Reduced MHC class II antigen expression on rat heart and kidney cells after in vitro exposure to longwave ultraviolet irradiation.

The pretreatment of both kidney and heart donor with the photosensitizer 8-methoxy-psoralen followed by in vitro longwave ultraviolet graft irradiation (PUVA therapy) significantly prolonged graft survival times in rats even across a strong major histocompatibility complex (MHC) barrier. Immunohistological studies using monoclonal antibodies (Mab) to rat MHC class I and II antigens showed a significant reduction of class II antigen expression after PUVA therapy in both Sprague-Dawley rat kidneys and hearts. Using MHC class I Mab there were no differences in distribution pattern in untreated as

Use of Phototherapy in Children.

BACKGROUND: Phototherapy is a well-recognized treatment in adults and children. Previous articles have reported success in treating recalcitrant skin disorders such as atopic dermatitis (AD), psoriasis, pityriasis lichenoides chronica, and vitiligo in children. METHODS: This was a retrospective review over an 18-month period from June 2012 to December 2013 of all children receiving phototherapy in a tertiary pediatric dermatology center. RESULTS: Seventy-five patients 3 to 17 years of age (mean 10.6 years; 35 male, 40 female) were included. Forty-eight (64%) patients had AD and 21 (28%) had

Exposure to Ultraviolet Radiation in the Modulation of Human Diseases.

This review focuses primarily on the beneficial effects for human health of exposure to ultraviolet radiation (UVR). UVR stimulates anti-inflammatory and immunosuppressive pathways in skin that modulate psoriasis, atopic dermatitis, and vitiligo; suppresses cutaneous lesions of graft-versus-host disease; and regulates some infection and vaccination outcomes. While polymorphic light eruption and the cutaneous photosensitivity of systemic lupus erythematosus are triggered by UVR, polymorphic light eruption also frequently benefits from UVR-induced immunomodulation. For systemic diseases such as

The bactericidal effect of far-UVC on ESBL-producing Escherichia coli.

Because extended-spectrum beta-lactamase (ESBL) infections can cause life-threatening disease and effective treatments need to be developed, we examined the bactericidal effect of far-ultraviolet C (far-UVC) light therapy on ESBL-producing Escherichia coli (E. coli). The bactericidal effect on 2 types of ESBL-producing E. coli was the same as that on the wild strain although the results of drug resistance tests varied among these strains. We believe that irradiation with far-UVC is effective in preventing infection by ESBL-producing E. coli in health care settings.

Phototherapy in the Evaluation and Management of Photodermatoses.

Ultraviolet light (UV) and visible light are important components in the diagnosis of photodermatoses, and UV has the unique ability to also be used to manage photodermatoses. Phototesting, provocative light testing, and photopatch testing can provide important information in diagnosing patients with photodermatoses; phototesting can be used to determine the starting dose for phototherapy in these patients. Once photosensitivity is established, narrowband UVB and UVA1 therapy have helped to improve the quality of life of photosensitive patients, such as those with polymorphous light eruption,

[The characteristics of the course and treatment of severe forms of leptospirosis].

At the resuscitation clinic 27 patients suffering from hepatorenal form of leptospirosis were observed. Some features of the course of hepatorenal insufficiency were noted in this disease: relatively short period of oligoanuria, absence of hyperkalemia, dependence of the degree of hemorrhagic syndrome on the level of thrombocytes and absence of correlation with prothrombin consumption index, an apparent brain edema in autopsy tissue. Timely delivery of intensive care including hemodialysis, hemosorption, oxybarotherapy, ultraviolet irradiation of blood, corticosteroid therapy enabled the recov

Response of psoriatic nails to oral photochemotherapy.

Ten patients with generalized psoriasis, which included nail involvement, were treated with total-body oral photochemotherapy. A combination of methoxsalen and high-intensity ultraviolet A radiation was employed and its therapeutic efficacy in nail psoriasis ascertained. Among the ten patients, there were 26 instances of the following nail signs: pitting, onycholysis, "oil drop" change, subungual hyperkeratosis, nail-plate crumbling, proximal nail-fold psoriasis, and onychorrhexis. Of the 26, eighteen improved by 50% or more. Only pitting failed to respond to therapy in all of the eight patien

Pemphigus Foliaceus Associated with Psoriasis during the Course of Narrow-Band UVB Therapy: A Simple Coincidence?

Although psoriasis and bullous diseases are considered to be completely different disease entities, the literature has reported a few cases of psoriasis associated with bullous diseases, most of which are bullous pemphigoid. In limited cases, pemphigus foliaceus has also been reported in association with psoriasis. In most of them, pemphigus lesions usually developed on an untreated patient with a chronic history of psoriasis. Herein, we report a case of 53-year-old male with a chronic history of psoriasis who first developed generalized erosive lesions after 26 cycles of narrow-band ultraviol

Cutaneous T cell lymphoma: update of treatment.

The state-of-the art therapy of cutaneous T cell lymphoma (CTCL) is reviewed. Commonly used treatments for early-stage (patch/plaque) mycosis fungoides (MF) include topical corticosteroids, mechlorethamine, carmustine, ultraviolet light B and PUVA. Total skin electron beam (TSEB) therapy is indicated for widespread infiltrated plaque and tumor stage disease. Low-dose methotrexate is often useful for resistant patch/plaque MF and erythrodermic CTCL. Interferon alpha (IFN-alpha) is indicated for methotrexate failures and recurrent tumors following TSEB therapy. Photopheresis may be helpful for e

Immunologic evaluation in mycosis fungoides. Before and after PUVA therapy.

Eight patients with mycosis fungoides (seven in plaque stage and one with tumors) were successfully treated with methoxalen-ultraviolet-A (PUVA) photochemotherapy. A selective immunity evaluation had been carried out prior to starting treatment and was repeated sevedn to 33 months (mean, 22 months) later. The results of pre-PUVA and post-PUVA therapy evaluation were compared. The delayed hypersensitivity responses improved in all eight patients, and four patients who were anergic before treatment manifested a normal response after PUVA therapy. The T lymphocyte counts were reduced in four pati

Functional nutrition as integrated approach in vitiligo management.

Vitiligo is a common disease of unknown cause that produces disfiguring white patches of depigmentation that can be treated using various new and experimental therapies, such as narrow-band ultraviolet B (NB-UVB) microphototherapy, NB-UVB excimer laser, and monochromatic excimer light. Medical treatments include topical corticosteroids and other topical treatments, such as antioxidants, tacrolimus and pimecrolimus, prostaglandin E, and vitamin D derivatives (Lotti, Berti, & Moretti, 2009). The goal of treating vitiligo is to make it less noticeable either by restoring lost pigment or by elimin

Study of THP-1 Macrophage Viability after Sonodynamic Therapy Using Methyl Ester of 5-Aminolevulinic Acid Gold Nanoparticles.

Sonodynamic therapy (SDT) is emerging as new atherosclerosis treatment. The use of gold nanoparticles (AuNPs) as the vehicle for a sensitizer delivery improves reactive oxygen species formation. In this study, methyl ester of aminolevulinic acid (MALA) gold nanoparticles (MALA:AuNPs) functionalized with polyethylene glycol (PEG) were synthesized by photoreduction and characterized by ultraviolet/visible optical absorption, zeta potential and electron microscopy. The reactive oxygen species generation induced by ultrasound irradiation of MALA:AuNPs solutions was studied by observing the decreas

PUVA treatment of erythrodermic and plaque-type mycosis fungoides. Ten-year follow-up study.

Since our preliminary report of psoralen plus long-wave ultraviolet A (PUVA) therapy in ten patients with erythroderma-type or plaque-type mycosis fungoides (MF), we have treated 38 patients with biopsy-proved MF. Approximately one third, mostly patients with erythroderma, received PUVA as primary therapy; the remainder had recurrent disease following electron beam irradiation or topical mechlorethamine (Mustargen) hydrochloride. Follow-up data are presented in 29 patients who completed an initial course of PUVA given two to three times weekly. A complete clinical response was observed in ten

Methoxsalen photochemotherapy for mycosis fungoides.

Methoxsalen photochemotherapy (PUVA; psoralen plus ultraviolet light) is effective in the treatment of mycosis fungoides (MF). The mechanism of this beneficial effect is unknown but probably involves covalent photo-binding of methoxsalen molecules to pyrimidine bases in DNA at the cellular level and impaired T-cell function or survival at the tissue level. Eleven patients (seven with plaques and four with erythroderma) were referred for PUVA therapy because of poor response to conventional therapy. Seven patients had complete clearing of skin lesions and three improved markedly. All ten of the

Phototherapy for Vitiligo.

Vitiligo is a common disorder with a severe impact on quality of life. The authors review recent advances in phototherapy for vitiligo focusing on narrowband ultraviolet B including mechanisms, treatment recommendations, and combination therapies. Phototherapy is the first-line treatment of choice for vitiligo with narrowband UVB preferred for widespread vitiligo and excimer used for localized lesions. However, unfamiliarity with prescribing phototherapy may be limiting clinicians from using it to its full potential. This article provides clinicians with the critical information needed to safe

Recall Dermatitis to Metronidazole.

BACKGROUND AND OBJECTIVE: Recall dermatitis is an uncommon entity that has been noted with patch testing, repeat exposures to a medication or allergen, and the concurrent use of ultraviolet radiation and certain medications. Recall dermatitis from primary cutaneous exposure and subsequent oral exposure are rare, and the etiology remains unknown. CONCLUSION: We report a case of recall dermatitis with a drug eruption secondary to metronidazole. Therapy with this medication was initially topical for rosacea and subsequently oral for a gastrointestinal infection. This case draws attention to the

The influence of psoriatic plaques pretreatment with crude coal tar vs. petrolatum on the efficacy of narrow-band ultraviolet B: a half-vs.-half intra-individual double-blinded comparative study.

BACKGROUND/PURPOSE: The value of psoriatic plaques pretreatment with specific topical preparations in the setting of NB-ultraviolet-B (UVB) therapy is debatable. It may be clarified through a comparative assessment between crude coal tar and petrolatum as pretreatment candidates. METHODS: A prospective study included 40 patients with plaque psoriasis undergoing NB-UVB therapy. We compared among three treatment regimens: NB-UVB alone (control group; n=20) as well as NB-UVB preceded by crude coal tar 3% on one side and petrolatum on the other side (cases group I and II; n=20). Tar and petrolatu

Effect of continued ultraviolet B phototherapy on the duration of remission of psoriasis: a randomized study.

Phototherapy using sunburn spectrum ultraviolet radiation (UVB) is now a frequently utilized treatment for psoriasis that is extensive or has not responded to topical preparations. Four university centers performed a prospective randomized clinical trial to compare remission times of patients with psoriasis who continued UVB phototherapy after initial clearing with this therapy and patients whose UVB phototherapy was discontinued within 3 weeks of clearing. As assessed by life table methods, the time to flare after initial clearing for patients on UVB maintenance therapy was significantly long
