Iatrogenic Bladder Rupture After Insertion of a Urinary Catheter in a Patient With Bladder Cancer: A Case Report.

BACKGROUND: Iatrogenic bladder rupture is a rare but potentially life-threatening complication. Bladder rupture resulting from the insertion of a urinary catheter is exceedingly uncommon. Here, we report a case of bladder rupture after urinary catheter insertion in a patient with bladder cancer. CASE PRESENTATION: An 80-year-old woman presented to the emergency department with fever, flank pain, and difficulty urinating. Her medical history included bladder cancer and neurogenic bladder. A urinary catheter had been placed owing to recent worsening of bladder function and had been maintained f

The relationship between urodynamic bladder compliance and urinary fibrosis markers in children with neurogenic bladder dysfunction.

BACKGROUND: Decreased bladder compliance is an important risk factor for upper urinary tract in children with neurogenic bladder dysfunction (NBD). Urodynamics is the gold standard in determining bladder compliance. OBJECTIVE: To investigate the relationship between low bladder compliance and urinary fibrosis markers in NBD. STUDY DESIGN: Spina bifida patients with NBD, who admitted between March 2021 and July 2021 were included. Patients with low compliant bladders, no renal scar, no recurrent urinary tract infections formed low compliance, LC group. Normal compliance, NC group, was compris

[Neoadjuvant therapy and complete response of muscle-invasive bladder cancer: may the urinary bladder be preserved?].

Radical cystectomy (RC) with pelvic lymphadenectomy followed by urinary diversion is the standard treatment for muscle-invasive bladder cancer (MIBC). Perioperative systemic therapy can improve the oncological outcome of RC. Despite the use of modern surgical techniques, RC is still associated with a high rate of perioperative complications and reduced quality of life. As an alternative to RC, organ preserving trimodal therapy can be performed in selected patients. In light of newer and more effective systemic therapies and the associated higher response rates to neoadjuvant systemic therapy,

[Leiomyosarcoma of the urinary bladder treated using laparoscopic cystectomy and bladder replacement by enterocystoplasty: about a case].

Our study aims to report a new case of leiomyosarcoma of the urinary bladder, a rare tumor, and to propose a new therapeutic approach, given the non-consensual nature of its treatment. A 31-year old patient, with no particular previous history, presented with total haematuria with clot formation. The clinical and paraclinical assessment showed a voluminous solid tumor mass occupying the apex of urinary bladder and extending to the right lateral wall, with infiltrating appearance, without lymph node involvement or invasion of local or distal organs. Surgical management was based on total laparo…

Serotonergic and noradrenergic facilitation of the visceromotor reflex evoked by urinary bladder distension in rats with inflamed bladders.

Bladder inflammation resulting from intravesical administration of zymosan significantly enhances the visceromotor reflex (VMR) evoked by urinary bladder distension (UBD). The present study examined whether intrathecal (i.t.) administration of receptor antagonists to either norepinephrine (NE) or serotonin (5-HT) altered this enhancement effect. I.t. administration of the non-specific 5-HT receptor antagonist methysergide (30 microg), the 5-HT(3) receptor antagonist ondansetron, or the 5-HT(1A) receptor antagonist WAY 100635 eliminated the enhancement effect produced by intravesical zymosan an…

Nocturnal bladder emptying: a simple technique for reversing urinary tract deterioration in children with neurogenic bladder.

PURPOSE: In this preliminary study we sought to determine the effect of instituting nocturnal bladder emptying (NBE) in children with neurogenic (NGB) or nonneurogenic neurogenic bladder (NNGNGB) in whom urinary tract deterioration developed despite optimal daytime clean intermittent catheterization (CIC) and urotropic medications. We hypothesize that a syndrome of nocturnal overdistention of the bladder (SNOB) can cause urinary tract deterioration through increased nighttime storage pressures manifested by recurrent urinary tract infection (UTI), worsening incontinence, hydronephrosis and/or

Serous adenocarcinoma of the uterus metastatic to the urinary bladder mimicking primary bladder neoplasia. A report of a case.

A 72-year-old woman with a history of transitional cell carcinoma of the urinary bladder was found to have a vesical neoplasm at cystoscopy. Microscopic examination of a biopsy specimen showed papillary adenocarcinoma. A subsequent endometrial biopsy, performed because of vaginal spotting, disclosed a serous papillary adenocarcinoma. A hysterectomy was performed. Pathologic examination showed that the tumor was deeply invasive of the myometrium. The uterine and bladder tumors were similar histologically. This case suggests that when examining a pure adenocarcinoma of the bladder, the pathologi…

Presumptive pseudohypoaldosteronism secondary to chronic urinary tract obstruction from sloughed urinary bladder mucosa and urinary tract infection in a cat.

OBJECTIVE: To describe a case of presumptive secondary pseudohypoaldosteronism (PHA) in a cat with urinary tract infection and chronic urethral obstruction. The obstruction was believed to have resulted from sloughed urinary bladder mucosa secondary to pressure necrosis. CASE SUMMARY: A 5-year-old, 4 kg, castrated male Siamese cat presented for vomiting and stranguria. Medical history included a perineal urethrostomy for urethral obstruction. Physical examination revealed a large, painful, nonexpressible urinary bladder. Point-of-care testing demonstrated electrolyte derangements consistent w…

The urinary microbiome, overactive bladder and bladder pain syndrome/interstitial cystitis - mechanisms, diagnostics and therapeutic opportunities.

Overactive bladder and bladder pain syndrome/interstitial cystitis are prevalent, multifactorial disorders with poorly understood pathophysiology. The discovery of the urinary microbiome has overturned the sterile urine paradigm and created new opportunities for mechanistic and translational research. Increasing evidence suggests that bladder microbial communities are associated with urothelial signalling, immune tone and neural pathways that influence urgency, pain and treatment response. In overactive bladder, Lactobacillus-predominant profiles are associated with lower symptom burden and im

Acute urinary bladder tamponade with hypovolemic shock due to placenta percreta with bladder invasion. Case report.

A rare case with residual tissue of placenta previa invading posterior parts of the bladder as placenta percreta complicated by massive late hematuria with hypovolemic shock two months after gynecologic-obstetric operation is presented. The patient was finally treated by emergency bladder resection. If a grand multipara with a history of hysterotomies, such as cesarean sections, presents lower abdominal pain involving hematuria during pregnancy, placental invasion of the bladder may be suspected. The primary treatment by hysterectomy should be complemented by bladder resection. This placental

Human bladder organoids model urinary tract infection and bacteriophage therapy.

Urinary tract infections (UTIs), primarily caused by uropathogenic Escherichia coli (UPEC), are among the most common antibiotic-resistant infections. Despite this, currently available preclinical UTI models lack the breadth of morphotypic and heterogenous cell populations of the human bladder, impairing the development of novel therapies. To address these limitations, we developed human bladder organoids derived from the bladder stem cells of multiple healthy donors which recapitulate cellular diversity of the urothelium. Using bulk and single cell RNA-sequencing, we characterized organoid re

Purported Bladder Irritant Intake in Women With Urgency Urinary Incontinence.

IMPORTANCE: Patients with urgency urinary incontinence are often recommended to avoid bladder irritants, but there is a lack of evidence for this. OBJECTIVE: The aim of the study was to compare consumption of purported bladder irritants between women with and without urgency urinary incontinence. STUDY DESIGN: We performed a case-control study of nonpregnant females aged ≥20 years using the National Health and Nutrition Examination Survey, 2007-2020. Cases were defined as participants with moderate, severe, or very severe urgency urinary incontinence without stress urinary incontinence, meas

Metabolite-mediated mechanisms linking the urinary microbiome to bladder cancer.

Bladder cancer is the most common malignancy of the urinary tract and is a major health burden globally. Recent advances in microbiome research have revealed that the urinary tract harbors a resident microbial community, overturning the long-held belief in its sterility. Increasing evidence suggests that microbial dysbiosis and microbially derived metabolites contribute to bladder cancer carcinogenesis, progression, and therapeutic responses. Distinct microbial signatures have been observed in bladder cancer patients, with notable differences across disease stages and between primary and recur
