Computer generated three-dimensional reconstruction of the bony labyrinth in Mondini's dysplasia.

The bony labyrinth obtained at necropsy in four cases was studied by a new computer-generated three-dimensional (3-D) system. One case was normal (control) and the other three were histopathologically confirmed cases of Mondini's dysplasia. In case 1, the cochlea had only 2 turns and the lateral semicircular canal did not make a circle but appeared as a spherical mass projecting from the utricle even though the posterior semicircular canal made a normal circle. In case 2, there were no turns in the cochlea even though the semicircular canals and the vestibule appeared normal. In case 3, the co

Vestibular peripheral function remains robust after two weeks of continuous ionic direct current stimulation.

Objective.Conventional neural prostheses use brief charge-balanced pulses to minimize the risk of electrode polarization and irreversible electrochemistry at the electrode-electrolyte interface, constraining the waveforms that can be delivered long-term. Direct current (DC) stimulation has shown advantages, such as direct excitation and inhibition of tissue, but priorin vivoreports have been largely limited to acute durations. This study explores the physiological and histological effects of prolonged DC stimulation at amplitudes effective for neuromodulation while isolating tissue from the el

Altered central vestibular processing in Parkinson's disease with Pisa syndrome.

In Parkinson's disease (PD) dizziness, imbalance, postural instability and impaired visuospatial orientation occur, about 10% of PD additionally present with Pisa syndrome (PS). Aim of the current study was (a) to evaluate the perception of verticality by subjective visual vertical (SVV) and visuospatial orientation by the Benton Judgement of Line Test (JLO) in PS, (b) to analyze vestibular reactivity after vestibular stimulation and (c) to compare isolated cervical dystonia (iCD) before and after botulinum neurotoxin (BoNT) treatment. Participants consisted of 20 PD patients (8 with PS), 32 i

A modeling approach to understanding poor stability in people with vestibular hypofunction.

People with vestibular hypofunction (PwVH) have poor walking stability, characterized by increased trunk sway, increased spatiotemporal variability, and reduced margins of stability. PwVH may have poor walking stability because of the vestibular loss itself or because they adopt cautious gait patterns to compensate for the vestibular loss (e.g., slower walking with wider and shorter steps). This study used a recently developed model of human walking stability to explore the cause of poor walking stability in PwVH. Results showed that incorporating decreased vestibular sensitivity in the model

Using Galvanic Vestibular Stimulation to Null and Enhance Real-World Motion Perception.

INTRODUCTION: Directional galvanic vestibular stimulation (GVS) influences the vestibular neural firing rate to provide sensations of yaw, pitch, or roll in the absence of real motion. However, there is also great interest in using GVS to actively modulate motion perception during physical movement in dynamic, real-world contexts. The purpose of this study is to evaluate the effectiveness of GVS in either nulling or enhancing full-body yaw and pitch perceptions of a subject seated upright in a rotating chair. METHODS: Using a randomized, counter-balanced design, 20 subjects completed yaw-righ

Quantifying postural responses to galvanic vestibular stimulation for a four-electrode montage.

Standing balance is maintained through the integration of signals from the proprioceptive, visual, and vestibular systems. Galvanic vestibular stimulation (GVS) delivered through head-worn electrodes is increasingly used in clinical vestibular rehabilitation and immersive entertainment interfaces, but it is known to impact on-feet postural stability. Prior studies assessing balance and postural effects have predominantly focused on bilateral mastoid-based uniaxial GVS paradigms. Limited research has characterized balance metrics under multiaxial GVS configurations using four-electrode montages

Galvanic vestibular stimulation activates the parietal and temporal cortex in humans: A functional near-infrared spectroscopy (fNIRS) study.

Galvanic vestibular stimulation (GVS) helps stabilize subjects when balance and posture are compromised. This work aimed to define the cortical regions that GVS activates in normal subjects. We used functional near-infrared spectroscopy (fNIRS) to test the hypothesis that GVS activates similar cortical areas as a passive movement. We used transcranial current stimulation (cathode in the right mastoid process and anode in the FPz frontopolar point) of bipolar direct current (2 mA), false GVS (sham), vibration (neutral stimulus), and back and forth motion (positive control of vestibular movement

Efficacy of the subjective visual vertical test performed using a mobile application to detect vestibular pathology.

BACKGROUND: The subjective visual vertical (SVV) test is a sensitive test of vestibular dysfunction that allows the evaluation of otolithic organs; however, with the current method, there are technical and logistical limitations that make the application of this test difficult in the conventional clinic. OBJECTIVE: The objective of this study is to assess the effectiveness of detecting vestibular pathology using the SVV via a new screening method. METHODS: A consecutive sample of 62 patients with suspected vestibular pathology was included in the study. The patients were clinically diagnosed

Optimizing VOMS for identifying acute concussion in collegiate athletes: Findings from the NCAA-DoD CARE consortium.

The Vestibular/Ocular-Motor Screening (VOMS), an important component in acute (<72 h) sport-related concussion (SRC) assessment, is increasingly used alongside the Sport Concussion Assessment Tool (SCAT) and as part of the Military Acute Concussion Evaluation 2 (MACE2). VOMS demonstrates clinically useful diagnostic accuracy for acute SRC and improves the overall utility when added to the SCAT3. However, potential overlap among VOMS's vestibular and oculomotor items suggests the possibility of a more efficient version. VOMS and SCAT3 scores were analyzed for 3,958 preseason (47.8% female) and

The human brain networks mediating the vestibular sensation of self-motion.

Vestibular Agnosia - where peripheral vestibular activation triggers the usual reflex nystagmus response but with attenuated or no self-motion perception - is found in brain disease with disrupted cortical network functioning, e.g. traumatic brain injury (TBI) or neurodegeneration (Parkinson's Disease). Patients with acute focal hemispheric lesions (e.g. stroke) do not manifest vestibular agnosia. Thus, brain network mapping techniques, e.g. resting state functional MRI (rsfMRI), are needed to interrogate functional brain networks mediating vestibular agnosia. Hence, we prospectively recruited

CACHD1-deficient mice exhibit hearing and balance deficits associated with a disruption of calcium homeostasis in the inner ear.

CACHD1 recently was shown to be an α2δ-like subunit that can modulate the activity of some types of voltage-gated calcium channels, including the low-voltage activated, T-type CaV3 channels. CACHD1 is widely expressed in the central nervous system but its biological functions and relationship to disease states are unknown. Here, we report that mice with deleterious Cachd1 mutations are hearing impaired and have balance defects, demonstrating that CACHD1 is functionally important in the peripheral auditory and vestibular organs of the inner ear. The vestibular dysfunction of Cachd1 mutant mice,

Vestibular evoked myogenic potentials.

Vestibular Evoked Myogenic Potentials (VEMP) are commonly recorded in patients experiencing vertigo or chronic instability. This test evaluates the patient's otolith function and is often combined with both Videonystagmography and Video Head Impulse Test. VEMP is a simple, reproducible test, in the absence of any pre-existing conductive hearing loss. Cervical VEMP explore both saccular function and the inferior vestibular nerve, whereas ocular VEMP assess utricular function and the superior vestibular nerve. In combination with previously described tests, VEMP allows characterization of vertig
