Clinical and Genomic Characterization of Bladder Carcinomas With Glandular Phenotype.

PURPOSE: To compare oncologic outcomes and genomic alteration profiles in patients with bladder and urachal adenocarcinoma, urothelial carcinoma (UC) with glandular differentiation, and UC, not otherwise specified (NOS) undergoing surgical resection, with emphasis on response to systemic therapy. METHODS: We identified patients with bladder cancer with glandular variants who underwent surgical resection at Memorial Sloan Kettering from 1995 to 2018 (surgical cohort) and/or patients who had tumor sequencing using a targeted next-generation sequencing platform (genomics cohort). Pathologic comp

Prostatic Ductal Adenocarcinoma with Papillary Features in a Bladder Washing: A Possible Pitfall in Urinary Cytology - Report of One Case.

In this article, we report on a case of combined, acinar and ductal prostatic adenocarcinoma affecting the prostatic urethra, which, due to a low degree of cytologic atypia and an exclusive papillary architecture with visible fibrovascular core, was erroneously diagnosed as a low-grade urothelial carcinoma based on its peculiar cytologic presentation in a bladder washing sample.

Inverted urothelial papilloma: a lesion with malignant potential.

Two cases of inverted urothelial papilloma are presented. In the first case the inverted papilloma was in the ureter and varying degrees of cellular atypia were demonstrated on histology: 7 years later, a single bladder lesion consisting of papillary transitional cell carcinoma and inverted papilloma developed in the same patient. In the second case a bladder tumor consisting of inverted papilloma mixed with papillary infiltrating transitional cell carcinoma was detected. The peculiar morphological findings, histogenesis and biological behavior of inverted urothelial lesions are discussed.

Undifferentiated sarcoma arising in an augmented ileocystoplasty patch.

We report an unusual case of a sarcoma at the patch site of an ileocystoplasty. The tumor proved to be an aggressive neoplasm that metastasized widely. The literature is reviewed concerning neoplasms occurring in urinary diversion procedures.

Proliferative epithelial lesions of the urinary bladder in cynomolgus monkeys (Macaca fascicularis) infected with Schistosoma intercalatum.

Five cynomolgus monkeys (Macaca fascicularis) were infected with Schistosoma intercalatum, a helminth that is morphologically similar to Schistosoma haematobium. Infections were readily established and remained active until the monkeys were sacrificed 21 to 84 weeks after exposure. Although the schistosomes were located predominantly in mesenteric and hepatic portal venules, schistosome eggs were found in the bladders of 3 monkeys. Nodules of atypical epithelial cells interpreted as superficially infiltrating undifferentiated bladder carcinomas were found in one monkey 23 weeks after infection

Prognosis in carcinoma of the urachus.

The literature regarding the prognosis of carcinoma of the urachus is reviewed. Factors affecting the prognosis are discussed. En bloc partial cystectomy, if feasible, is the form of treatment which gives the best long-term survival rate. A series of 5 cases is presented with a 13-year survival rate of 40%.

An unusual presentation of acute abdomen: infarcted peritoneal cyst-a probable asbestos-related benign cystic mesothelioma.

This is a report of a rare case of an infarcted pelvic intra-abdominal cyst, having no mesenteric connection presenting as an acute abdomen. The patient had significant asbestos exposure. The cyst was treated successfully by surgical excision. Histopathology showed an infarcted cyst; the lining was destroyed, precluding marker studies. A diagnosis of benign cystic peritoneal mesothelioma (BCPM) was made by excluding other causes of solitary pelvic intra-abdominal cysts. BCPM has been classified as an asbestos-related neoplasm and is usually seen in the pelvis adjunct to the urinary bladder. On

Operative duration and short term morbidity and mortality following radical cystectomy with urinary diversion.

INTRODUCTION: To elucidate the association between operative duration (OD) and postoperative complications, which has been poorly studied in radical cystectomy. We hypothesize an increase in morbidity in radical cystectomy cases which have a longer OD. MATERIALS AND METHODS: Data from the National Surgical Quality Improvement Program (NSQIP) between the years 2012 and 2018 were reviewed for radical cystectomy with ileal conduit urinary diversion or continent diversion. Total operative time was divided into deciles and stratified comparisons were made using univariable and multivariable analys

[Stenosis following a direct uretero-ileal anastomosis technique in substitutive enterocystoplasty].

OBJECTIVES: Stenosis of the uretero-ileal anastomosis and reflux are the commonest causes of secondary deterioration of renal function following enterocystoplasty. Various direct anastomosis and antireflux techniques have been proposed in order to reduce the risk of stenosis and reflux. In this retrospective study, the authors evaluated the risk of stenosis and reflux after right uretero-ileal anastomosis on an invaginated ileal loop. MATERIAL AND METHODS: The authors evaluated the uretero-ileal anastomosis stenosis and reflux rate and problems of the invaginated ileo-ileal valve in 157 patie

Ectopic prostate tissue presenting as a bladder tumor in a young man- a case report.

Ectopic prostate tissue, defined as the aberrant localization of prostate tissue, is often asymptomatic but may occasionally present with various clinical symptoms. A definitive diagnosis is established through surgical excision and subsequent histopathological analysis. This report discussess a case of ectopic prostate tissue identified in the bladder trigone of a 31-year-old patient who presented with lower urinary tract symptoms. Imaging studies revealed a suspicious lesion within the bladder, initially concerning for a potential tumor. Following cystoscopic examination, the lesion was tran

Ureteral calculus complicated by bladder malakoplakia: A case report.

RATIONALE: Bladder malakoplakia is a rare granulomatous inflammatory condition that mimics bladder tumors clinically and radiologically. This case is reported to highlight diagnostic challenges and prevent unnecessary radical surgery, emphasizing the critical role of histopathology in identifying this underrecognized entity. PATIENT CONCERNS: A female patient presented with recurrent urinary frequency persisting for >1 year. No hematuria, dysuria, or flank pain was reported. DIAGNOSES: Diagnoses included bladder tumors, right renal calculus and upper ureteral calculus with hydronephrosis, re

Near-infrared spectroscopy as a novel method of ex vivo bladder cancer tissue characterisation.

OBJECTIVE: To evaluate near-infrared (NIR) spectroscopy in differentiating between benign and malignant bladder pathologies ex vivo immediately after resection, including the grade and stage of malignancy. PATIENTS AND METHODS: A total of 355 spectra were measured on 71 bladder specimens from patients undergoing transurethral resection of bladder tumour (TURBT) between April and August 2022. Scan time was 5 s, undertaken using a portable NIR spectrometer within 10 min from excision. Specimens were then sent for routine histopathological correlation. Machine learning models were applied to the

Evaluation of a New Cystoscopic Cryocatheter and Method for the In Situ Destruction of Bladder Cancer: Preliminary In Vivo Study.

Purpose: To assess the ability to deliver full-thickness bladder wall cryoablation through a cystoscopic approach using a new closed-loop 6F cryocatheter and thermal dose-controlled protocol. Materials and Methods: Evaluations were conducted using a chronic porcine model wherein 10 lesions/animal were created throughout the bladder (bladder wall, trigone region, ureteral orifice, and distal ureter). A 6F cryocatheter was passed through the working channel of a flexible cystoscope. Single 1- and 1.5-minute freeze protocols in a saline environment were evaluated and resultant lesion size was det
