbers of tuberculin reactors or the mean tuberculin reaction were not affected by malnutrition. BCG scar status neither affected the mean tuberculin s…

Utility of an interferon-gamma release assay for latent tuberculosis diagnosis in a case of bullous pemphigoid.

BACKGROUND: With increasing use of immunosuppressive therapy, including tumor necrosis factor alpha inhibitors, there is concern about infectious complications, including reactivation of latent Mycobacterium tuberculosis infection. Routine testing prior to administration of systemic immunosuppression includes the tuberculin skin test, which lacks sensitivity and specificity and may be difficult to interpret in the presence of extensive cutaneous disease. Treatment of individuals with latent tuberculosis infection is recommended when immunosuppressive medications are to be employed. OBSERVATIO

Tuberculin reactivity in schoolchildren, Kassala State, Sudan.

BACKGROUND: Tuberculin skin test (TST) is widely used for the assessment of Bacillus Calmette-Guérin (BCG) vaccine efficacy and screening of latent TB infection (LTBI). Poor or no data are available on the reactivity of tuberculin in Kassala State. The aim of the present study was to assess the response to the BCG vaccine and to estimate the prevalence of LTBI and the annual rate annual risk of tuberculous infection (ARTI) among vaccinated school children using TST. METHODS: School-based cross-sectional study was conducted in three localities of Kassala State during 2016-2018. A cluster rando

High levels of anti-tuberculin (IgG) antibodies correlate with the blocking of T-cell proliferation in individuals with high exposure to Mycobacterium tuberculosis.

OBJECTIVES: To determine the effect of anti-tuberculin antibodies in the T-cell proliferation in response to tuberculin and Candida antigens in individuals with different levels of tuberculosis (TB) risk. METHODS: Sixteen high-risk TB individuals, 30 with an intermediate TB risk (group A), and 45 with a low TB risk (group B), as well as 49 control individuals, were studied. Tuberculin skin test (TST) results were analyzed and serum levels of antibodies (IgG and IgM) against purified protein derivative (PPD) were measured by ELISA. Tuberculin and Candida antigens were used to stimulate T-cell

Cost-effectiveness of alternative strategies for tuberculosis screening before kindergarten entry.

OBJECTIVE: We undertook a decision analysis to evaluate the economic and health effects and incremental cost-effectiveness of using targeted tuberculin skin testing, compared with universal screening or no screening, before kindergarten. METHODS: We constructed a decision tree to determine the costs and clinical outcomes of using targeted testing compared with universal screening or no screening. Baseline estimates for input parameters were taken from the medical literature and from California health jurisdiction data. Sensitivity analyses were performed to determine plausible ranges of assoc

Risk of tuberculosis infection among community health agents.

OBJECTIVE: To estimate the risk of tuberculosis infection among community health agents involved in disease control. METHODS: A prospective cohort was followed up from April 2007 to May 2008 in the municipality of Cachoeiro de Itapemirim, Southeastern Brazil. The cohort was composed of 61 community agents, divided between unexposed individuals (n = 37) and exposed individuals (who were following up tuberculosis patients; n = 24). Over the 12-month follow-up, the tuberculin test was performed, using the tuberculin PPD RT23. The relative risk and 95% confidence interval were calculated, and the…

Tuberculin conversion in HIV seropositives.

The human immunodeficiency virus (HIV) epidemic is associated with increased incidence of tuberculosis. There is a need to provide prophylaxis for the same. Mycobacterium w, a non-pathogenic, atypical mycobacterium, shares antigenic determinants with Mycobacterium leprae and Mycobacterium tuberculosis. The tuberculin response, which is a delayed hypersensitivity reaction, indicates the status of cellular immunity against tuberculosis. The objective of this study was to evaluate potential of mycobacterium w for tuberculin conversion in HIV seropositives. Fifty HIV positives (tuberculin negative

Tuberculin conversions in Indochinese refugees. An assessment of boosting and anergy.

Indochinese refugees entering the United States have a high rate of tuberculosis and tuberculin reactivity. In addition, several investigators have noted that a large number of refugees with initial tuberculin tests that are "not significant" change to "significant" reactions when retested within 8 wk. This "conversion" phenomenon has been reported in 21 to 43% of refugees and has been unexplained by antigen, testing, demographic, or exposure risk factors. A prospective evaluation of 218 refugees, conducted to assess the role of anergy and boosting, confirmed earlier findings, with 52% of 118 …

The multi-facets of tuberculin standardization.

Some parameters affecting the standardization of a tuberculin preparation have been evaluated and it has been shown that: 1) The loss of tuberculoprotein on the surfaces of containers is in the order of 0.15 mug/cm2 and the 0.18 mug/cm-2 for glass and plastic surfaces respectively, and that this loss can be prevented by the addition of a nonionic anti-adsorption agent such as Tween 80. 2) For non-stabilized tuberculin PPD solutions the losses of PPD due to adsorption increased linearly with the Surface to Volume ration. 3) Exposure to light had a deleterious effect on tuberculin PPD decreasing

Infectiousness of Mycobacterium tuberculosis in HIV-1-infected patients with tuberculosis: a prospective study.

BACKGROUND: Previous studies concerning the relative infectiousness of HIV-1-positive individuals with pulmonary tuberculosis have produced conflicting results. Thus, we assessed the effect of HIV-1 on the infectiousness of Mycobacterium tuberculosis in a prospective study. METHODS: We organised in Santo Domingo, Dominican Republic, a cohort study of household contacts of HIV-1-positive and HIV-1-negative individuals with newly diagnosed pulmonary tuberculosis. Household contacts were assessed at their houses at baseline and followed up for 14 months for evidence of M tuberculosis infection a

Latent tuberculosis infection in rural China: baseline results of a population-based, multicentre, prospective cohort study.

BACKGROUND: Prophylactic treatment of individuals with latent Mycobacterium tuberculosis infection is an essential component of tuberculosis control in some settings. In China, the prevalence of latent tuberculosis infection, and preventive interventions against this disease, have not been systematically studied. We aimed to assess the prevalence of latent tuberculosis and its associated risk factors in rural populations in China. METHODS: Between July 1, and Sept 30, 2013, we undertook a baseline survey of a population-based, multicentre, prospective cohort study of registered residents (≥5

Rate of latent tuberculosis infection detected by occupational health screening of nurses new to a london teaching hospital.

The prevalence of latent tuberculosis infection in a cohort of nurses new to a London hospital was 7.6% (13 of 171), using an interferon-gamma (IFN-gamma) release assay, and 16.2% (24 of 148), using the tuberculin skin test. On multivariate analysis, birth in a country with tuberculosis prevalence of more than 40 cases per 100,000 population was associated with positive results of both the IFN-gamma release assay and the tuberculin skin test.
