Preservation of native sigmoid colon for secondary continent cystostomy after multivisceral transplantation for chronic intestinal pseudo-obstruction.

Chronic intestinal pseudo-obstruction (CIPO) is characterized by severe digestive +/- urinary dysmotility. If the conservative management fails, multivisceral transplantation (MVT) may be needed. However, urinary dysmotility remains after MVT and requires to continue urinary catheterizations and/or drainage. We report on a boy with severe CIPO complicated by (1) chronic intestinal obstruction requiring total parenteral nutrition, decompression gastrostomy, and ileostomy; (2) recurrent line infections; (3) hepatic fibrosis; and (4) distension of the bladder and upper urinary tract, and recurren…

Urinary catheter use in patients with hip fracture: Are current guidelines appropriate? A retrospective review.

BACKGROUND: Guidelines for urinary catheterization in patients with hip fracture recommend limiting catheter use and using intermittent catheterization preferentially to avoid complications such as urinary tract infection (UTI) and postoperative urinary retention (POUR). We aimed to compare current practices to clinical guidelines, describe the incidence of POUR and UTI, and determine factors that increase the risk of these complications. METHODS: We retrospectively reviewed the charts of patients with hip fracture who presented to a single large tertiary care centre in southeastern Ontario b…

Catheter-associated bacteriuria.

Catheter-associated urinary tract infections are the most common nosocomial infection and a frequent cause of significant morbidity, sepsis, and death. The pathogenesis is multifaceted. Most frequently, bacteria from the urethral meatus ascend to the bladder between the mucosal and catheter surfaces. Alternatively, bacteria may ascend within the drainage system following contamination of the drainage bag or disruption of the catheter tubing junction. The incidence of infection is approximately 5 to 7 per cent for each day of catheterization and closely linked to unalterable host factors such a

Water versus antiseptic periurethral cleansing before catheterization among home care patients: a randomized controlled trial.

BACKGROUND: There is a lack of research studies on periurethral cleansing before catheterization among home care patients. The purpose of this study was to compare the risk of acquiring symptomatic urinary tract infections through the conventional practice of using 0.05% chlorhexidine gluconate (CHG) versus sterile water for periurethral cleansing before insertion of an indwelling urinary catheter. METHODS: A randomized controlled trial was used, and subjects were randomly allocated to either the sterile water group or the 0.05% CHG group. Urine specimens for culture were collected 4 times fo

[Study of incidence and risk factors of nosocomial urinary tract infection in patients with indwelling urinary catheter in intensive care units].

A prospective study was carried out in a medical and surgical ICU to determine the incidence of nosocomial urinary tract infection (NUTI) and to identify the most important risk factors. Over a 6 month period, 180 patients were included. All had an indwelling catheter. Six risk factors were studied: age, sex, illness (medical, surgical, trauma), hospital or extra-hospital origin, simplified acute physiology score and length of bladder catheterization. Forty three patients developed a NUTI. Length of bladder catheterization was the only significant different risk factor in infected and non-infe

New tubeless video-assisted thoracoscopic surgery for small pulmonary nodules.

OBJECTIVES: Problems associated with intubation, chest drainage and urinary catheterization can have a negative impact on patient's recovery after thoracic surgery. We therefore evaluated the feasibility of a new tubeless (spontaneous ventilation without tracheal intubation, urinary catheterization, and no post-operative chest drain placement) approach to perform video-assisted thoracoscopic surgery (VATS) for small pulmonary nodules (SPN) less than 2cm in diameter. METHODS: From 1 January 2012 to 31 December 2014, 34 patients with SPNs were treated using tubeless VATS in our centre. To be el…

A standardized educational Intervention to enhance acceptance and adherence to self-intermittent catheterization: Protocol for a multicenter randomized controlled trial.

Intermittent catheterization (IC) is the gold-standard management for individuals with non-obstructed urinary retention and neurogenic bladder. Learning IC is physically and psychologically challenging, and several men express fear of practicing IC because of the risk of urethral trauma and urinary infections. This study aims to evaluate the effectiveness of an enhanced educational intervention in improving IC acceptance and adherence compared to standard clinical practice training. This multicenter, parallel-group, randomized controlled trial will enroll 128 male patients, randomly assigned t

Laparoscopic continent cutaneous urinary diversion using a modified Yang-Monti technique in an adult: A case report including 5-year follow-up.

Continuous cutaneous urinary diversion is challenging when the appendix is physically unavailable. The Yang-Monti channel is an alternative to the tunneled appendix for urinary diversion. We present a case involving a 49-year-old man who underwent total urethrectomy and cystostomy 10 months previously. No tumor recurrence was observed; however, the patient experienced severe catheter-related bladder irritation after the procedure. The patient was readmitted to the authors' hospital and underwent laparoscopic continent cutaneous urinary diversion using extracorporeal construction of a modified

Discovery of a polymer resistant to bacterial biofilm, swarming, and encrustation.

Innovative approaches to prevent catheter-associated urinary tract infections (CAUTIs) are urgently required. Here, we describe the discovery of an acrylate copolymer capable of resisting single- and multispecies bacterial biofilm formation, swarming, encrustation, and host protein deposition, which are major challenges associated with preventing CAUTIs. After screening ~400 acrylate polymers, poly(tert-butyl cyclohexyl acrylate) was selected for its biofilm- and encrustation-resistant properties. When combined with the swarming inhibitory poly(2-hydroxy-3-phenoxypropyl acrylate), the copolyme

[IIA2-Y-type urethral duplication].

Urethral duplications are extremely rare congenital malformations. The most used classification is that of Effmann and Lebowitz, describing 6 types of urethral duplications. The under type IIA2-Y is one of the most rare duplications corresponding to duplicated urethra originating from the bladder neck and extending toward the ectopic perineal or anal insertion. We here report the case of a 32 year patient, with no previous personal history, suffering from urinary leakage during and after urination since childhood. Clinical examination showed a bottleneck at the level to the perineum initially

Perforation of the augmented bladder in patients undergoing clean intermittent catheterization.

During urological reconstructive surgery small or large bowel is used to increase bladder capacity or to create a urinary reservoir. In most patients clean intermittent catheterization is necessary for urinary drainage. We report on 4 patients with a sigmoid cystoplasty who perform clean intermittent catheterization and who have experienced a serious long-term complication, urinary reservoir perforation, which was fatal in 1. Two were adolescent girls with myelodysplasia and 2 were prepubertal boys with bladder exstrophy. One patient experienced 2 separate reservoir perforations. Rupture occur

Bladder augmentation using the stomach in spinal cord injured patients with impaired renal function.

A small capacity, poorly compliant neurogenic bladder is a difficult problem in spinal cord injured patients. Bladder augmentation with intermittent catheterization offers an alternative to indwelling catheterization. Ileum, cecum, and sigmoid colon have been most commonly used for enterocystoplasty but may, however, cause hyperchloremic metabolic acidosis in patients with impaired renal function. This can be prevented by performing a gastric augmentation. We present two cases of gastrocystoplasty in spinal cord injured patients with a small capacity, poorly compliant neurogenic bladder, impai
