Patients who inquire about microsurgical correction of vasal and epididymal obstructions should be informed about the postoperative rates of both return of sperm to the semen and pregnancy. They also should be informed about the possibility of a successful outcome, depending on their individual circumstances. The various preoperative, intraoperative, and postoperative factors that influence success rates are discussed in this article.

The possibility of aspirating sperm from the first segments of the seminal tract by microsurgical techniques has permitted in vitro fertilization of the oocytes of the partners of men with obstructive azoospermia and ejaculation disorders. To perform AIH, an artificial spermatocele was inserted into the epididymis for obstructive azoospermia (probably caused by congenital defect of the spermiduct on one side and by accidental vasosection in hernioplasty on the other). A male pseudohermaphrodite white-tailed deer (Odocoileus virginianus) is described. The animal shot in Aiken County, South Carolina on November 4, 1971, was 1.5 years old and had the external appearance of an antlered doe. The 6 point antlers were in velvet, a vulva was present, and the testes were located in the body cavity. Other male reproductive organs present were epididymides, vasa deferentia and ampullae. Female reproductive organs present were cervix, vagina and clitoris.

This case report describes successful IVF of oocytes of a 22-year-old female with epididymal spermatozoa aspirated from alloplastic spermatocele of the husband presenting with bilateral congenital absence of the vas deferens. Motile spermatozoa were aspirated from the reservoir 3 months after implantation. Abortion occurred 22 days after embryo replacement.

Different techniques for reanastomosing the dissected vas deferens with and without an intraluminal splint were investigated in a rat model. The operative results were established by mating with fertile females. Statistically there was no significant difference in the frequency of pregnancy, although the splintless technique seems to have been more successful.

The extremely rare occurrence of genital tuberculosis affecting the penis, seminal vesicles, epididymides, vas deferens and Cowper's gland is described. Diagnosis was made by demonstration of fragmented acid fast bacilli in the discharge, and caseating epithelioid cell granulomata with Langhan's giant cells detected on histopathology. Response to antitubercular drugs was prompt.
