Specific synovitis of a knee as the first manifestation of miliary tuberculosis.

Tuberculosis (TB) is declared global emergency. Miliary TB is a treatable, potentially lethal form of TB resulting from massive lympho-hematogenous dissemination of Mycobacterium tuberculosis. Impaired cell-mediated immunity underlies the disease's development. We present a case of specific synovitis in a 21-year-old Caucasian HIV-seronegative woman. She presented with high fever and swelling of the right knee. Chest radiograph revealed bilateral nodular opacities in upper pulmonary lobes and signs of pleural effusions. Sputum samples were negative for Acid Fast Bacilli (AFB) and Löwenstein-Je

Age : pleural fluid ADA ratio and other indicators for differentiating between tubercular and malignant pleural effusions.

Most of pleural effusions are caused by tuberculosis and malignant tumor. Difficult sampling and bacterial sparing nature of these diseases challenge doctors' diagnosis in China. This study aimed to develop a new convenient and effective method for the differentiation of tuberculous and malignant pleural effusion. A prospective cohort study of patients hospitalized with malignant (n = 90) and tuberculous (n = 130) pleural effusions from September 2018 to October 2020 was performed. The diagnostic performance of the age to pleural fluid ADA ratio (age/ADA) and other indicators to distinguish tu

Current status of medical pleuroscopy.

Medical pleuroscopy (MP) offers a safe and minimally invasive tool for interventional pulmonologists. It allows diagnosis of unexplained effusion, while at the same time allowing drainage and pleurodesis. It can also help in the diagnosis of diffuse interstitial disease or associated peripheral lung abnormality in the presence of effusion. It can have a therapeutic role in pneumothorax and hyperhidrosis or chronic pancreatic pain. This article reviews the technical aspects and range of applications of MP.

Adenosine deaminase estimations in the differentiation of pleural effusions.

Adenosine deaminase (ADA) estimations were performed on the pleural fluid from 368 effusions. The mean (+/-SD) ADA concentration in tuberculous effusions was 92,11 +/- 37,05 U/l, and these values were found to be highly statistically different from the 23,23 +/- 13,15 U/l in secondary malignant tumours of the pleura, the 34,86 +/- 14,2 U/l in mesotheliomas, and the 23,81 +/- 15,07 U/l in pulmonary embolism. The ADA values of 64,3 +/- 44,95 U/l in lymphoproliferative disorders were less significantly different. No statistical difference could be found between values in the tuberculous group and

Steroids for treating tuberculous pleurisy.

BACKGROUND: TB of the pleura is associated with inflammation and fibrosis. Steroids could reduce the effects of the inflammation, but the immunosuppression could make patients vunerable. OBJECTIVES: This review aims to summarise the evidence about the effects of corticosteroids in patients with TB of the pleura, and explores if HIV status is associated with differences in effect estimates. SEARCH STRATEGY: We searched the Cochrane Infectious Diseases Group trials register, the Cochrane Library, MEDLINE, and EMBASE. Lists of references from review articles and primary studies were scanned and

Exogenous intrapleural injection of interleukin-27 may improves outcome and prognosis in patients with tuberculous pleural effusion.

We hypothesize that exogenous intrapleural injection of interleukin-27 may improve outcome and prognosis in patients with tuberculous pleural effusion (TPE). Studies have found that the balance of Th1/Th2 determines the development trend of TPE. High concentrations of IFN-γ and TNF-α in pleural effusion are associated with pleural adhesion in patients with TPE. Interleukin-27 is a member of the IL-12 family, and IL-27 has a dual regulatory effect on Th1 immunity. On one hand, IL-27 can promote the initial CD4+ T cell proliferation by inducing the expression of T-bet, IL-12Rβ2 and ICAM-1 in the

Pulmonary tuberculosis and tuberculous pleuropericarditis in a patient with pyoderma gangrenosum treated with adalimumab: a case report.

BACKGROUND: Adalimumab is a tumor necrosis factor-α inhibitor, which is associated with an increased risk of tuberculosis. However, evidence regarding tuberculosis development in patients with pyoderma gangrenosum is limited, and simultaneous involvement in lungs, pleura, and pericardium is rare. This report describes a case of pulmonary tuberculosis and tuberculous pleuropericarditis associated with adalimumab administration. CASE PRESENTATION: The patient was a 59-year-old Filipino female. She began receiving adalimumab to treat pyoderma gangrenosum at the dermatology department of our hosp

[Congenital tuberculosis: difficulties in early diagnosis].

BACKGROUND: Neonatal and/or congenital tuberculosis is insufficiently understood. CASE REPORTS: Case 1. A premature hypotrophic neonate presented at the age of 45 days, without any maternal contact, a bilateral bronchopneumopathy. Whilst the pregnancy and birth had not been affected by any noteworthy problem, the mother died from miliary tuberculosis despite rifampin, isoniazid and pyrazinamide treatment. Her baby also died on day 52 from multivisceral failure. Culture of tracheal secretions confirmed a few weeks later the diagnosis of tuberculosis. Case 2. A premature, hypotrophic neonate pr

Disseminated tuberculosis in a renal transplant recipient: presentation as an anterior mediastinal mass.

A 26-year-old man who had had a successful cadaveric renal transplant six months previously was admitted for evaluation of fever and dysuria. Chest x-ray revealed a new left pleural effusion, bilateral nodular parenchymal densities, and an anterior mediastinal mass. A biopsy of the mediastinal mass to exclude concurrent malignancy and positive AFB stains from pleural fluid and urine confirmed a diagnosis of disseminated tuberculosis.

Beyond Adenosine Deaminase (ADA) and Mycobacterial Growth Indicator Tube (MGIT): The Utility of Xpert MTB/RIF Ultra for Diagnosis and Resistance Determination in Suspected Tubercular Pleural Effusion in a High-Burden State.

BACKGROUND: Diagnosis of tubercular pleural effusion (TPE) is difficult and always controversial unless proved microbiologically. Pleural fluid is almost always paucibacillary, leading to low microbiological yield. Physicians often rely on pleural fluid biochemistry and clinical judgment for initiating antitubercular therapy (ATT). Xpert Mycobacterium tuberculosis/rifampicin resistance Ultra (Xpert MTB/RIF Ultra) (Cepheid, Sunnyvale, CA) has been designed to ease the diagnostic dilemma and improve sensitivity in paucibacillary tuberculosis (TB). OBJECTIVES: This study aimed to evaluate the di

Eosinophilic pleural effusions.

We present 2 cases of eosinophilic pleural effusion (EPE) seen recently at Straub Hospital. One occurred in a patient with pneumococcal empyema; the 2nd patient had 2 episodes of EPE secondary to malignant histiocytic lymphoma. Eosinophilic pleural effusion (EPE) is defined as having eosinophils exceeding 10% of the pleural fluid WBC differential. EPE is usually exudative, typically accounting for 1% to 8% of all pleural effusions. Up to 30% to 35% of EPE are idiopathic, while other frequent causes include air in the pleural space (30%), and pulmonary infections (10%). Collagen vascular diseas
