One hundred and forty-nine cases of childhood pulmonary tuberculosis were analysed being a 77.6% of the 192 cases of tuberculosis diagnosed at the Pediatric Infectious Diseases Department of "La Paz" Hospital, in a ten year period. The children were aged 3 months to 14 years; 65% of them were less than 6 years of age. The source case was found in 67.1% of the children. Sixty-two cases of bacteriologically positive pulmonary tuberculosis were studied in the Department of Tuberculosis and Chest Diseases at the V.S.S. Medical College Hospital, Burla, India. Apart from other investigations, all patients underwent laparoscopy to determine the incidence of genital involvement. Positive findings were observed in 37 women; tubercles were present in the tubes of 15 women. At dilatation and curettage, the endometrium was obtained in 37 women. Tuberculous endometritis was detected in 4 women.

A middle aged man presented as a recalcitrant case of borderline leprosy with concomitant pulmonary tuberculosis and urolithiasis which continued to progress relentlessly despite adequate multiduring antileprosy and antitubercular treatment. After detailed and relevant workup, the diagnosis of sarcoidosis was made. Rapid clinical improvement occurred with steroid therapy.

During the period from 1968 to 1987, 50 children, adolescent and adult patients with acute disseminated tuberculosis of the lung were examined in a Republican Tuberculosis Hospital. A preliminary diagnosis of tuberculosis can be established by the detection of acid-fast bacilli (AFB) and confirmed by culture of the microorganism. To evaluate an alternative method of diagnosis, the distribution of mycobacterial antigens in lung tissue specimens was characterized by an indirect peroxidase-antiperoxidase method and was compared to the detection of AFB by Ziehl-Neelsen stain. Guidelines available for the diagnosis of pulmonary tuberculosis (TB) in children vary widely. In an area of high prevalence, pulmonary TB was often suspected but difficult to confirm. In a prospective study, diagnostic methods were recorded in 144 consecutive children admitted and diagnosed as having pulmonary TB. The numbers of cells bearing the T3 (pan-T cell), the T4 (putative helper/inducer cells), the T8 (putative suppressor/cytotoxic cells) and B cell phenotypic markers were counted in venous blood samples from 26 newly diagnosed pulmonary tuberculosis patients and 29 healthy controls from East Java. Comprehensive investigations including clinical examination, x-ray and scintigraphy of the lungs were conducted in 163 patients with sarcoidosis, Stages I, II, III, disseminated tuberculosis and tuberculosis of the intrathoracic lymph nodes. The scintigraphic method is more sensitive to initial pulmonary changes in the process of treatment than the x-ray one. Earlier changes in the course of disease are recorded by pulmonary circulation changes (microcirculation) revealed at pulmonoscintigraphy.

Eleven cases of pulmonary aspergilloma complicating active cavitating pulmonary tuberculosis are reviewed. Nine of the 10 patients who had combined medical (antituberculosis drugs) and surgical treatment were cured of their disease; one patient, who had bilateral multiple aspergillomas, died from massive haemoptysis after resection of one of the affected lobes. Possible toxic side-effects of antituberculous chemotherapy are studied in 718 children affected with pulmonary tuberculosis. 26 (3.62%) presented adverse side-effects and one drug had to be changed in 8 (1.11%). Treatment had to be stopped in one (0.13%) due to toxicity. Liver toxicity was specially studied, showing that younger age is a risk factor (p less than 1 X 10(-10). In 44 cases (16.54%) transient increases of no more than triple of maximum normal value, were found in SGOT and/or SGPT. Toxicity observed in controlled clinical studies and guides for treatment are exposed.

Two observations by the authors of a chronic cavernous histoplasmosis in a patient of 13 and a histoplasmoma of the lungs in a patient of 53 are described. The diagnosis was established by the examination of the operation material. Clinically, these patients were diagnosed as pulmonary tuberculosis cases. The attention of pulmonologists is drawn to the features and difficulties of differential diagnosis of histoplasmosis and tuberculosis.
